3. Tipe imunogen yang digunakan untuk imunisasi 1. Live attenuated/kuman hidup yang dilemahkan : Virus : measles, mumps, rubella,OPV,Yellow fever, Varicella, RSV, Parainfluenza* Bacteria : Oral typhoid, BCG, Tularema 2. Inactiv/mati : Virus : Influenza, Polio Salk, Rabies, Hepatitis A. HPV* Bacteria : Anthrax, Pertussis, Plague, Typhoid and paratyphoid Coxiella burnetii, Gonococcus* 3. Sub unit : tetanus, diphteria, pertussi, Hib, Penumococcus, Hepatitis B, DNA*
4. Hepatitis B immunoglobulin Source : Human Indications and dose : Acute exposure to blood containing HBsAg, 0.06 ml/kg BW within 24 hour Perinatal exposure of infants born to HBsAg Pos mothers, 0.5 ml within 12 hours of birth Sexual exposure to HBsAg pos persons, 0.06 ml/kb BW within 14 days after sexual contact Household exposure to persons with acute HBV, infant 0.5 ml Dose : By I.m injection Cautions and contraindications Prior systemic allergy to human Ig ;Severe thrombocytopenia Coagulation disorder, pregnancy Side effects : Local pain & tenderness, allergic reactions
5. Live Inactivated Replikasi Yes No Dosis Small Higher Jalur Natural Injection Efek samping More Less Kemampuan mutasi Yes No Live vs inactivated vaccine
8. Rabies immunoglobulin Sumber : manusia Indikasi : setelah terpapar dengan virus rabies Dosis : 20 iu/kg BW setiap kali injecsi Efek samping : nyri, alergi ringan Catatan : biasanya diberikan bersamaan/berselingan dengan virus hidup yang dilemahkan (imunisasia aktif)
18.
19.
20. Poliomyelitis vaccine Latar belakang : menyebabkan polio dengan berbagai tingkatan infeksi. WHO : menurunkan infeksi polio melalui program Oral Polio Vaccine ( OPV ) Indonesia sebelumnya sejak tahun1995 tidak ada lagi dilaporkan kasus polio, tapi dua tahun belakangan ini ditemukan kembali vaksinasi : Live attenuated ( OPV ) , ( Sabin). Dapat merangsang terbentuknya Ab dalam waktu lama Killed vaccine ( IPV, injected s.c ), ( Salk). Vaksin baru yang lebih potensial
21.
23. Hepatitis B vaccine Latar belakang : Hepatitis B tersebar diseluruh dunia Indonesia adalah daerah endemic HBV vaksinasi Subunit, HbsAg merespon terbentuknya Ab sekitar 90 % Indikasi Orang yang belum memiliki respon imun sangat dianjurkan pada org yang beresiko tinggi (pekerja kesehatan, pasiens ygmembutuhkan tranfusi darah, homo/hetero seksual, pengguna i.v drug users, sex workers, sexual contact of HBV carrier, menmgunjungi daerah endemik.
24.
25. Post exposure Hepatitis B vaccination Exposure HBIG Vaccine Dose Timing Dose Timing Perinatal 0.5 ml im < 12 hrs of birth 0.5 ml im <12 hrs of birth Percutaneus 0.06 ml/kg im < 12 hrs 1.0 ml im sesegra mungkin Sexual 0.06 ml/kg im <14 days contact For homosexual and regular sexual contact of chronic HBV Household - - Same as pre exposure contact vaccination
26. Diphteria-Tetanus and Pertussis ( DTP ) vaccine Background Diphteria ( kulit dan saluran pernafasan tempat inokulasi bakteri. Memberikan kekebalan selama 10 th tetanus disebabakan oleh toksin, memberikan kekebalan 10 years perinatal tetanus is fatal pertusis, ditularkan melalui jalan nafas, kekebalan alami dapat terbentuk dan berlangsung lama. 20% penerima vaksin kehilangan imunitas setelah berusia 23 th, 90% setelah 19 th
27. DTP vaccine Vaccine : Two form of pertussis vaccine, whole-inactivated cells and acellular vaccine. Whole-inactivated vaccine account for higher rates of adverse effects in adults and children of age of 2 years or more Available as DTP, DT or single antigen product Vaccine administration Starting from 2-3 months of age. Primary vaccination consist of three doses, given 4-8 week apart. For adult and school children, the 3 dose is given 6-12 months apart from the 2 dose. Booster is given every 10 years. As for traveler, recommended tetanus booster is given 5 years apart
28. DTP vaccine Cautions Previuos allergic or neurological reaction to vaccine Patients receiving immunosuppressive within 3 months .
29. BCG vaccine Background : TB is a chronic diseases leading to a great economic loss in endemic countries Indonesia is on the 3-4 highest position in term of number of TB patients in the world Vaccine : BCG ( Bacillus Calmette-Guerin ), an attenuated of Mycobacterium bovis . Conflicting vaccine efficacy from 0-75 % for pulmonary TB in adult High efficacy for preventing meningitis and disseminated TB ( miliary TB ) in children. Effective for prevention of primary TB and endogenous reinfection, not for exogenous reinfection
30. BCG vaccine Caution and contraindications : 1. HIV infected persons 2. Other immunodeficient persons Side effects ( strain variation and inaccurate injection ) Induration and ulceration at the vaccination site Regional suppurative Lymphadenitis & Osteitis
