การคำนวณปริมาตรของกอนเลือด
(Calculation of Volume of Hematoma)21
ปริมาตรของกอนเลือดคำนวณจากเครื่องเอ็กซเรยคอมพิวเตอรโดยตรง หรือ ใชสูตรคำนวณ
สูตร ปริมาตรของกอนเลือด (volume of hematoma) = 0.542 * X * Y * Z
( X, Y, Z คือ เสนผาศูนยกลางของกอนเลือดในแนวแกน X, Y, Z )
โดยการนำฟลมเอ็กซเรยคอมพิวเตอรสมองครั้งแรกของผูปวยมาพิจารณาดังนี้
1. แตละ slice มีความหนา (slice thickness) กี่ ซม. ซึ่งจะมีบันทึกไวบนแผนฟลมหรือ ในรายงานผล
สวนใหญจะหนา 1 ซม. ในที่นี้สมมุติวา slice thickness = ‘ t ’
2. พิจารณาวามีกอนเลือด (hematoma) อยูที่ตำแหนง (location) ใด
3. พิจารณาวา slice ที่มีกอนเลือดอยูจำนวนกี่ slices ในที่นี้สมมุติวา จำนวน = ‘ n ’ slices
4. เพราะฉะนั้นคา Z = t * n สมมติวาเทากับ c
5. พิจารณาวา slice ไหนมีกอนเลือดขนาดใหญที่สุด นำ slice นั้นมาเปนเกณฑวัดขนาด
6. วัดขนาดของกอนเลือดในแนวแกนที่กวางที่สุดของกอนเลือด สมมุติแกน X= a และแกน
Y= b (โดย a และ b ตั้งฉากกัน ดังรูป)
7. ปริมาตรของกอนเลือด ( volume of hematoma ) = 0.542* a * b * c

การประเมินผูปวย การเฝาระวัง และติดตาม
แพทยควรตรวจและบันทึกผลในเวชระเบียน เพื่อใชเปนขอมูลในการเฝาระวังและติดตามผูปวย
ทั้งในระยะแรก และเมื่อผูปวยเขารับการรักษาในโรงพยาบาล โดยใชการตรวจรางกายที่สำคัญดังนี้
1. Vital signs:
a. Blood pressure (BP) : ผูปวยที่มีความดันในกะโหลกศีรษะเพิ่มขึ้นมักจะมี
Cushing’s reflex คือ BP สูงขึ้น Pulse pressure กวางขึ้น
b. Heart rate (HR) : ผูปวยที่มีความดันในกะโหลกศีรษะเพิ่มขึ้นมักจะมี Cushing’s reflex คือ HR ชาลง
c. Respitatory rate (RR) : ผูปวยที่มีการดำเนินโรคที่แยลง (worse) มักจะมีการกดศูนยหายใจ
ทำใหมี RR ชาลง หรือ Apnea ได
2. Glasgow Coma Scale (GCS) ดูหนา 31 : การเปลี่ยนแปลงของ GCS score ที่ต่ำลง 2 ระดับ
ใหถือวาผูปวยมีอาการที่แยลง
3. Pupil :
a. Size : ขนาดของ pupil ที่เปลี่ยนแปลงอาจมีภาวะ Brain herniation (uncal herniation)
b. Equality : ขนาดของ pupil ไมควรแตกตางกันเกิน 1 mm
c. Reaction to light : การไมตอบสนองตอแสงของ pupil อาจมีภาวะ Brain herniation
(uncal herniation)
การเปลี่ยนแปลงของ pupil ที่แตกตางจากการประเมินครั้งกอน (ขนาดใหญขึ้น ไมตอบสนองตอแสง
หรือ ขนาดมานตาตางจากอีกขางหนึ่ง) ใหถือวาผูปวยมีอาการแยลง
4. Motor power : การเปลี่ยนแปลงของ motor power ที่ลดลงจากการประเมินครั้งกอน 1 grade
ใหถือวาผูปวยมีอาการแยลง
5. อาการอื่นๆ เชน อาการ increased intracranial pressure หรือ hydrocephalus (vomiting,
sixth nerve palsy), อาการ brain herniation

Brainstem dysfunction
ผูปวย Hemorrhagic stroke อาจจะมีอาการ brainstem dysfunction ไดแก
1. Vital signs : RR และ รูปแบบของการหายใจ
2. Level of consciousness : เนื่องจาก brainstem เปนที่อยูของ reticular activating
system ซึ่งควบคุมระดับความรูสึกตัวของผูปวย
3. Brainstem
a. Brainstem function : ตรวจ cranial nerve ตางๆ เชน การกลอกตา (CN. III, IV, VI)
การขยับกลามเนื้อของใบหนา (CN. VII) การกลืน (CN.IX, X)
b. Brainstem reflexes : Corneal’s reflex (CN. V&VII), Doll’s eye (CN. III&VI&VIII), Caloric
test (CN.III&VI&VIII), Nystagmus (CN.III&VI&VIII), Gag reflex (CN.IX)

การวินิจฉัย Hydrocephalus จาก CT Brain
(Diagnosis of Hydrocephalus by CT brain)
Hydrocephalus was defined as the presence of
1. The presence of a dilated contralateral temporal horn.
2. Unilateral dilated frontal and posterior horn of the lateral ventricle from
deep cerebral hemorrhage.
3. Bilateral dilated lateral ventricles with or without a dilated third ventricle
resulting from obstruction by a clot in the ventricle or the mass effect of deep
cerebral hemorrhage.
4. The frontal horn is considered to have enlarged if there is an increased
radius, decreased ventricular angle, and sulcal effacement of the frontal lobe.
5. The posterior horn is considered to have enlarged if there is rounding of the
posterior horn with sulcal effacement of the parieto-occipital lobe.
6. The third ventricle is enlarged if the width is increased (> 7 mm.) and if there
is ballooning of the anterior recess.
7. The fourth ventricle is considered enlarged if there is bulging.

ปจจัยที่มีผลตอการรักษา
(Factors affect outcome)
ปจจัยดังตอไปนี้มีผลตอการพยากรณโรค ไดแก
1. GCS score/stroke score
2. Location of hematoma
3. Size of hematoma
4. Coagulopathy
5. Hydrocephalus
จากการศึกษาแบบ prospective ของ Castellanos et al.41
พบวาผูปวยที่มีการพยากรณ
โรคที่ดี สวนใหญมีปจจัยตอไปนี้ คือ higher Canadian stroke score; lower blood pressure
higher fibrinogen concentrations และมี CT findings คือ smaller volume of ICH,
more peripheral location of hematoma, less intraventricular spread of bleeding,
less midline shift
ฉะนั้น การอธิบายใหผูปวยและญาติรับทราบถึง prognosis & outcome ใหพิจารณาตาม
ปจจัยขางตน เพื่อใหเกิดความเขาใจตรงกัน

(Risk of hematoma expansion after initial CT Scan)
The cutpoint for hematoma enlargement was determined as
V2-V1=12.5 cm3
or V2/V1=1.4 (sensitivity=94.4%, specificity=95.8%).

ไดศึกษาการขยายขนาดของกอนเลือดในสมองหลังจากทำ CT scan ครั้งแรกพบวา
Initial CT scan at < 3 hours after onset of stroke มีความเสี่ยง 36% ที่ขนาดกอนเลือดใหญขึ้น
Initial CT scan at 3 to 6 hours after onset of stroke มีความเสี่ยง 16% ที่ขนาดกอนเลือดใหญขึ้น
Initial CT scan at 6 to 12 hours after onset of stroke มีความเสี่ยง 15% ที่ขนาดกอนเลือดใหญขึ้น
Initial CT scan at 12 to 24 hours after onset of stroke มีความเสี่ยง 6% ที่ขนาดกอนเลือดใหญขึ้น
Initial CT scan at 24 to 48 hours after onset of stroke มีความเสี่ยง 0% ที่ขนาดกอนเลือดใหญขึ้น

จากขอมูลดังกลาวขางตน แพทยผูรักษาอาจใชเปนเครื่องมือในการพิจารณาถึงความเหมาะสม
ของการทำ CT scan brian ซ้ำ เพื่อดู hematoma expansion ซึ่งจะเปนประโยชนในการใหการรักษา
กอนที่ผูปวยจะมีอาการเลวลง
