                          The proponent of this form is Commandant, Army School of Music
                                    DATA REQUIRED BY THE PRIVACY ACT
Authority: Title 5, sec.3012; Title 10, U.S.C.E.O. 9397. Principal purpose: Use to determine the auditionee's
technical qualification for enlistment, initial classification, reenlistment, or classification into the military band program.
Routine uses: To initiate processing into the military band program, and as a record of the individual's technical
ability and progress. Disclosure: Voluntary. Failure to provide Social Security number may result in a delay or error
in the processing of this form.

                   Part 4 (maximum score of 4 points) is added to final rubric score
                                Maximum score for audition is 40 points
