Este documento describe el caso de una paciente con neutropenia febril y presenta lineamientos sobre el manejo de esta condición. Detalla la evaluación e investigación requerida, incluyendo exámenes físicos minuciosos y múltiples cultivos. También cubre la clasificación de riesgo, el tratamiento empírico con antibióticos de amplio espectro de manera intravenosa e intrahospitalaria para pacientes de alto riesgo, y la necesidad de evaluar la respuesta diariamente.

Lower extremity involvement, with several Fusarium skin lesions of different types and ages. Patient was a 32-year-old woman with relapsed leukemia who had undergone allogeneic bone marrow transplantation and developed disseminated and fatal fusarial infection.
60 year old malnourished female burn patient with Pseudomonas bacteremia and biopsy confirmed ecthyma gangrenosum.
Enterocolitis neutropénica o tiflitis: Esta entidad clínica debe sospecharse en pacientes que
cursan con neutropenia severa, fiebre, intenso dolor abdominal, difuso o localizado en fosa ilíaca derecha, más la presencia de diarrea, con o sin sangre. El evento fisiopatológico cardinal es un compromiso inflamatorio/ séptico de la mucosa del intestino grueso que permite la translocación de microorganismos desde la microbiota del tubodigestivo hacia el torrente sanguíneo. El estudio estos pacientes debe hacerse a través de imágenes abdominales, como ecografía o TAC que ponen en evidencia un engrosamiento de la pared intestinal, y mediante hemocultivos, donde se aíslan, en alrededor de 40% de los casos, microorganismos aeróbicos y anaeróbicos: E. coli, Klebsiella sp, Pseudomonas sp, Streptococcus sp,
Bacillus sp, Enterococcus sp, Clostridium sp y Bacteroides sp58.
TIFLITIS NEUTROPÉNICA.
La tiflitis suele darse en pacientes neutropénicos y suele afectar el ciego, colon ascendente y a veces el íleon terminal. La TC demuestra distensión y engrosamiento de la pared del ciego que puede ser hipodensa por el edema. La grasa pericecal suele estar infiltrada por cambios inflamatorios y puede complicarse con neumatosis, neumoperitoneo y formación de colecciones pericecales.

En diferentes ensayos clínicos randomizados de pacientes con episodios de NF de bajo riesgo se ha observado una eficacia terapéutica semejante al comparar la antibioterapia oral (ciprofloxacina o cefixima) con ceftriaxona (con o sin adición de amikacina), como parte de una terapia secuencial.
Se acepta hoy que el uso de antimicrobianos orales es una alternativa adecuada y segura (A1) En dos ensayos clínicos randomizados doble ciego, se comparó terapia oral con amoxicilina/ ácido clavulánico más ciprofloxacina versus ceftazidima y versus ceftriaxona más amikacina
