While our crane operator was uploading the last empty container on the ship CSAV MARESIAS

While the operator of the mule ML07 was moving a container he did not realize that about him
was the Mardom’s Top Loader, making contact with the spreader of the same which caused him

At the time of the express visual inspection during the dispatch of the container, the client

Containers numbers CMCU555223, CMCU4559749 and TTNU9244916, had to be loaded onto
the ship Marcaja V. 57 dated 23/06/2010 and later on the ship Crawler chipper as of

26/06/2010 and was not made in any of the two occasions because of a human error.
During verification of the container, it began to rain and immediately the goods were covered
13-7-2010 with canvas, but still rain penetrated resulting wet 239 boxes.
While the operator of the gantry crane was uploading to the vessel the container with goods for
18-7-2010 exports, he caused a bulge at the lower right side panel of the container.
While the container was being moved, one of the twist lock of the crane caused a hole in the

port, at the moment that the operator of the crane PP4 placed the container at the floor the
container suffered a dent on the left side panel due to the pressure that received from the front
tires of the crane.

At the time that the container was at the dispatch ramp, it was discovered that the container
28-04-2011 had a cut in the roof caused by a twist lock of a crane causing the wetting of the goods.
During operation of the vessel the crane operator was preparing to mount the container in the
mule ML-04 while placing the container on the tray made contact with it. Also when the gantry
25-05-2011 crane no.1 proceeded to release a F/ R that this had above a chassis, one of the twist lock did
not open completely and the crane lifted the container with the chassis and the mule.
While the vehicle Toyota Tacoma was being mobilized, the stabilizer bar fell off and hit a
27-05-2011 Mercedes Benz causing damages to the same.
At the moment that the container was open at the verification area it was discovered that the
30-05-2011 roof had a cut causing that 20 boxes containing backpacks and other accessories got wet.
Operations of the ship were delayed by the gantries, at the time of termination of operations in

reversing and turn, the twist lock were detached themselves from the front corner, which

While the hold of the ship was being loaded with one of its own cranes, at the time of the
container was being placed in the cells the equipment started to swing and made contact with

one of the cells of the ship causing damages to the same, as well as to a platform and a cover.
At the time that HIT’s crane proceeded to release the container from the chassis number
Tca200733 carried by a truck Volvo license plate number L-256448, one of the pins on the
12-07-2011 chassis remained closed and the crane raised the equipment with the chassis, resulting in
kinking of the chassis structure.
8-07-2011 When the operator was moving a container loaded with scrap metal for export into the ship
Franklin Straint, the floor gave and the crane bent down abruptly. When its back side was raised
the twist locks fell off and the container was trapped between the tower crane and the
pavement. This shipment was canceled.

The operator of the crane PP-13 was about to place a container in stowage destined to import
goods and did not realize that the position was occupied which provoked to make contact with

During operation of the ship Caribbean Jade, the operator of the crane was lifting the container
23-2 of the hold No. 20 and while ascending to it, the spreader made contact with the container

doors. The spreader experienced cuts and dents in the top of one end.
