1. Silver Grove School-Based Health Center Our school-based Health Center will help students be healthier happier individuals

2. Objective Our health center will provide students with a place to go for psychological, physiological and emotional needs. The center will encourage students to come in and improve their learning experience by talking about the things that are holding them back which may be many things in the environment or with their health.

5. Statement of Need The individuals have no access to health care within the community besides what the school offers. The students are the only ones offered health services at this time. The school has lost previous funding.

7. Current Funding A Grant from the Health Foundation of Greater Cincinnati. Private and Business donations

8. Budget Since the program has no overhead, as mentioned before all proceeds will be directly given back to the students needs. Some things that the proposed $1000 will be use for: Pamphlets Immunizations Vaccinations Supplies Healthcare information Updating resources available Time to talk to students individually and by classes
