23. METABOLISMO DAS DROGAS

Metabolismos de FASE I : convertem o fármaco original em
um metabólito mais polar através de oxidação, redução ou
hidrólise. O metabólito resultante pode ser
farmacologicamente inativo, menos ativo ou, às vezes, mais
ativo que a molécula original. Algumas drogas polares são
conjugadas na sua forma original sem passarem por reações
da Fase I.

This presentation considers the topic of context-sensitive half-time. This means that the elimination of a drug will depend on how long it has been infused. And so context-sensitive half-time really means the time dependant elimination of a drug. The slides in this sequence move automatically and do not require you to press a key.
If we imagine a 3 compartment model and we infuse a drug to achieve a certain concentration within the blood, and therefore within compartment one. As we infuse the drug the drug will be removed from compartment one by the processes of elimination and also redistribution into compartments 2 and 3.

Let us consider the situation where we infuse the drug only for a short duration of time
When we switch the drug off the drug will be removed from the central compartment by the process of elimination but also by redistribution to the peripheral compartments C2 and C3 drug will therefore move into these compartments as the time passes.
If we infuse alfentanil for example to achieve a blood concentration of 200ug per ml and switch of the infusion after 10 minutes then the time taken for the concentration to half is shown in the next slide
It takes about 10 minutes for the concentration to decrease from 200 - 100 ug per ml
Let us know consider the situation where we have infused the drug for a long duration of time. Drug has now moved into the peripheral compartments C2 & C3
When we switch the infusion off drug is removed from the centre compartment now only by elimination. There is no redistribution of the drug from the central to the peripheral compartments, and indeed drug is now able to return from the peripheral compartments, which increases the load in compartment one.
The effect of this is to slow the elimination of drug from compartment one following a prolonged infusion
If we now graph the results of that long duration of infusion where we infused alfentanil at a blood concentration of 200 ug per ml but this time we infused it for not 10 but for 100 minutes.
The time taken to half the concentration has now increased to about 40 minutes.
We can see clearly therefore that if we infuse alfentanil for 10 minutes or for 100 minutes the time taken to half the concentration will be markedly different.
With the short duration, the concentration is halved in about 10 minutes whereas with the long duration, it will now take about 40 minutes for the concentration to decrease exactly the same amount.
If we have a drug which has a rapid clearance like remifentanil and infuse it for a long duration again drug will move into the peripheral compartments. If we switch off the infusion
then the drug is removed solely by the process of elimination
However, in this example the elimination rate is so rapid that it really does not matter that drug comes back from the peripheral compartments. The metabolic rate is so high it can easily deal with drug returning.
The context-sensitive half-time is the time required to half the concentration by 50% The decrement time is the time required to decrease the concentration by a different value for example 25 % or 75 % Decrement times may be of more value clinically because it is rare that we wish to reduce the concentration by precisely by 50% however to overall concept is exactly the same and this understanding gives us a better insight into how to select and how to use the drugs administer in Intravenously Anaesthesia
If we graph the changes in blood concentration for remifentanil we can see that remifentanil has been infused to achieve a blood concentration for 10ug per ml and it has been infused for 100 minutes
The time taken for the concentration of remifentanil to half is about 3-4 minutes.
We can therefore see if we infuse alfentanil to a target of 200ug per ml but the duration is 100 minutes of infusion it will take about 40 min for the concentration to half. Infusing alfentanil to the same blood concentration but for only for 10 minutes will allow the concentration to half in about 10 min. Whereas with the remifentanil if we infuse it for 100 minutes it will still only take a few minutes for the concentration to half.

If we now consider remifentanil we see that it has a very low context-sensitive half-time indeed it is context-insensitive this means that even though we infuse remifentanil for a prolonged period of time it still takes only a few minutes for the concentration to be halved. Remifentanil therefore can be thought of context-insensitive and indeed alfentanil after infusing for about 3 hours is also context-insensitive In that no matter for how much longer we infuse alfentanil it will always take the same time for the concentration to half.
This understanding of the context-sensitive half-time helps us to explain why propofol is a better drug than thyopent for Total Intravenous Anaesthesia We can see that if we infuse propofol for 6 hours then it will take about 30-35min for the concentration of propofol to half, in contrast if we infuse thiopentone for 6 hours then it takes over two and a half hours for thiopentone concentration to half. This really make thiopentone not a useful drug to reduce hypnosis for a long period of time and this is why propofol is now the standard hypnotic that we use for Total Intravenous Anaesthesia
