Item Description
This is page 1 of 4 pages.
Scope of Work: Summary Replacement Value Appraisal Report:
This Summary Replacement Value Appraisal report is for the function of obtaining insurance. It is a statement of
the approximate retail replacement cost of the jewelry item(s) described herein, is the appraiser’s opinion and is for
the express purpose of use in insurance scheduling. Acceptance of this appraisal is considered to be your agreement
with the limiting conditions, as stated here and elsewhere in this report, and is also your agreement to hold the
appraiser, and participating companies, harmless regarding any litigation regarding this report. If you do not agree with
these conditions, and conclusions of the report, contact the appraiser for clarification or return the original and all
copies of this report to the appraiser. The valuation(s) shown in this report are valid only as of the date of the report.
This report gives no implication or suggestion of any past or future value.
Client and Intended Users:
This appraisal report is made at the request of the above named person or persons (defined as the client) and is
for his/her use, and the use of the client’s insurance company only (defined as the intended user), and no others. This
report is not a statement or indication of title or ownership. Only the client or the intended user has the right to request
a copy of this report or to request any information contained in this report. Possession of this report or any copy does
not carry with it the rights of publication for any purpose, nor does possession carry with it the right of use for any
purpose or function other than that stated in this report.
This report may not be used for any purpose by anyone but the above named person or persons, and their
insurance company, without the previous written consent of the appraiser, who holds the copyrights to this report.
This report may not be used for any advertising purpose, for selling this or similar types of jewelry or for price
comparison purposes by anyone.
Limiting Conditions:
This appraisal represents the opinion of the appraiser, based on limited information, and is not a guarantee or
warranty. The appraisal was performed at a special in-store appraisal event, where time constraints and non-laboratory
conditions existed, and where the stones appraised (diamonds or colored stones) were not removed from the mounting
for weighing and/or grading. This report, and its conclusions, is therefore provisional, pending laboratory grading and
identification.
Limited portable laboratory and grading equipment was used for this appraisal and some information may have
been provided by the customer in the form of store receipts or laboratory reports, on which the appraiser may have
relied. The identification of natural vs. synthetic diamonds (CVD or other synthetic growth processes) and the
separation of natural vs. synthetic or glass filled gemstones, cannot always be made during these in-store special
event appraisals, whose function is to provide the insurance company with a report that reasonably identifies the item
and allows the holder to obtain personal insurance on their items.
Each page of this report is numbered in sequence. Pages of this report may be separated only if the separate
pages are accompanied by this page (page 1) of this report, otherwise important information may be excluded and not
provided to the insurance company. Unauthorized use of this appraisal in advertising can constitute a deceptive trade
practice as defined in Federal Trade Commission guidelines. Acceptance of this appraisal constitutes an agreement, on
the part of the client and intended users, to these terms and conditions and those listed elsewhere in this appraisal,
which further defines aspects of this appraisal and its use.
Preparing this appraisal report does not require the appraiser to perform any additional services except under
separate and special arrangements, and at additional and customary fees. This includes updating this appraisal,
providing a lost copy, changing an item specification, or a name, giving testimony, appearing in any court or hearing,
or engaging in post appraisal consultation with the clients or intended users. If testimony or deposition is required, the
above named client and/or intended users shall be responsible for any additional time, fees, and charges. A retainer
will be required.
The item values listed in this report do not include sales tax and are based on the item being replaced with a
similar item. Gemstones without a certificate or sales receipt will have their weights estimated by using standard
formulae and estimated measurements. Stones are not removed from the mounting for weighing unless stated
otherwise. The grading and identification of diamonds and colored stones mounted in jewelry is provisional.
The pictures in this report are for descriptive purposes and may not represent the actual size of the item

Definitions of Clarity Grades:
Flawless
Stones of this category are free from all blemishes or inclusions when examined by a Gemologist with a 10x binocular
microscope equipped with dark field illumination. The following conditions will not remove a stone from a flawless grade:
1. An extra facet located on the pavilion, which cannot be seen from the face-up position.
2. A natural that does not extend beyond the maximum width of the girdle or break the symmetry of the girdle outline and
cannot be seen from the face-up position.
3. Internal graining which neither is reflective, whitish, colored, nor significantly affects transparency.
Internally Flawless
Stones of this category are free from all inclusions but do possess blemishes. Normally these stones may be made flawless
by minor polishing. Exceptions to this general rule would be diamonds that exhibit surface grain line that could not be
removed by re-polishing. Such stones may still be graded IF.
VVS1 and VVS2 (Very Very Slightly Included)
These grades contain inclusions so small that they are difficult for a gemologist to locate under 10x. Pinpoints, feathers,
minor bruises, and internal graining difficult to locate characterize the VVS grades.
VS1 and VS2 (Very Slightly Included)
These grades contain inclusions clearly noticeable under 10x. Small included crystals, small feathers and small clouds
characterize this grade.
SI1 and SI2 (Slightly Included)
The grades contain inclusions clearly noticeable under 10x. Stones in these grades (particularly the SI2 Grade) may
disclose inclusions visible to the un-aided eye when placed table down on a white background. Included crystals, clouds
and feathers characterize the SI grades.
I1, I2, and I3 (Included)
The included categories contain inclusions, which are quite obvious when viewed under 10x. And which can be seen with
the unaided eye. The grades vary from stones with inclusions so numerous that they affect the transparency and brilliance
to those with severe cleavages that are likely to extend during ordinary wear.
Typical Clarity Characteristics:
External Characteristics (blemishes) Plotted in Green and Black
1. Natural a portion of the original crystal “skin”. Naturals may be located anywhere on a stone but are usually on or
near the girdle.
2. Extra facet any polished surface in addition to the number of facets required for a particular cutting style and placed
without regard for the stone’s symmetry. Extra facets are often found around the girdle.
3. Pit a very small opening on the surface of a facet.
4. Cavity, chip or nick an opening or hole on a polished surface. Nicks are usually found around the girdle edge.
5. Abrasion minute chipping or white frost appearance of facet junctions resulting from one diamond rubbing against
another or from blows during normal wear.
6. Scratch or wheel mark surface marks caused either by another diamond being drawn across the surface of the stone
or by irregularities in the polishing lap.
7. Surface graining visible surface indications of irregular crystal growth.
Internal Characteristics (inclusions) Plotted in Red
1. Feather any break in a diamond (either cleavage or fracture).
2. Included crystal a crystal, of s diamond or of as mineral, which is contained with in a diamond.
3. Dark included crystal an included crystal, which is colored or appears black.
4. Knot an included crystal of diamond that breaks the surface.
5. Pinpoint a very small included crystal ordinarily appearing as a tiny white dot under 10x. Pinpoints may occur singly
or in groups.
6. Cloud any hazy or “milky” area in a diamond.
7. Internal graining lines visible internal indications of irregular growth.
This is page 3 of 4 pages.
8. Laser drill hole a man-made hole in the stone resulting from laser drilling, usually tubular in appearance. Laser drilling
is usually done to lighten dark inclusions.
9. Bearded of feather girdle a girdle exhibiting hairline feathers extending into the stone.
10. Bruise or percussion mark—a crumbling or indentation in the surface often attended by root-like feathers.
This is page 4 of 4 pages.
