2. Our overall aim is to demonstrate
Better Outcomes
for
More People
for
Same or Less Resource
3. How will we achieve this aim?
QuickTime™ and a
decompressor
are needed to see this picture.
The Road to Transformation
Gareth Morgan
4. Why we think this will work
• Because we use more resource by doing
the wrong things or doing the right things
too late
• We use resources un-necessarily by
providing interventions people don’t want
or need
5. Who are the sites?
Perth and Kinross CHP
North Lanarkshire CHP
Midlothian CHP
6. How are they going about making the changes?
Process mapping, feedback from
Understand how well current people with dementia and carers,
system/processes work focus groups, SWOT analysis,
comparative data etc
eg Integrated Dementia Service; Integrated
Day Services; Family Group Conferencing;
Design a Better System
Comprehensive Case planning; post diagnostic
support teams etc
Mix of small scale tests of change and
Test/Implement the Change
larger scale pilots
Set of measures to evaluate impact
Evaluate the Change Integrated Resource Framework to
look at financial changes
8. Perth and Kinross CHP

resources moved to the community
• Testing a range of services including:
– Open Access Memory Clinics
– Improved post diagnostic support
– Care Home Liaison Service

12. Key learning to date…
• Housing
• Interagency working – including third sector
• Value of data – qualitative and quantitative
• Cross site peer support
• Some dedicated time to lead the work pays back
ten-fold
• Integrated Resource Framework
• Closer working nationally
• Integration with wider redesign work
13. We are also sharing the learning…
2. Between the sites
3. To inform national policy
4. Between national programmes
5. To other services across Scotland
• Launching web-ex sessions – 13th & 27th
July 11am
• Alzheimer Scotland Newsletter
• Community of Practice

of-practice/dementia-demonstrator-sites.aspx
14. In summary Dementia Demonstrators are
demonstrating
better-more-less
by
redesigning pathways
across whole system
and
sharing
outcomes and learning widely
