(PCCM) Program, Passport to Health
Wanted to move Passport to medical home program,
not sure where to start
Wanted to take full advantage of the technical
assistance (TA) grant; decided to go multi-payer
Set out to form broad group of stakeholders

2. Forming Key Partnerships
Engaged major payers, associations, federally
qualified health centers (FQHCs), hospitals, provider
based clinics, rural providers, tribal health, etc.
Sent letters and followed up with personal phone calls
Talked up participation of other groups and grant
from the National Academy for State Health Policy
(NASHP)
Medicaid convened group with the help of NASHP
Transitioned to neutral convener, Insurance
Commissioner’s Office

3. Lessons Learned
Every time we met we realized someone was missing
from the discussion
Some of the stakeholders’ involvement was not
necessary so early in the process
Convening such a broad group in our vast state has
made communications challenging
