FBI FD-258FA - FFL Application ORI WVATF1100 – 2 Fingerprint Cards (order TWO cards per person for each person you hope to add to your FFL License, default ATF normally sends 4 cards}<br /> Once you have applied for these forms, it takes about a week before it arrives to your stated address.
