When will I need it?

 Possibly never

 Before you have an accident
 When you have had an accident:
   Collate your evidence
   Demonstrate duty of care
   Avoid litigation
   Ignorance no excuse

Some Observations
 My findings - after years of conducting ship and company audits
 and reviews:
    SMS is not user friendly. Mostly hard copy documents, not computerized.
    Cumbersome. Too many manuals.
    Burden of paperwork for front line personnel doing the business of the
    company.

    Too many forms to fill out.    If too many, they just do not fill them out.
    Redundancy of forms. Parallel reporting of same information.       Why not allow
    the computer to fill in the similar required forms with field personnel only filling
    in the fields?
    Lack of Feedback
    Black hole submissions. Field personnel feel they are submitting information
    and it goes into a black hole
    One way street back to the company for reporting. The company only
    sends down more for the field personnel to record and submit without additional
    resources.

  • 10. Some Observations  My findings - after years of conducting ship and company audits and reviews:  SMS is not user friendly. Mostly hard copy documents, not computerized.  Cumbersome. Too many manuals.  Burden of paperwork for front line personnel doing the business of the company.  Too many forms to fill out. If too many, they just do not fill them out.  Redundancy of forms. Parallel reporting of same information. Why not allow the computer to fill in the similar required forms with field personnel only filling in the fields?  Lack of Feedback  Black hole submissions. Field personnel feel they are submitting information and it goes into a black hole  One way street back to the company for reporting.
