resources moved to the community
• Testing a range of services including:
– Open Access Memory Clinics
– Improved post diagnostic support
– Care Home Liaison Service
– Intensive community support in a crisis
– Comprehensive case planning
– Hospital inpatient liaison and transitional care

Mar 12 with 10-15 individuals
• Single health and social care multidisciplinary
co-located dementia team which forms one stop
service from diagnosis to end of life
• Post diagnostic support team – LAC/AS/SW
• Comprehensive skills audit and training
programme
