COURSE TITLE Capacity Development for Care Coordinators
SYNOPSIS Care coordination is evaluating Patient's needs and resources to determine
appropriate services with available funding sources and monitoring care provided
over an extended period of time.
Care coordinators arrange for and authorize services, control payment for those
services, improve availability and quality of services, contain costs by ensuring
appropriate use of services, justify expenditure of funds and ensure that only
appropriate enrolees utilize Health service.
Care Coordinators also conduct evaluation of Patient Outcomes, Connection To
Services, Adherence To Medication, Service Utilization and Cost of Service.
Care coordinators should also monitor Patient Satisfaction and provide appropriate,
comprehensive, and coordinated response to Patient's needs.
Care Coordination should focus on prevention as well as rehabilitation and Health
maintenance
