1- TRATAMIENTO PERCUTÁNEO DE LA HIDATIDOSIS HEPÁTICA
La hidatidosis es una enfermedad parasitaria que afecta al hígado con relativa frecuencia
en nuestro medio. Consiste en el crecimiento en el humano (huésped accidental) del
metacestode del Equinococus granulosus. Ése se contamina al ingerir los huevos
excretados por el helminto en la materia fecal del perro (huésped definitivo), que a su vez
adquirió el parásito por la ingesta de quistes de las vísceras del huésped intermediario
(oveja u otro herbívoro) 17, 157.
El quiste hidatídico (QH) está compuesto por el tejido de granulación y fibrosis del
huésped, que lo encapsula (periquística) y el parásito. Éste posee dos capas, que desde el
interior al exterior son:
-Germinal o prolígera, de donde nacen las vesículas hijas. Secreta el líquido
hidatídico, en donde flota la arenilla hidatídica, rica en escólices con capacidad de
desarrollar nuevos quistes si contamina otros tejidos.
-Cuticular, membrana acelular de escasa permeabilidad que forma una barrera que
defiende al parásito del ataque inmunitario del huésped.
El QH puede se unilocular (sin septos, sin vesiculización, pared lisa y regular, líquido
claro), multilocular (con septos, vesículas, poco líquido). Estas características tienen
importancia desde el punto de vista terapéutico.
A su vez, el QH puede estar activo (generalmente sintomático, crece, es inmunógeno y
se puede complicar, tiene el líquido claro y a alta presión 181) o inactivo.
La clínica del QH no complicado es variable. Puede ser asintomático, y constituir un
hallazgo ecográfico o tomográfico. Los síntomas cuando se presentan son: dolor en el
abdomen superior, dispepsia, y se llega a palpar la masa si ésta se proyecta en el
hipocondrio o epigastrio.
Las complicaciones posibles son : infección, compresión de estructuras vecinas (vía
biliar, vena cava, etc) y ruptura: a la luz de la vía biliar (produciendo colestasis), al tórax (se
puede manifestar por vómica), a la cavidad peritoneal (produciendo reacciones alérgicas
que pueden llevar al shock anafiláctico, con el riesgo de implante secundario en peritoneo)

El diagnóstico se puede confirmar con la serología (ELISA, Western Blot, arco 5,

aunque, debido a múltiples factores, ésta puede no ser sastisfactoria .

La ecografía , la tomografía computada , y en menor grado la resonancia

magnética son los métodos de imagen que en general confirman el diagnóstico y además
permiten hacer el tratamiento percutáneo y el seguimiento evolutivo luego del mismo. Gharbi
clasificó los QH por su aspecto ecográfico 157 (Tabla 1).

Esta clasificación es útil porque en ella se basan la mayor parte de los autores para
decidir la indicación de abordaje percutáneo.
En caso de duda se puede realizar una punción, que sirve para certificar definitivamente

el diagnóstico de QH en casos dudosos .

El tratamiento médico con mebendazol o albendazol no es
necesariamente curativo, y tiene sus indicaciones limitadas (pacientes que se niegan a la
cirugía, QH inoperables, alto riesgo quirúrgico, asintomáticos)157.
El tratamiento quirúrgico convencional (resección del parásito, del quiste completo,

hepatectomía) sigue vigente, aunque algunos autores ya lo
consideran de segunda elección, o reservado al manejo de QH grandes, múltiples,
complicados o recurrentes 212.

Se ha intentado, con éxito en casos seleccionados, el manejo videolaparoscópico .
Con este abordaje las maniobras son similares a la realizadas en cirugía abierta.
Tradicionalmente se consideraba contraindicada la punción de los QH porque podían

producir shock anafiláctico o derrame del contenido y posterior implante peritoneal , tal

como ocurre cuando éstos se rompen espontáneamente . Algunos reportes de punciones
accidentales a principio de los 80, y los trabajos experimentales de Ben Amor realizando en

ovejas (1983) fueron los precedentes del primer trabajo sobre el tratamiento percutáneo
en humanos, realizado en 1985 por Mueller y cols.148, que informaron sobre el tratamiento
exitoso de un QH hepático secundario en una mujer. De ahí en más se suceden múltiples

estudios en donde aparecen variantes técnicas y comienza a acumularse una casuística
importante en cuanto al número de pacientes tratados, resultados obtenidos y seguimientos
a largo plazo. Incluso autores como Saremi han ampliado las posibilidades de la técnica

introduciendo instrumentos que permiten la trituración de tabiques y membranas .
El uso del mebendazol y del albendazol, que suministrados en el pre y postoperatorio
profilácticamente, marcó un hito importante en el tratamiento médico, quirúrgico y
percutáneo de esta enfermed 100, 144, 152, 187.
La efectividad del uso de solución salina hipertónica como escolicida ha sido
demostrada 6, y es la sustancia que generalmente se utiliza, aunque hay trabajos que

recurren a otros como el nitrato de plata . Asimismo el alcohol posee un efecto

esclerosante conocido . Hay que recordar, no obstante, que ningún escolicida es

totalmente seguro, existen reportes de colangitis en cirugía , aunque aún ninguno con el
tratamientio percutáneo. Hay trabajos que intentan reemplazar estas sustancias por otras
con menos potencial cáustico, como el mebendazol o albendazol 55, 157.

Los excelentes resultados obtenidos a corto plazo , que se mantuvieron

cuando se fueron acumulando series con años de seguimiento , y las

ventajas frente al tratamiento quirúrgico , llevó a que esos autores fijaran este
procedimiento como una alternativa válida a la cirugía, y posteriormente incluso a indicarlo
como de primera elección 58, 233.
Existe evidencia histopatológica del éxito del tratamiento percutáneo 6, que además ha
demostrado cumplir con los criterios de curación del QH: inactivación del parásito,
eliminación del efecto de masa y prevención de las complicaciones 142.
Indicaciones: Los criterios de inclusión están determinados principalmente por la

clasificación de Gharbi (Tabla 1). Así, hay consenso en tratar percutáneamente los tipo I,
II, III (con pocas vesículas hijas). Se contraindica en los tipo III (con múltiples vesículas y
material sólido), IV (masas heterogéneas) y V (paredes calcificadas), debido a la
imposibilidad de drenar el contenido, de la acción inhomogénea del escolicida y la falta de
colapso de la cavidad residual 57, 68, 69, 142, 223.
La existencia de varios quistes no contraindica el método 142, 223.
La presencia de componentes exofítico y de conductos periquísticos dilatados son
contraindicaciones de tratamiento no quirúrgico, la cirugía no debe ser postergada ante tales

signos . La ictericia y dilatación de la vía sugiere la ruptura a la vía biliar, son excluidos
esto QH por riego de colangitis 233. Las coagulopatías y los quistes en sitios inaccesibles son
obvias contraindicaciones.
Los pacientes que se niegan a la cirugía, que no pueden recibir anestesia general, los
tratados con albendazol que no responden, pueden ser abordados percutáneamente 157.

En cuanto a la edad, hay quienes fijan un límite en los 18 años , pero varios autores

que han aplicado con éxito el tratamiento en niños . El embarazo no es una

contraindicación .
Como todo procedimiento quirúrgico, debe ser realizado en centros especializados, con
equipos entrenados en el abordaje percutáneo de patologías abdominales142.
Técnica: Existen distintas variantes del método:

a) Punción, aspiración, instilación y reaspiración (PAIR) . Es
el método que se utiliza corrientemente para quistes de menos de 6 cm de diámetro, que no
necesitan escleroterapia para provocar su colapso. Sus pasos fueron fijados por Ben Amor y
cols. 63, y son, con algunas modificaciones, los que más adelante se desarrollan.

b) PAIR + cateterización . Es la forma más ususal de tratar los QH de más

de 6 cm, en línas generales es la descripta por Akhan y cols. . El PAIR con o sin
caterización es el procedimiento que más se utiliza y con el que existe una mayor
experiencia, por lo cual es el que se describe con más detalle, recurriendo a distintos
autores.

c) Punción y aspiración (PA) . Cuando se detecta al realizar la PAIR con o sin
cateterización bilis o una fístula en el cistograma el procedimiento queda inconcluso,

dejando un drenaje que se retira cuando el débito es menor a 10 ml/d . Semanalmente se
deben realizar controles ecográficos y análisis del líquido.

d) Doble punción, aspiración e instilación (D-PAI) . Giorgio y cols. han ensayado este
método con éxito en quistes uniloculares. Consiste en punzar, aspirar e inyectar alcohol, sin
reaspirar. Al tercer día se repite el procedimiento.

e) Mebendazol – PAIR (M-PAIR) . Erzurumlu y cols. en 1996 presentaron el primer caso
tratado percutáneamente de esta forma (y otro por videolaparoscopia). A un tratamiento con
mebendazol VO (50 mg/kg/d por seis meses) se le suma la inyección de mebendazol en el
QH (en una solución con 2,4 microg/ml).

f) Punción, curetaje o cánula de tripsia , drenaje. Estos métodos pretenden no solo
drenar los componentes líquidos del QH, sino también los sólidos (membranas, tabiques,
etc.). Saremi y Mc Namara lo aplicaron a quistes tipo I a V, e incluso infectados. Usando un

sistema coaxial, se introduce un instrumento de aspiración y corte, y se elimina totalmente
el contenido del QH, dejando un drenaje hasta que disminuye su débito a 20 ml/d. A la
segunda semana se realiza un cistograma, donde no debe haber restos de membranas ni
vesículas.
Punción, aspiración, instilación y reaspiración (PAIR)

1. Un mes antes, y otro después del tratamiento , albendazol 15-20 mg/kg en dos
tomas VO diarias (con las comidas).
2. Paciente en ayunas, canalizado, y controlado por el anestesista (en adultos se utiliza
anestesia local asistida, en pacientes pediátricos, anestesia general).
3. El ámbito donde se realiza debe disponer de ecógrafo o tomógrafo, intensificador de
imágenes, agujas y catéteres. Es indispensable disponer de elementos para hacer frente
a complicaciones como el shock anafiláctico y un quirófano y equipo quirúrgico
disponible en caso de necesidad de intervenir quirúrgicamente.
4. Inyección EV de maleato de feniramina (50 mg en adultos, 5 mg/kg en niños) u otro

antihistamínico (la dosis se repite a las 12 hs) . Se puede agregar 200 mg de
hidrocortisona EV 142.
5. Bajo guía ecográfica o tomográfica, punción a través de tejido hepático sano y
procurando acceder en la zona menos declive del QH (la más rica en escólices), con una
aguja de 18 a 20 F 142. La vía de acceso puede ser anterior, lateral o intercostal 157.
6. Aspiración del la mitad del contenido líquido del QH. Este material es centrifugado a
1500 rpm y teñido con rojo neutro al 0,1 % (para detectar organismos vivos) y azul de

metileno al 0,03 % (para organismos muertos) . De esta forma se confirma el
diagnóstico y se controla la vitalidad del QH. Además se debe dosar bilirrubina, para
detectar comunicación con la vía biliar. En caso de detectar bilirrubina, no se puede
inyectar el escolicida.
7. Inyección de solución salina hipertónica (al 20 %) en un volumen estimado en un 70 %
del líquido aspirado 223. Se deja actuar el escolicida 10 157 a 20 142 minutos (fig. 1).
8. Aspiración de todo el contenido, luego de observar el despegamiento de las membranas

internas del QH (fig. 2 B). Algunos autores consideran este signo como
patognomónico del QH, aunque otros autores, al observarlos en quistes simples, no

concuerdan con esa opinión . Se puede volver a realizar el control de vitalidad con las
tinciones antes mencionada.
9. El paciente permanece internado una noche, luego de la cual puede ser dado de alta.

10. El seguimiento de la evolución será clínico, serológico, y fundamentalmente ecográfico
(al 1º, 3º, 6º, 12º y 18º meses) y tomográfico (anual) en busca de recurrencia local o
diseminación secundaria 142 (fig. 2 C y D, fig. 3 D y E, fig. 5 C y D).
PAIR + cateterización
Luego de realizar la PAIR, cuando el quiste es de más de 6 cm, se continúa con los
siguientes pasos:

1. Después de aspirar, se coloca un catéter tipo pigtail de 6 a 9 F en la cavidad, y se
deja abierto (no aspirativo) por 24 hs.
2. Se realiza el análisis del líquido drenado (vitalidad, bilis), y una cistografía inyectando un
50% del volumen inicial del QH de medio de contraste, guiado fluoroscópicamente (fig.
nº 4 y 3 B y C). Si se detecta bilis o se demuestra en la cistografía una comunicación con
la vía bilar no puede inyectarse el esclerosante. La presencia de estructuras vasculares
importantes cercanas demandan la abstención del uso del alcohol 142.
3. Aspiración e inyección de alcohol al 96 % (en una cantidad igual al 50 % del volumen
inicial del QH). Se deja actuar por 20 minutos.
4. Aspiración de todo el contenido del QH, lavado con solución salina (0,9 %) y extracción
del catéter.

Resultados: La recurrencia del QH es atribuída a infraexposición al agente escolicida por
las irregularidades y ondulaciones del endoquiste, con bolsillos con contenido de parásitos
223
vivos con reducido tiempo de contacto . El tiempo de recurrencia estimado es entre 6 y 36
meses. Los indicadores de esta situación (la serología no es concluyente) están dados por
el seguimiento eco o tomográfico. La persistencia de la forma redondeada, el aspecto
anecoico, la regularidad y delgadez de la pared quística y la ausencia de membranas rotas
durante los seguimientos ecográficos de rutina, son signos de recurrencia. El cambio con el
tiempo de quístico a sólido, con un remanente final de patrón sólido seudotumoral es un

signo de curación ampliamente aceptado . No obstante hay trabajos con de más de 5
años en donde el patrón seudosólido se describe en solo 26 de 362 pacientes 180.

No hay casos informados de diseminación. Quizás esto se deba a una exposición

mucho menor al fluido del quiste , y al uso de albendazol, efectivo en la profilaxis de

hidatidosis peritoneal experimental . Sí existen casos de derrame peritoneal del líquido

(después de la infusión de escolicida) que obligó a prolongar por 3 meses la profilaxis con
albendazol, sin aparición de hidatidosis peritoneal 142.

La morbilidad temprana del tratamiento percutáneo es del 0 al 71,4% . Lo que se
describe más constantemente es fiebre sin infección, y reacciones menores de
hipersensibilidad (que responden al tratamiento sintomático). La fiebre que se detecta
inmediatamente después de la intervención se acepta como una reacción inmune, por
mediadores pirógenos provenientes de los productos de degradación de los tejidos inducida
por la solución hipertónica, desaparece en 12 hs 233.
Mayor trascendencia tienen la fístula biliar, la infección de la cavidad (que responde
normalmente al tratamiento antibiótico, aunque a veces requiere drenaje), las reacciones
anafilácticas y el derrame pleural (que en general cede espontaneamente).
Las fístulas se presentan en el 1,7 al 6,2%142 de los casos, mientras que con cirugía
conservadora se manifiesta en el 14,1 al 22,3%, y si se trata de cirugía radical, en el 0 al 7%

En trabajos como el de Saremi, en donde los componentes sólidos del quiste son
evacuados el porcentaje de fístulas es mayor: 34,4%183. Esta observación indicaría que
dejar componentes sólidos en la cavidad colapsada ocluiría las posibles fístulas, que sólo
ganan patencia después de la completa descompresión de la la cavidad. Hay que destacar
que aunque haya bilis en el drenaje sólo se aprecia fístula en la ERCP en el 54% (quizás las

fístulas son comunicaciones muy pequeñas). Otro dato importante es que no se han
descripto casos de colangitis esclerosante, situación que apoya la aseveración anterior.
Hay que usar fluoroscopia de rutina y dejar por 24 hs catéter en los quistes grandes
para dar tiempo a que las comunicaciones se manifiesten142 con los cambios en los
gradientes de presión: Los conductos previamente no comunicantes que rodean al quistes
se convierten en un área de baja resistencia donde los conductos pueden drenar, si estos
cambios de gradiente afectan grandes áreas, aumentan las posibilidades de fístula en
quistes grandes (fig. 4).
Hay evidencias de reacciones alérgicas con el tratamiento percutáneo, pero suelen ser
menores (urticaria) 63, 2, 102, 142.

Las reacciones anafilácticas seguidas de muerte se han descripto en cirugía y
se sabe que la ruptura espontánea puede 106 o no puede 1, 102 producir anafilaxia.

No hay una premedicación establecida de la anafilaxia, pero un estudio prospectivo
demuestra que los antihistamínicos parecen efectivos.
Otras complicaciones reportadas son: Ruptura del quiste a la vía biliar, en un caso sin
evidencia de fístula, 2 años después del tratamiento, atribuída a necrosis de la pared
quística (confirmada histopatológicamente), reacción vasovagal 183, fístula con la vesícula 59.
Comparado el tratamiento percutáneo con la cirugía revela que ésta tiene una

morbilidad del 8 al 80% (complicaciones infecciosas, fístula biliar,
complicaciones pulmonares, hemorragia intraabdominal, colangitis esclerosante), mientras

que la morbilidad del tratamiento percutáneo no es mucho mayor del 20% . Ya se
mencionaron índices menores de recidiva y fístula con el tratamiento percutáneo.

La hospitalización con tratamiento percutáneo es de 1 a 17 días (ésta última cifra en
82,134
casos complicados), con cirugía de 15 a 40 días .
Las cirugías repetidas son dificultosas (por adherencias, fibrosis), implican mayor riesgo
anestésico; no se han reportado riesgos adicionales por sesiones repetidas de tratamiento
percutáneo. El número de QH condiciona la duración de la cirugía, a diferencia del
tratamiento percutáneo que permite tratar muchos QH con una mínima prolongación del

procedimiento . Ventajas adicionales del tratamiento percutáneo es el resultado estético,
la ausencia de morbilidad parietal y rápida reincorporación a las actividades habituales.
No obstante estas ventajas del tratamiento percutáneo, la cirugía sigue siendo el método
electivo para tratar QH que escapan a las indicaciones del tratamiento percutáneo o éste
está contraindicado.
