Duties and Responsibilities: Provides ongoing assessment of the individual needs of each assigned
child and/or family. Establishes, updates and monitors service plan for assigned clients in
collaboration with youth, birth parents, foster parents, adoptive parents, and other members of the
clinical/permanency team.
Provides casework services consistent with service plan goals. Arranges for therapeutic or related
services designed to meet the well-being needs of children, youth and families. Maintains

Functions as a liaison between CFS and other community-based organizations and provides
technical assistance to other agencies as needed to advance CFS mission.Conduct a strength-
based assessment of the family~s and child~s strengths & needs. Work with each family to identify
and convene the Care Planning Team to develop an Individual Care Plan (ICP), as well as other
relevant plans such as a Behavior Management Plan and Crisis Response Plan. Secure family and
child participation in the development and understanding of the ICP goals and interventions.
Manage and deliver therapeutic services including family, individual, and group therapy, cognitive-
behavioral interventions, behavior management training, solution-focused counseling, psycho-
education, and crisis counseling as indicated by the ICP. Provide after-hours phone support to
address urgent child or family crises. Facilitate regular communication with the ICP Team members
and convene the ICP Team at least quarterly to review and revise various practices.
