13. DOS 2/18/2009: X-Ray after placement of Neoss Implants 4.5 x 9
with a torque of 40 for tooth 4, and 4.0 x 11 with a torque of 40
for tooth 5 (No Implant placed for missing tooth 3 due to forward shifting of tooth 2)

14. DOS 8/4/2010: Post Op X-Ray after final restorations were
completed on teeth 4 and 5

15. DOS 7/14/2008: Pre-Op X-ray of Root Tips of
teeth 14 and 15, & missing tooth 16

16. DOS 2/18/2009: Sinus Augmentation using a Lateral Window approach and immediate placement of
Neoss Implant, 4.5 x 9 with a torque of 35 for teeth 14 & 15. MinerOss bone graft, a mixture of allograft
mineralized cortical & cancellous chips, was placed followed by a DynaMatrix Bioresorbable
Membrane. Sutured with an Interrupted suture using Cytoplast PTFE 4.0 White Monofilament
Nonabsorbable Suture material.
