A copy of the plan I developed for the Family Health History program during the course of my internship with the South Central AHEC.

1. Family Health History Program Plan
Train Librarians
Have focus group create their own family health history and research conditions on MedlinePlus
Develop librarian training programs, including a test
Identify bilingual librarians and incorporate them into the project
Make sure that bilingual librarians are able to answer questions about the project and MedlinePlus in Spanish
Train in health privacy and brief on HIPPA laws (CITIProgram PBRN)
Develop Promotional
Materials All promotional materials must be in English and Spanish. Materials should target middle-aged women
Design language-appropriate mass emails.
Develop posters
Ask librarians what already existing group would be a good target. Incorporate the project into their regularly scheduled
activities
Bring in Additional
Stakeholders Bring in the schools, as this program is educational and potentially for the whole family
- See if the libraries can advertise via school handouts
- See if some teachers want to get students to do their family health history as an assignment
Notify the doctors, see if some of them want to inform their patients
- Create an asset map using the libraries’ zipcodes
- Send out mass emails to doctors informing them of the project
- To create awareness of the project, and smooth over potential rough patches.
Implementing the
Program
Put up posters and send up mass emails
Hold informational sessions at already existing library events
Have patrons complete Surgeon General’s tool

2. - Schedule group classes with multiple patrons at convenient times
Evaluating the
Program
Gather demographics on libraries
Evaluate librarian training
- Training should include a pre- and post- test
Evaluate stakeholder buy-in
- Number of schools participating, number of doctors involved, etc.
Use pre- and post-test to gather patron data – must be done for patrons to get card
- Should ask if the librarians were helpful, where they heard about the program, how useful they think the
pedigree will be, if they feel they need to / should see their doctor, if they will see their doctor, and other
questions relevant to the principal investigator.
Survey the librarians to see if they feel more comfortable acting as a health resource person
