        Summary of benefits and coverage (SBC), is a four page summary 
        of the benefits available under a groups health insurance plan.  
        The purpose of this document is to provide individuals with a 
        better understanding of the health coverage offered under a plan 
        and a means for comparing various health plans from different 
        sources (employers, private exchange or State exchange).  This 
        document will be provided for all new hires after September 23. 
        2012 and to all employees for renewals begining October 1st 2012
