I am Cathy Radke and I am the founder of hazels angels. Our program started in 2012 and with our ongoing
research we believe that our online recreational website will encourage cared for loved ones to receive important
services closer to home and offer caregivers opportunities to adapt to change before accepting the role of
caregiving through community services.
We have prepared a proposal for you to help enhance the lifestyle of caregivers and their cared for loved ones.

Caregivers are lacking several important services for adults between the ages of 22-64.
Hazels Angels is a community support group that helps caregivers find local support
that is needed the help them care for their loved ones. We have been involved with the
community since 2012.
This is a program that will not just offer revenue in your pocket but also help so many
others along the way.
Brittany, my daughter, is 22 and has an undiagnosed illness that has cost her, her
freedom. She could not receive any services due to an unclear illness that has caused
many disorders, such as seizures. Disorders such as these can be very overwhelming
for the caregiver. Many care givers are forced to leave their job and our economy is
paying for loved ones to stay home and care. The problem is with the overwhelming
baby boom stage and new illnesses many are forced to quit their career for good. After
a stroke myself, I was promised so much support from family and the health care
system, however things went back to normal shortly after I recovered. Many loved ones
would love to help but work is a valuable income to many families. To avoid the guilt of
not having the ability to help, family members will often turn on a caregiver and cause
even more unwanted stress.
Now after four long years, Drs. have given her a diagnosis that allows her to take
advantage of special services she had been denied in the past. Although our journey is
far from over, we at least have hope.
It was in December 2014 when I awoke from an induced coma caused by a stroke that I
quickly realized the importance of caring. The question that haunted me the most was
the day my other daughter was giving me a shower. It was that moment that I felt the
guilt that Britany felt and realized how it felt to need to be cared for and how it felt to
care for by others.
When I first began my struggles with my own illness and with my daughter, Brittany’s,
disability I was feeling strong and appreciated for all my hard work. I was so grateful that
my mother gave me the ability to care and felt a sense of goodness in me. That
changed very quickly once doctors stopped looking for answers. Many family members
started to walk away and began being frustrated, my anxiety was too much to handle. I
began to think caring was a curse.
It was then that I knew I had to do something to help caregivers and our cared for loved
ones.
We, Hazel’s Angels, have designed a social, online recreational site for the community
that we hope to eventually utilize our community centers to bring services close to the
homes of cared for loved ones and are encouraging others to find ways to help their
community.

Proposal
Many of our caregivers are up against a health care system that understands their
frustrations but are limited to providing resources and an updated action plan.
Over the past four years we have helped several members receive important services
that they were not aware of.
Financial opportunities such as CPP Disability are often missed along with other
financial opportunities. Our social site has developed a group of giving and receiving
angels that believe in working together. We share any health information and links to
services. We inspire our members to be creative and offer inspirational links along with
challenges to encourage creativity. Many caregivers have too much time on their hands
but want to offer assistance so they can feel like they are an important part of society
again. We all have our talents and allowing caregivers and their Cared for loved ones
an opportunity to share some of their talents with the community while helping others
inspires them and gives them something to hold on to.
We offer several small business opportunities and can help you get your feet off the
ground. This will enhance our local community and later branch out to others. We are
proud of our accomplishments but only have our feet in shallow waters. We do not want
to dive into such an important responsible role alone. We know you have the ability to
help our business not only grow but raise spirits, unite families and bring back loved
ones careers and homes.
Hazel’s Angels has a well-deserved reputation for quality customer service. However,
faced with changes in distribution systems, economic impacts to transportation and
logistics, and limitations that prevent taking full advantage of improvements in
technology, Hazel’s Angels faces the possibility of decreasing sales revenues due to
customer perceptions of slow delivery and services.
Hazel’s Angels has designed an action plan for our cared for loved ones and caregivers.
Our methods and actions have helped many caregivers connect with important services
like 211, an emergency line that offers help during a crises. We have met and discussed
solutions with our social group to understand the best methods of bring communities
together. Many Doctors and nurses are speaking out and we take full advantage of
understanding the struggles and actions that must be met for a better health care.
The Objective/Expected Results
We have big plans for the future and so much more room to grow. Hazel’s Angels
believes working together will offer continued care and opportunities for caregivers.
Here are our future goals that we want to introduce to our members:
Our self-published book, “I Was a Kid Once Too”, is complete and ready to make
revenue.
We are building members on our website to offer opportunities for volunteering,
asking for help, or to engage with an event that could benefit from their hobby or
talent.
We are continuously sharing our ideas with Jim Watson, our Mayor, and have had
positive feedback.

“If you build it they will come” Imagine an opportunity to live with your loved ones
and could care for your loved one in a community filled with caring loving people. We
believe a home similar to The Ronald McDonald home but fixed for young adults
could lead to a better lifestyle for both the loved ones and the care givers. Services
would be very similar to the home and we can provide the best care for our loved
ones and each other. United, illnesses without prejudice to their mental or physical
disabilities. Our cared for adults between the ages of 22 and 64 are in desperate
need of constant supervision and this can be exhausting 24/7 care for the care
givers.
Free events and contests will encourage our loved ones to get out doors. Just a
simple meet and greet can put so many smiles on our loved ones faces and help
them get over social anxieties.
Do you like to shop online? We have developed a special shopping page that is
administered by my disabled daughter. She has a room full of fun and exciting things
and we invite our members to sell online also. Trade or buy. This is a great example
of how our cared for loved ones can make money while staying in the comfort of
their home.
We have already designed a caregiving action plan and want to offer them to our
community for a low cost. This information can be found on the Hazel’s Angels

We expect our proposed solution to Hazel’s Angels requirements to provide the
following results:
Transportation
Our cared for loved ones typically have many appointments and other places that
they need to be. Transportation can be especially costly with the addition of parking
fees. We believe in carpooling but if a taxi company was willing to do a regular stop
at community centers, and hospitals we could split the cost and save money on
parking.
Cleaning Services
Cleaning services are available, however, many caregivers and cared for loved ones
are lonely and prefer to have the company of others instead of a clean home. If we
can offer not only a clean home but also personal support, then our cared for loved
ones will have the best support, services and the best of both worlds.
Personal Income
Many of our loved ones would love to work however their disabilities prevent them
from doing so.

We are working on a community page where members can volunteer their time and
choose whichever service is best and most suited for them to assist.
Services/examples could include:
Carpooling - Do you have time to drive others?
Cooking - Is this a passion of yours that you have time to help others with?
Cleaning - Do you have exceptional cleaning or organizins skills that you are
willing to share?
Clerical - Do you have skills that could help others fill out paperwork such as
crucial medical forms or help with annual income taxes.
Social Events
Social events are a great way to get out of the house, into the community and start
to connect with your neighbors. It is time to start raising money for marketing and
raise money to introduce new and exciting, free programs that are closer to home.
Our Facebook page is full of talent and many are just anxiously waiting for our
program to lift off so they can share it with others.
Many times caregivers are desperately searching for forms or information on
Government services. We hope to someday be able to have a community to help
with these important services to take away the stress of a caregiver.
Action Items
Pre-order logo items to prepare a shop online where the company can make some
money and help spread awareness.
Hire a professional website designer to add more traffic.
Open a Foundation to secure the funding raised by our members.
Continue with Blogs and newsletters to raise awareness.
Work with our community through free events and getting involved with local media.
Open new service closer to home for our loved ones.
Build our Hazel’s Angels home to allow our caregivers an opportunity to live in a
supportive community with important services to help enhance the loved ones and
support our disabled in a loving, protective community.
Our Community starts right here in Bells Corners but with the right connections we can
work together and build online recreational websites in communities across the country.
We are not too far from our goal but it is imperative that we gain trust and with your
Company we will be more recognized as a trusting partner

The following is a description of our project methods, including how the project will be
developed, a proposed timeline of events, and reasons for why we suggest developing
the project as described.
Technical/Project Approach
To build multiple community websites across Canada and encourage communities
to work together.
Resources
We have connected with several online courses and training program to enhance
our ability to continue going forward. Working close with our Ministry of Health and
contact with Jim Watson has allowed a positive and encouraging impact to others
and has given comfort to several new caregivers just from knowing we understand
and appreciate their hard work. Bringing smiles and laughter back in the community
will in return offer great revenue through their ability to learn new talents and hobbies
and return back to work knowing there loved one is cared for in a safe environment.
Cooking and meals
Hazel’s Angels is always searching for innovative ways to help our cared for loved
ones and offer a better, less stressful life as individuals. Cooking is a struggle for
caregivers on all levels. Often we are up against special diets. When your days get
to busy you forget to take that important roast out for your family. Many times getting
to the grocery store can take hours or be an inconvenience depending on your loved
ones illness. Many who suffer from mental illness refuse to go out and would rather
eat canned food or frozen meals. Some people like to have control of where their
food is being cooked, how it is prepared and what goes in it.
Planning and shopping for that special meal is not always possible however, now
with your partnership, we can offer simple easy meals close to their home for a
reasonable price. In order to meet our project milestones, this material must be
supplied on schedule. We need supply on demand to fill our online shop with
wonderful items made by caregivers and items with our logo.

Financial Benefits
Share profits amongst sponsors. We have a variety of ways to share profit and
would like the opportunity to discuss options with a trusted Partner.
Revenue on the self-published book, “I was a Kid Once Too”. I have written a book
of my past adventures. Most of the profits will be used for free events and better
opportunities within our local centers
Membership profits. Our special pages offer opportunities for other entrepreneurs.
We offer free advice and mentorship to help inspire and enhance their future
careers.
Subscription to online special pages and events calendar with opportunities to pick
up local sponsors and allow others to know what is happening in their community..
This allows our cared for loved ones opportunities to continuously keep in touch with
our health care changes.
We can utilize the many talents and entrepreneurs in local areas by offering free
access to centers in their area; the possibilities are endless. This gets your
entrepreneurs started with their own business.
We have designed a special logo to raise funding and to offer financial opportunities
to our local caregivers by allowing them to sell the merchandise and keep a
percentage of the profit. This could help many care givers, who would benefit from a
financial break.

Qualifications
We are the very first recreational website registered in Ontario.
Wide populations of caregivers are at a disadvantage due to their age and disability.
Our past personal and ongoing history has allowed an open door opportunity for
growth through our online website, our action plan to provide company for our loved
ones and various social avenues to build clientele.
We work with several creative people online in a social caring and educating way.
A similar home already exists for young united illnesses however, it is only present in
Toronto that we are aware of. We would like to see one in Ottawa.
Conclusion
This is a program that will not just offer revenue in your pocket but also help so many
others along the way. We all know the state of our economy. It is crucial with the baby
boom stage and so many young adults not working that we find ways to work together. I
can now confidently say that we no longer have to search for answers. Caregivers are
often left alone in the role but with our program they will never have to be alone again.
My mother gave me all the tools I needed to understand the pain and suffering others
go through. She made sure we heard the word of God through Sunday school and
Church. I was offered important opportunities in our community such as Pioneers and
Girl Guides.
Many caregivers have suffered the same struggles that I have. I hope to change that for
all caregivers.
I was meant to help others and now we have the tools for our answer to helping
caregivers and we are ready to go.
My only set back is we need to take our program outdoors and that is where we know
you can assist us the most with financial aid for marketing and the trust and support of
registered institutions, such as yours.
We are very pleased that you have taken your valuable time to understand the urgent
need for our caregivers and cared for loved ones.
