- I’ll start by letting you read the Long Term Conditions Alliance Scotland vision. This vision is something that we at LTCAS ( as you will hear Jen and I refer to it throughout our presentation) keep in mind in all areas of our work. I am going to begin our talk by giving you an overview of the work of LTCAS, an umbrella organisation for the larger set of groups that support or represent people living with long term conditions and their unpaid carers.
- I’ll start by letting you read the Long Term Conditions Alliance Scotland vision. This vision is something that we at LTCAS ( as you will hear Jen and I refer to it throughout our presentation) keep in mind in all areas of our work. I am going to begin our talk by giving you an overview of the work of LTCAS, an umbrella organisation for the larger set of groups that support or represent people living with long term conditions and their unpaid carers.
- The role of NHISS is being well explored today, but I want to tell you about ALISS – Access to Local Information to Support Self Management. This is the messy, grubby information. This is the evening class that moves venue, changes the contact telephone number and skips a couple of weeks because the tutor is ill. This is the hard to capture and keep clean end of information. But it’s also very often the most useful – it’s in your neighbourhood.
- In ALISS we want to get this really rich useful information scraped off the poster boards and into mobile phones, blogs websites – making it freely available to all however they get their information.
- Have good information about areas of relatively high cost – hospital emergency admissions and specialist community based services Need more intelligence about primary and community care – primary care team skills and knowledge and patterns of consultation
