8. How Do I Participate?
Step 1:
Complete and return the enrollment form
enclosed in the WellTrak packet. You can
mail it in the business reply WellTrak
envelope (also included in the packet) or
you can fax it to 913.322.8497.
You may also enroll over the phone by
calling the WellTrak Help Desk at
1.877.750.9355.
Step 2:
Once WellTrak receives your enrollment
form, a representative will contact you to
schedule your first appointment with a
WellTrak Pharmacist. It’s that easy!

WellTrak is offered to prevent the health
risks that occur when medications aren’t
working well to treat your conditions.
With our guidance as your pharmacy advocate,
our goal is to ensure:
You understand your medications
Your medications work well for you and your conditions – e.g. you aren’t
experiencing side effects, can afford to regularly fill your meds, etc.
Your care plan is safe and effective – with multiple physicians, it can be
hard to manage all of your medications. WellTrak connects the dots and
works with your physicians to create a synergistic care plan.
