SECTION - 2 Philosophy & Purpose
EPCL believes that any individual responsible for emergency response should have adequate
knowledge about the hazards involved and his role statement. To standardize the level of
knowledge required for a SBM there should be minimum criteria all across EPCL.
2.2 Purpose
A SBM is required where there is a confined space hazard or high probability of electrocution .To
help protect a worker inside a confine space, many stake holders consider it a best practice to have
a stand by man the outside. OSHA advises these workers to always maintain contact through either
visual, or radio means. This monitoring allows the attendant to order the worker in the confined
space to evacuate and to contact emergency personnel to help with the rescue if necessary.
Note-1: According to OSHA about 90% deaths involving confined spaces occur every year.
Note-2: One study considered 40% of confine space fatalities involve attempted rescuers.

SECTION - 3 Confined Space?
Any work space that has a limited means of access or egress and is not intended for regular
human occupancy and has one or more of the following characteristics:
 Is large enough and so configured that an employee can enter and perform assigned work.
 Has limited or restricted means for entry or exit.
 Is not designed for continuous employee occupancy.
 No proper ventilation
 Contains or has the potential to contain a hazardous atmosphere.
 Contains a material that has the potential for engulfing an entrant.
 Has an internal configuration that might cause an entrant to be trapped or asphyxiated by
inwardly converging walls or by a floor that slopes downward and tapers to a smaller cross
section.
 Pits, dikes, ditches or excavations more than 04 feet deep or pits greater than 04 feet deep
that requires personnel to work with their heads below the rim.
 Contains any other recognized serious safety or health hazards for example;
O Process Vessels
o Boiler Feed Water Tanks
o Gas valve pit
o Waste Water pit
o GT’s Suction / Discharge plenum
o Internals of most Cooling Towers
o Effluent Treatment Tanks or Ponds
o Septic Tanks, Sewerage gutter
Entry requires special precautions, including a qualified and authorized Standby Person, and a
special authorization (Entry Permit)
3.1 Non permit Confined Space (non- permit space)
Means a confined space that does not contain any hazard itself or exposed to atmospheric
hazard which can cause death or serious physical harm. Provided safe means of entry / egress
from the confined space have been provided.
Unit Manager Operation, in consultation with HSE department, if required, shall decide
whether the confined space is non-work permit required or not.
Confined Space Entry permit can be exempted for excavated trenches of more than4 feet with
following arrangements:

1. Free from Slush & water.
2. Emergency exit route is free from tripping hazard (due to iron cage, demolished piles,
improper sloping etc.) and firm footing should be ensured at areas of entry / exit (access
and egress).
3. Proper means of entry / exit (ramps, runway or ladder) are available at every 75-ft or as
recommended by EPCL safety interface. These should be provided at least at two locations
in a pit.
4. Proper lighting arrangements should be provided at work area and points of entry /exit,
not less than 54-Lux.
3.2 Electrical Standby Person
On assessment of all risks involved in undertaking work associated with either lives (energized) or
De-energized equipment must consider all factors which may have the potential to cause injury or
damage, If this potential exists or any work in close proximity to live electrical equipment. EPCL
Electrical Engineer / Supervisor will depute a standby electrical person for precautionary measures.
3.3 Hazards of Confined Spaces
 Oxygen Deficient Atmospheres
 Oxygen Enriched Atmospheres
 Flammable Atmospheres
 Toxic Atmospheres
 Temperature Extremes
 Electrical hazards
 Noise, Wet Surfaces, Falling Objects

SECTION - 4 Role of SBM
4.1 Confined space SBM role
 Know potential hazards; beware of the effect of hazards on entrants and how to control.
 Evaluate any changes of internal conditions as a result of work being performed within the
Confined space (smoke accumulation, oxygen deficiency during hot job inside).
 Know how to use entry & rescue equipment.
 Be familiar with the location of safety shower, exits, emergency siren, and the contingency
plan.
 Understand the rescue plan developed for that specific confined space
 Be physically capable of performing the duties assigned. The standby person is
responsible for stopping the job and ensuring personnel’s exit from the confined space if
emergency siren is sounded or any unsafe environment at the outside of confine space.
 Remain at the entry point as long as anyone is inside the confined space and remain alert
for anything that could affect the employee’s safety (e.g., fumes and noise). Keep
restricting entry of unauthorized persons. Perform rescue that does not require entry into
confined space
 Have a safety harness, plus a lifeline, a whistle (in case of work at heights or unavailability
of walkie-talkie / intercom/ whistle) and either a SCBA or fresh-air mask.
 Avoid the activities that interfere with primary role of attendant.
 Know / maintain a separate list of all those who are inside the confined space at all times
which must be placed on the entry/ exit point of the confined space.
 Frequently confirm that the personnel inside the confined space are all right. Alert and
inform them about any hazard. Ensure air hoses / exhaust pipes and life lines are intact
and remain in good shape. During an emergency situation (gas release /fire, sounding of
emergency siren), require them to exit the confined space and then discontinue use of
plant air.
 Assist in safe handling of materials and tools and in conveying messages.
 Use the walkie-talkie / intercom / whistle to call help in case of emergency. Communicate
with Control Room to sound the siren in case emergency warrants so. The standby person
must not enter the confined space, even in an emergency, until another qualified standby
person arrives and unless he has been trained as a rescuer.
 The standby person must not allow anyone to enter the confined space to start a rescue
unless he is part of the emergency squad / designated member of rescue team and is
wearing the necessary personal protective equipment. Also mention then name of rescue

workers whosoever in the list of confined space entrant personnel on the back of
Confined space entry permit.
 Should have passed the standby person test & interview, Site management should
maintain up-to-date records of individuals qualified as standby person. The list of
qualified personnel should be displayed in the Control Room.
 Each standby person should get refresher training at an interval of no more than three
years from the previous training.
 Standby person should be;
o Permanent employee.
o Trade Apprentice/ TAE at least having six months experience at plant.
o Qualified contract person only (approved by Safety Section).
 Ensure that Vessel Entry Work Permit has been taken and all requirements of the permit have
been met.
 Ensure the full compliance of PPE’s for the Vessel Entrants especially the Safety Harness,
Lifelines, Supplied Air Masks.
 Knows the hazards that may be faced during entry.
 Ensure the Equipment isolation diagram is pasted at the entry point and physically check the
positive isolations.
 Remains outside the confined space during entry operations until relieved by another
attendant.
 Witness the checking of O2/LEL as per frequency.
4.2 Electrical SBM Role
Where a person, equipment or mobile plant may inadvertently contact live electrical equipment,
such as bare or overhead wires, a standby person must be posted. The standby person’s duty is as
follows:
 Know / beware all risks involved in undertaking work associated with either lives
(energized) or de-energized equipment must consider all factors which may have the
potential to cause injury or damage and how to control.
 Be familiar with the location of isolation breaker of affected circuit, exits, emergency siren,
and the contingency plan.

 In case of emergency (electrical shock to person working on / near live circuit) the Electrical
standby person has an authority to put off / de-energize the upstream breaker for life
saving.
 Be physically capable of performing the duties assigned. The standby person is responsible
for stopping the job, and ensuring personnel’s exit from the job if Level-I, II Emergency
siren is sounded.
 Use the walkie-talkie / intercom / whistle to summon help in case of emergency before
attempting rescue when an emergency develops.
 Can describe / execute the emergency rescue procedures (First Aid) for electrical shock
incidents, cardiopulmonary resuscitation (CPR) and (AED’s) automated external
defibrillators (if available).
 Should be authorized by Electrical Unit Manager, Electrical Unit Manager should maintain
up-to-date records of individuals qualified as Electrical standby person. The list of
authorized personnel should be displayed in Electrical Unit Manager Room & I&E
workshop.
 Each standby person should get refresher training at an interval of no more than three
years from the previous training.
 Standby person should be;
• Permanent employee.
• Electrical Trade Apprentice at least having six months experience at plant.
• Qualified contract person only (authorized by Electrical Unit Manager).
Note: All SBM’s should be trained and certified by HSE as per request. Approved list of SBM’s to
be maintained by HSE & Electrical Section Head. Refresher training to be provided on yearly basis
for TA/TAE’s and on six monthly basis for contractor.

SECTION - 5 SBM Certification & Recordkeeping
The EPCL provide training so that all employees whose work is related to confine space acquire the
understanding, knowledge and skills necessary for the safe performance of their duties. The aim of
training in confined spaces is to remove or minimize risks in the workplace associated with
confined spaces
Training will be provided for every staff member, including stand-by persons before the employee
is assigned to duties in a confined space or electrical job.
HSE should allow the SBM to perform this work on the basis of qualified /trained for this job so it is
mandatory to complete the SBM test & interview prior to performing confined entry work.
It is necessary to pass the SBM test with 100% marks and after successfully completion of test, SBM
must be qualify the interview for stand by man.
Upon successfully completing the interview, HSE Engineer/ Officer will issue a Certificate to each
SBM. In case, SBM is not a permanent work force at EPCL this certificate will be valid for any revisit
within 6 months.
5.1 Procedure for record keeping (Confined space SBM)
HSE should maintain up-to-date records of individuals qualified as standby person. The list of
qualified personnel should be displayed in the Control Room. Each standby person should get
refresher training at an interval of no more than three years from the previous training.
5.2 Procedure for record keeping (Electrical SBM)
Electrical Unit Manager/ HSE Advisor should maintain up-to-date records of individuals qualified as
Electrical standby person. The list of authorized personnel should be displayed in Electrical Unit
Manager Room & I&E workshop.

Prepared by: MFB -2014 Department: HSE Unit / Section: Safety Area : Plant Site SECTION - 2 Philosophy & Purpose EPCL believes that any individual responsible for emergency response should have adequate knowledge about the hazards involved and his role statement. To standardize the level of knowledge required for a SBM there should be minimum criteria all across EPCL. 2.2 Purpose A SBM is required where there is a confined space hazard or high probability of electrocution .To help protect a worker inside a confine space, many stake holders consider it a best practice to have a stand by man the outside. OSHA advises these workers to always maintain contact through either visual, or radio means. This monitoring allows the attendant to order the worker in the confined space to evacuate and to contact emergency personnel to help with the rescue if necessary. Note-1: According to OSHA about 90% deaths involving confined spaces occur every year. Note-2: One study considered 40% of confine space fatalities involve attempted rescuers.

Any work space that has a limited means of access or egress and is not intended for regular human occupancy and has one or more of the following characteristics:  Is large enough and so configured that an employee can enter and perform assigned work.  Has limited or restricted means for entry or exit.  Is not designed for continuous employee occupancy.  No proper ventilation  Contains or has the potential to contain a hazardous atmosphere.  Contains a material that has the potential for engulfing an entrant.  Has an internal configuration that might cause an entrant to be trapped or asphyxiated by inwardly converging walls or by a floor that slopes downward and tapers to a smaller cross section.  Pits, dikes, ditches or excavations more than 04 feet deep or pits greater than 04 feet deep that requires personnel to work with their heads below the rim.  Contains any other recognized serious safety or health hazards for example; O Process Vessels o Boiler Feed Water Tanks o Gas valve pit o Waste Water pit o GT’s Suction / Discharge plenum o Internals of most Cooling Towers o Effluent Treatment Tanks or Ponds o Septic Tanks, Sewerage gutter Entry requires special precautions, including a qualified and authorized Standby Person, and a special authorization (Entry Permit) 3.1 Non permit Confined Space (non- permit space) Means a confined space that does not contain any hazard itself or exposed to atmospheric hazard which can cause death or serious physical harm. Provided safe means of entry / egress from the confined space have been provided. Unit Manager Operation, in consultation with HSE department, if required, shall decide whether the confined space is non-work permit required or not. Confined Space Entry permit can be exempted for excavated trenches of more than4 feet with following arrangements:

Emergency exit route is free from tripping hazard (due to iron cage, demolished piles, improper sloping etc.) and firm footing should be ensured at areas of entry / exit (access and egress). 3. Proper means of entry / exit (ramps, runway or ladder) are available at every 75-ft or as recommended by EPCL safety interface. These should be provided at least at two locations in a pit. 4. Proper lighting arrangements should be provided at work area and points of entry /exit, not less than 54-Lux. 3.2 Electrical Standby Person On assessment of all risks involved in undertaking work associated with either lives (energized) or De-energized equipment must consider all factors which may have the potential to cause injury or damage, If this potential exists or any work in close proximity to live electrical equipment. EPCL Electrical Engineer / Supervisor will depute a standby electrical person for precautionary measures. 3.3 Hazards of Confined Spaces  Oxygen Deficient Atmospheres  Oxygen Enriched Atmospheres  Flammable Atmospheres  Toxic Atmospheres  Temperature Extremes  Electrical hazards  Noise, Wet Surfaces, Falling Objects

Prepared by: MFB -2014 Department: HSE Unit / Section: Safety Area : Plant Site SECTION - 4 Role of SBM 4.1 Confined space SBM role  Know potential hazards; beware of the effect of hazards on entrants and how to control.  Evaluate any changes of internal conditions as a result of work being performed within the Confined space (smoke accumulation, oxygen deficiency during hot job inside).  Know how to use entry & rescue equipment.  Be familiar with the location of safety shower, exits, emergency siren, and the contingency plan.  Understand the rescue plan developed for that specific confined space  Be physically capable of performing the duties assigned. The standby person is responsible for stopping the job and ensuring personnel’s exit from the confined space if emergency siren is sounded or any unsafe environment at the outside of confine space.  Remain at the entry point as long as anyone is inside the confined space and remain alert for anything that could affect the employee’s safety (e.g., fumes and noise). Keep restricting entry of unauthorized persons. Perform rescue that does not require entry into confined space  Have a safety harness, plus a lifeline, a whistle (in case of work at heights or unavailability of walkie-talkie / intercom/ whistle) and either a SCBA or fresh-air mask.  Avoid the activities that interfere with primary role of attendant.  Know / maintain a separate list of all those who are inside the confined space at all times which must be placed on the entry/ exit point of the confined space.  Frequently confirm that the personnel inside the confined space are all right. Alert and inform them about any hazard. Ensure air hoses / exhaust pipes and life lines are intact and remain in good shape. During an emergency situation (gas release /fire, sounding of emergency siren), require them to exit the confined space and then discontinue use of plant air.  Assist in safe handling of materials and tools and in conveying messages.  Use the walkie-talkie / intercom / whistle to call help in case of emergency. Communicate with Control Room to sound the siren in case emergency warrants so. The standby person must not enter the confined space, even in an emergency, until another qualified standby person arrives and unless he has been trained as a rescuer.  The standby person must not allow anyone to enter the confined space to start a rescue unless he is part of the emergency squad / designated member of rescue team and is wearing the necessary personal protective equipment. Prepared by: MFB -2014 Department: HSE Unit / Section: Safety Area : Plant Site workers whosoever in the list of confined space entrant personnel on the back of Confined space entry permit.  Should have passed the standby person test & interview, Site management should maintain up-to-date records of individuals qualified as standby person. The list of qualified personnel should be displayed in the Control Room.  Each standby person should get refresher training at an interval of no more than three years from the previous training.  Standby person should be; o Permanent employee. o Trade Apprentice/ TAE at least having six months experience at plant. o Qualified contract person only (approved by Safety Section).  Ensure that Vessel Entry Work Permit has been taken and all requirements of the permit have been met.  Ensure the full compliance of PPE’s for the Vessel Entrants especially the Safety Harness, Lifelines, Supplied Air Masks.  Knows the hazards that may be faced during entry.  Ensure the Equipment isolation diagram is pasted at the entry point and physically check the positive isolations.  Remains outside the confined space during entry operations until relieved by another attendant.  Witness the checking of O2/LEL as per frequency. 4.2 Electrical SBM Role Where a person, equipment or mobile plant may inadvertently contact live electrical equipment, such as bare or overhead wires, a standby person must be posted. The standby person’s duty is as follows:  Know / beware all risks involved in undertaking work associated with either lives (energized) or de-energized equipment must consider all factors which may have the potential to cause injury or damage and how to control.  Be familiar with the location of isolation breaker of affected circuit, exits, emergency siren, and the contingency plan.

Prepared by: MFB -2014 Department: HSE Unit / Section: Safety Area : Plant Site  In case of emergency (electrical shock to person working on / near live circuit) the Electrical standby person has an authority to put off / de-energize the upstream breaker for life saving.  Be physically capable of performing the duties assigned. The standby person is responsible for stopping the job, and ensuring personnel’s exit from the job if Level-I, II Emergency siren is sounded.  Use the walkie-talkie / intercom / whistle to summon help in case of emergency before attempting rescue when an emergency develops.  Can describe / execute the emergency rescue procedures (First Aid) for electrical shock incidents, cardiopulmonary resuscitation (CPR) and (AED’s) automated external defibrillators (if available).  Should be authorized by Electrical Unit Manager, Electrical Unit Manager should maintain up-to-date records of individuals qualified as Electrical standby person. The list of authorized personnel should be displayed in Electrical Unit Manager Room & I&E workshop.  Each standby person should get refresher training at an interval of no more than three years from the previous training.  Standby person should be; • Permanent employee. • Electrical Trade Apprentice at least having six months experience at plant. • Qualified contract person only (authorized by Electrical Unit Manager). Note: All SBM’s should be trained and certified by HSE as per request. Approved list of SBM’s to be maintained by HSE & Electrical Section Head. Refresher training to be provided on yearly basis for TA/TAE’s and on six monthly basis for contractor.

Prepared by: MFB -2014 Department: HSE Unit / Section: Safety Area : Plant Site SECTION - 5 SBM Certification & Recordkeeping The EPCL provide training so that all employees whose work is related to confine space acquire the understanding, knowledge and skills necessary for the safe performance of their duties. The aim of training in confined spaces is to remove or minimize risks in the workplace associated with confined spaces Training will be provided for every staff member, including stand-by persons before the employee is assigned to duties in a confined space or electrical job. HSE should allow the SBM to perform this work on the basis of qualified /trained for this job so it is mandatory to complete the SBM test & interview prior to performing confined entry work. It is necessary to pass the SBM test with 100% marks and after successfully completion of test, SBM must be qualify the interview for stand by man. Upon successfully completing the interview, HSE Engineer/ Officer will issue a Certificate to each SBM. In case, SBM is not a permanent work force at EPCL this certificate will be valid for any revisit within 6 months. 5.1 Procedure for record keeping (Confined space SBM) HSE should maintain up-to-date records of individuals qualified as standby person. The list of qualified personnel should be displayed in the Control Room. Each standby person should get refresher training at an interval of no more than three years from the previous training. 5.2 Procedure for record keeping (Electrical SBM) Electrical Unit Manager/ HSE Advisor should maintain up-to-date records of individuals qualified as Electrical standby person. The list of authorized personnel should be displayed in Electrical Unit Manager Room & I&E workshop.
