Open Campus Proctored Testing
Handbook for Faculty
Introduction
This proctored testing handbook has been created for Open Campus faculty to serve as a proctored
testing reference guide. It is our hope that the information contained herein will enable better
understanding about the Open Campus Assessment Center practices and testing procedures. We thank
you for your continued support and acknowledge the extensive part faculty play to ensure academic
integrity andapositive and successful testing experience for our students at Florida State College at
Jacksonville.

Proctored exams and tests are locally administered in classrooms, computer labs and Assessment
Centers, and at remote sites around the globe. While the Open Campus Assessment Center at the Urban
Resource Center is the main testing site for online students, other campus locations such as South,
Deerwood, Kent and Nassau help to accommodate student testing needs.

Additionally, the Assessment Centers at Open Campus, North, Kent, and Nassau provide special testing
accommodations during certain times and days for self-paced, online math students (MAT0025, MAT
1033). Students are expected to select their testing locations and times at the beginning of the semester
term via an electronic appointment scheduling system.

Types of Testing Supported by the Open Campus Assessment Center
    PC exam: an exam administered on a computer
    Paper exam: an exam administered in paper format
    Make-up exams: an exam that a student missed but has permission to reschedule and complete the
    test
    Exit exams: a state required exam in order to pass a course. Remediation courses require the
    student to complete and pass a state exit exam.
    Commercial exams: exams administered for local businesses.

Local Testing Procedures
Proctored testing activity is a collaborative effort between faculty and the Open Campus Assessment
Center. To begin the proctored testing process, Assessment Centerstaff sendsout a welcome letter to
faculty several weeks before the term begins requesting their preferred testing dates. Once this
information is collected, it is recorded on a planning spreadsheet.

If a course shell requires pencil/paper exams, a copy of the exam shouldbe sent via e-mail (as an
attachment) to the Assessment Center two weeks before the date the test is to be administered. If the
test is computer based, the instructor sends the testing access password along with backup tests to the
Open Campus Assessment Center. Back-up paper/pencil tests are reserved for times when there is a
problem administering a test or exam via the computer.

Remote Site Testing
If students do not live in Nassau, Duval, St. Johns, Clay, Bradford, Union, or Baker County, they are
provided remote site testing accommodations. The Open Campus Assessment Center facilitates remote
site testing services providing students with the opportunity to attend Florida State College from
anywhere in the world while taking assessments near their home.

Remote test site students are asked to complete a Remote Test Site and Proctor Form. These forms are
available on the Distance Learning Website http://www.fscj.edu/mydegree/distance-
learning/testing/assets/documents/OnlineRemoteProctor.pdf.

Once a remote site test form is received in the Assessment Center all information is verified by an
Open Campus Assessment Center staff member. Afterthe qualified testing environment and proctor
are verified and approved, the student will receive an e-mail confirmation along with testing
information. Should the student’s desired testing location not be approved, Test Center staff will
select an appropriate remote test proctor near the student’s designated Zip Code.

Make-up Exams
Students who miss a scheduled exam date must contact their professor for next steps. Instructors
should note make-up testing policy and procedures clearly in the course syllabus at the beginning of
each term. It is the student’s responsibility to know their instructor’s make-up/retesting policies.

Make-up exams must be taken no later than the week following the regularly scheduled exam.
(Specially assigned Exit Testing dates/times are the exception.)

Both instructor and student have make-up scheduling responsibilities that include the following:

Instructor responsibilities
    Once student has informed instructor about a missed exam in a timely manner, and the instructor
    agrees to a make-up exam, the instructor will send this information to the student:

            Go to this web site:http://www.fscj.edu/onlinetesting
            Select “Special OC Testing”
            Select desired test date and time

    The instructor will also send an e-mail to assessme@fscj.edu with the following information
    included:

            Student name
            Course name
            Course reference number
            Exam name that student is taking
            If needed, a make-up exam must be sent at this time as well

Student responsibilities
   If a scheduled exam or test has been missed, it is the student’s responsibility to contact their
   instructor as soon as possible and request a make-up exam.
   The student will follow the procedures provided by the instructor for scheduling their make-up
   exam.
   Students are responsible for bringing their testing confirmation information with them on the
   scheduled testing date at the Open Campus Assessment Center.

Exit Exams
TheOpen Campus Assessment Center coordinates and communicates testing activity related to state
and College mandated exit examinations. Students taking an Exit Exam for ENC0025, MAT0018,
MAT0028 and/or REA0017 must schedule and complete their Exit Exam on the dates/times provided by
the Open Campus Assessment Center. The Open Campus Assessment Center assigns specially
designated Exit Testing dates/times to meet college mandated Final Grade deadlines and to comply with
college Institutional Effectiveness standards. Faculty are responsible for communicating Exit Testing
dates/times to their students and ensuring that all Exit Testing students complete their Exit Exams on
the scheduled date/time.

Open Campus faculty living in Jacksonville administer initial exit exams to their students. If a student
misses their initial exit exam,they may be able to schedule a make-up exam. Instructor approval is
needed for a make-up or retest for an exit test. Exit testing make-ups and retests are only offered at the
Open Campus Assessment Center.

Open Campus faculty living outside of Jacksonville are assigned a Test Examiner to streamline the Exit
Test process. The Test Examinerfacilitates communication with the instructor about exit testing
schedules and exit testing procedures for students to follow. Once a student has scheduled an exit exam
appointment, they will receive e-mail confirmation that must be shown to Assessment Center Staff at
the start of a testing session.

Services for Students with Disabilities
The college fully complies with the Americans with Disabilities Act (ADA). Students who require
accommodations for online courses should contact the Office of Services for Students with Disabilities to
arrange services. The Director of Services for Students with Disabilities can be reached for more

As appropriate, the following services can be provided upon receipt of documentation of a
student’sdisability:

Student Survey
At the end of the term students will be asked to complete a survey related to their testing experience.
Student feedback is an important part of the Open Campus Assessment Center continuous improvement
effort.

Blackboard Maintenance
There are limited times in each term during which the Blackboard Course Management System is
unavailable due to maintenance and upgrades. The times and dates of scheduled maintenance are
posted on the Blackboard home page at https://bb.fscj.edu/. Faculty should be aware of and remind
students of Blackboard outage dates.

Testing Site Guidelines and Procedures
The following items outline expectations and requirements for all testing locations.

1. Students are required to present valid government-issued photo identification with signature.

2. The name of the student must match the identification name.

3. Students must present e-mail test confirmation for MAT 0028 and MAT 1033.

4. Materials or supplies are not admitted into the testing room. ONLY materials provided by the

    proctor (i.e., paper and pencils) or allowed by the instructor (calculators, pocket dictionary, etc.)

    are permitted in the testing area.

5. No food or drinks are allowed in the testing area.

6. Students may only use materials provided by the proctor, unless the instructor has authorized

    supplemental testing materials.

7. Students are not permitted to leave the testing area once the test/exam begins. Students are only

    permitted to leave the testing area to use the restroom when necessary, and only one student is

    permitted to leave at a time.

8. All electronic devices must be turned off during testing.

9. All scratch paper, used or unused, are to be returned to the proctor along with any other testing

    items.

10. Students requiring special needs accommodations must maintain

    the proctor prior to testing.

11. No children are permitted in the testing area.

12. A quiet testing atmosphere is expected at all times. No talking or

    excessive noise is allowed during testing periods.

    Earplugs are available upon request.

13. All tests must be completed prior to closing.

                              Frequently Asked Questions
1. I am a local student. Where and when can I take my proctored exams?

    Local students can choose from different campus testing locations if the exam is pencil/paper: Open
    Campus, South, Kent, and Nassau. Computer-based exam students can choose from certain campus
    testing locations –Open Campus, South, Deerwood, Kent, and Nassau. Students may also choose
    from different testing times and dates.
2. Is there any cost?

    No.
3. If I am not a local student, how and where can I take my proctor exams?

    Remote students can take their exams with approved test proctor near their Zip Code. Students
    should review their course syllabus for more information on remote testing.
4. Are appointments required for testing?

    Appointments are required for testing. Students with appointments are always tested first. Walk-ins
    may be tested only if there are available tests, seats, and time.
5. What if I have an emergency and cannot test when I scheduled my appointment?

    The student can ask their instructor whether a make-up exam is permitted. Only the instructor has
    the authority to allow make-up tests.
6. What information do I need to bring with me at my testing location?

    Students must present valid government-issued photo identification with signature at the time of
    testing. Active duty military may use their military ID card in order to test.
7. What happens if I arrive late at my site?

    It is possible that students could be turned away and need to reschedule. If within the allowed
    testing week, students may be able reschedule an appointment at another testing location. Not all
    professors will permit students to take a make-up exam.
8. Is my exam paper/pencil or computer based?

    This information will be provided in each course syllabus.
9. How much time do I have to take my exam?

    Time allotment is at the discretion of the instructor or course shell.

10. May I use a calculator?

    Some exams do allow a calculator. Individual instructors will furnish the Assessment Center with
    those instructions.
11. What type of exam will it be? Multiple choice, fill in the blank etc.?

    The choice for the type of exam is the instructor’s decision.
12. Where is the Open Campus Assessment Center located?

13. Is it possible to cancel my appointment in the online appointment software?

    Yes, it is possible. Students should be mindful of classmates’ appointment needs and cancel
    unneeded appointments to free up testing times and locations for other students

  • 3. Open Campus Proctored Testing Handbook for Faculty Introduction This proctored testing handbook has been created for Open Campus faculty to serve as a proctored testing reference guide. It is our hope that the information contained herein will enable better understanding about the Open Campus Assessment Center practices and testing procedures. We thank you for your continued support and acknowledge the extensive part faculty play to ensure academic integrity andapositive and successful testing experience for our students at Florida State College at Jacksonville. Proctored exams and tests are locally administered in classrooms, computer labs and Assessment Centers, and at remote sites around the globe. While the Open Campus Assessment Center at the Urban Resource Center is the main testing site for online students, other campus locations such as South, Deerwood, Kent and Nassau help to accommodate student testing needs. Additionally, the Assessment Centers at Open Campus, North, Kent, and Nassau provide special testing accommodations during certain times and days for self-paced, online math students (MAT0025, MAT 1033). Students are expected to select their testing locations and times at the beginning of the semester term via an electronic appointment scheduling system. Types of Testing Supported by the Open Campus Assessment Center PC exam: an exam administered on a computer Paper exam: an exam administered in paper format Make-up exams: an exam that a student missed but has permission to reschedule and complete the test Exit exams: a state required exam in order to pass a course. Remediation courses require the student to complete and pass a state exit exam. Commercial exams: exams administered for local businesses. Local Testing Procedures Proctored testing activity is a collaborative effort between faculty and the Open Campus Assessment Center. To begin the proctored testing process, Assessment Centerstaff sendsout a welcome letter to faculty several weeks before the term begins requesting their preferred testing dates. Once this information is collected, it is recorded on a planning spreadsheet. If a course shell requires pencil/paper exams, a copy of the exam shouldbe sent via e-mail (as an attachment) to the Assessment Center two weeks before the date the test is to be administered. If the test is computer based, the instructor sends the testing access password along with backup tests to the Open Campus Assessment Center. Back-up paper/pencil tests are reserved for times when there is a problem administering a test or exam via the computer. 1|Page
  • 4. Remote Site Testing If students do not live in Nassau, Duval, St. Johns, Clay, Bradford, Union, or Baker County, they are provided remote site testing accommodations. The Open Campus Assessment Center facilitates remote site testing services providing students with the opportunity to attend Florida State College from anywhere in the world while taking assessments near their home. Remote test site students are asked to complete a Remote Test Site and Proctor Form. These forms are available on the Distance Learning Website http://www.fscj.edu/mydegree/distance- learning/testing/assets/documents/OnlineRemoteProctor.pdf. Once a remote site test form is received in the Assessment Center all information is verified by an Open Campus Assessment Center staff member. Afterthe qualified testing environment and proctor are verified and approved, the student will receive an e-mail confirmation along with testing information. Should the student’s desired testing location not be approved, Test Center staff will select an appropriate remote test proctor near the student’s designated Zip Code. Make-up Exams Students who miss a scheduled exam date must contact their professor for next steps. Instructors should note make-up testing policy and procedures clearly in the course syllabus at the beginning of each term. It is the student’s responsibility to know their instructor’s make-up/retesting policies. Make-up exams must be taken no later than the week following the regularly scheduled exam. (Specially assigned Exit Testing dates/times are the exception.) Both instructor and student have make-up scheduling responsibilities that include the following: Instructor responsibilities Once student has informed instructor about a missed exam in a timely manner, and the instructor agrees to a make-up exam, the instructor will send this information to the student: Go to this web site:http://www.fscj.edu/onlinetesting Select “Special OC Testing” Select desired test date and time The instructor will also send an e-mail to assessme@fscj.edu with the following information included: Student name Course name Course reference number Exam name that student is taking If needed, a make-up exam must be sent at this time as well 2|Page
  • 5. Student responsibilities If a scheduled exam or test has been missed, it is the student’s responsibility to contact their instructor as soon as possible and request a make-up exam. The student will follow the procedures provided by the instructor for scheduling their make-up exam. Students are responsible for bringing their testing confirmation information with them on the scheduled testing date at the Open Campus Assessment Center. Exit Exams TheOpen Campus Assessment Center coordinates and communicates testing activity related to state and College mandated exit examinations. Students taking an Exit Exam for ENC0025, MAT0018, MAT0028 and/or REA0017 must schedule and complete their Exit Exam on the dates/times provided by the Open Campus Assessment Center. The Open Campus Assessment Center assigns specially designated Exit Testing dates/times to meet college mandated Final Grade deadlines and to comply with college Institutional Effectiveness standards. Faculty are responsible for communicating Exit Testing dates/times to their students and ensuring that all Exit Testing students complete their Exit Exams on the scheduled date/time. Open Campus faculty living in Jacksonville administer initial exit exams to their students. If a student misses their initial exit exam,they may be able to schedule a make-up exam. Instructor approval is needed for a make-up or retest for an exit test. Exit testing make-ups and retests are only offered at the Open Campus Assessment Center. Open Campus faculty living outside of Jacksonville are assigned a Test Examiner to streamline the Exit Test process. The Test Examinerfacilitates communication with the instructor about exit testing schedules and exit testing procedures for students to follow. Once a student has scheduled an exit exam appointment, they will receive e-mail confirmation that must be shown to Assessment Center Staff at the start of a testing session. Services for Students with Disabilities The college fully complies with the Americans with Disabilities Act (ADA). Students who require accommodations for online courses should contact the Office of Services for Students with Disabilities to arrange services. Student feedback is an important part of the Open Campus Assessment Center continuous improvement effort. Blackboard Maintenance There are limited times in each term during which the Blackboard Course Management System is unavailable due to maintenance and upgrades. The times and dates of scheduled maintenance are posted on the Blackboard home page at https://bb.fscj.edu/. Faculty should be aware of and remind students of Blackboard outage dates. 4|Page
  • 7. Testing Site Guidelines and Procedures The following items outline expectations and requirements for all testing locations. 1. Students are required to present valid government-issued photo identification with signature. 2. The name of the student must match the identification name. 3. Students must present e-mail test confirmation for MAT 0028 and MAT 1033. 4. Materials or supplies are not admitted into the testing room. ONLY materials provided by the proctor (i.e., paper and pencils) or allowed by the instructor (calculators, pocket dictionary, etc.) are permitted in the testing area. 5. No food or drinks are allowed in the testing area. 6. Students may only use materials provided by the proctor, unless the instructor has authorized supplemental testing materials. 7. Students are not permitted to leave the testing area once the test/exam begins. Students are only permitted to leave the testing area to use the restroom when necessary, and only one student is permitted to leave at a time. 8. All electronic devices must be turned off during testing. 9. All scratch paper, used or unused, are to be returned to the proctor along with any other testing items. 10. Students requiring special needs accommodations must maintain the proctor prior to testing. 11. No children are permitted in the testing area. 12. A quiet testing atmosphere is expected at all times. No talking or excessive noise is allowed during testing periods. Earplugs are available upon request. 13. All tests must be completed prior to closing. 5|Page

  • 9. Appendix B: Frequently Asked Questions Frequently Asked Questions 1. I am a local student. Where and when can I take my proctored exams? Local students can choose from different campus testing locations if the exam is pencil/paper: Open Campus, South, Kent, and Nassau. Computer-based exam students can choose from certain campus testing locations –Open Campus, South, Deerwood, Kent, and Nassau. Students may also choose from different testing times and dates. 2. Is there any cost? No. 3. If I am not a local student, how and where can I take my proctor exams? Remote students can take their exams with approved test proctor near their Zip Code. Students should review their course syllabus for more information on remote testing. 4. Are appointments required for testing? Appointments are required for testing. Students with appointments are always tested first. Walk-ins may be tested only if there are available tests, seats, and time. 5. What if I have an emergency and cannot test when I scheduled my appointment? The student can ask their instructor whether a make-up exam is permitted. Only the instructor has the authority to allow make-up tests. 6. What information do I need to bring with me at my testing location? Students must present valid government-issued photo identification with signature at the time of testing. Active duty military may use their military ID card in order to test. 7. What happens if I arrive late at my site? It is possible that students could be turned away and need to reschedule. If within the allowed testing week, students may be able reschedule an appointment at another testing location. Not all professors will permit students to take a make-up exam. 8. Is my exam paper/pencil or computer based? This information will be provided in each course syllabus. 9. How much time do I have to take my exam? Time allotment is at the discretion of the instructor or course shell. 7|Page
  • 10. 10. May I use a calculator? Some exams do allow a calculator. Individual instructors will furnish the Assessment Center with those instructions. 11. What type of exam will it be? Multiple choice, fill in the blank etc.? The choice for the type of exam is the instructor’s decision. 12. Where is the Open Campus Assessment Center located? 601 W.
