2. Wound Mender Description:
The Wound Mender system is a mobile health care application designed for the iPhone.
The website, www.woundmender.com, and backend database are used to store and
distribute pressure ulcer risk assessment data uploaded from the mobile devices.
The website is used by both patients and health care teams. Patients can create an account
to access their pressure ulcer risk assessment data and also view which health care
providers have access to their data. Health care provider’s accounts link their patients and
mobile devices together. The health care team’s mobile devices must be linked to a
provider’s account in order to download or upload their patient’s pressure ulcer risk
assessments.
Completed pressure ulcer risk assessments are immediately available for downloading
through the Wound Mender website and registered mobile devices. The assessments can
be downloaded through the website as Clinical Document Architecture (CDA)
documents. The documents contain individual pressure ulcer risk assessments coded
using LOINC® and Systematized Nomenclature of Medicine Clinical Terms (SNOMED
CT) and a Base64 encoded zip archive containing images that were captured using the
mobile device’s camera or other sources.
Data can be entered through the mobile device for the following pressure ulcer risk
assessment categories

Braden Scale, Braden Q (PICU), and Neonatal Braden (NSRAS)

Medications
o RxNorm Drug Search

Mobile Device Features
Health care teams can search for patients and download their past pressure ulcer
risk assessments.
Uploaded data is immediately available for downloading to other registered
mobile devices.
The mobile device application does not require a constant network connection to
function. Only data syncing and RxNorm drug searches require a network
connection.
Application interface guides the medical personal through the collection of the
data
Images of pressure ulcers can be captured using the camera on the device and then
linked to the patient’s risk assessment.

.assessments, patients, health care providers, and mobile device registrations
Website and database can operate independently or be linked to a provider’s
electronic medical record system to extract relevant data.
Website and database are scalable to support additional users and devices.
