8. Background of the Study:
Nursing process as the intellectual basis of the nursing profession
in solving patient’s problems is defined as a systematic method of
planning and rendering care to the patients. This process has been used
by the nursing profession since it was articulated in the 1960’s. This
intellectual basis of care was taught in the nursing education as a
thinking guideline in solving patients’ problems. For the past years,
writing of student nursing care plan has been a part of the teaching-
learning tool of the nursing education to prepare the students in the
application of nursing process in the nursing practice. However,

problems sought in the evaluation of these nursing care plans were
revealed by the different authors on teaching strategies in nursing. It
was revealed that nursing care plan writing of the students was a
misnomer, written only after the care has been given and students
resort to copying already written standardized care plans in their
nursing text books. Evaluation on the written care plans depends on the
preference of the clinical instructor, which created further confusion on
the students. To settle these arguments a study leading to the proposal
of common assessment guidelines in the preparation and evaluation of
student nursing care plans was conducted.
9. Statement of the Problem:
This study critically analyzed the student nursing care plans from
the different schools of Nursing in Dagupan City. The study had focused
on the application of Nursing Process in the written student nursing
care plans along the phases of: assessment, diagnosis, planning,
implementation and evaluation. Results of the study served as the
groundwork in the creation and proposal of common guidelines in the
preparation and evaluation on the nursing care plans of student nurses.
Specifically, it answered the following questions:
1. To what extent does the level of accomplishment of the student
nursing care plan entries meet the requirements of nursing process
along the following areas:

a. Assessment;
b. Diagnosis;
c. Planning;
d. Implementation; and
e. Evaluation?
2. What are the differences in the level of accomplishment of student
nursing care plan along the:
a. Five areas of the nursing process; and
b. Different participating schools?
3. What common guidelines can be proposed in the preparation and
evaluation of the student nursing care plans?
10. Research Method Used: Descriptive- Evaluative Method
11. Summary of Findings:
The study findings were as follows:
I. Level of Accomplishment of Student Nursing Care Plans along the
Requirements of the Nursing Process
Assessment- The analyzed student nursing care plan entries had least
accomplished the indicator requirements of nursing process in the area of
assessment with an average weighted mean of 1.96.

Diagnosis- The analyzed student nursing care plan entries had least
accomplished the indicator requirements of nursing process in the area of
diagnosis with an average weighted mean of 2.09.
Planning- The analyzed student nursing care plan entries had least
accomplished the indicator requirements of nursing process in the area of
planning with an average weighted mean of 2.04.
Implementation-The analyzed student nursing care plan entries had least
accomplished the indicator requirements of nursing process in the area of
implementation with an average weighted mean of 1.93.
Evaluation- The analyzed student nursing care plan entries had least
accomplished the indicator requirements of nursing process in the area of
evaluation with an average weighted mean of 1.79.
II. Differences in the Level of Accomplishment of the Student Nursing
Care Plan Entries
Five Areas of Nursing Process -There were no differences found in the level
of accomplishment of the student nursing care plan along the five areas of
nursing process. All the indicator requirements of the five areas were least
accomplished as reflected on the analyzed student nursing care plan
entries, with a pooled mean of 1.9.

Different Participating Schools-Out of five participating nursing schools only
the HEI-D had not accomplished the indicator requirements of nursing
process along the five areas, with a pooled mean of 1.36. On the other
hand, four out of five participating schools had least accomplished the
indicator requirements of nursing process. Results pooled mean of the four
participating schools that had least accomplished the indicator
requirements of nursing process were as follows: (HEI-A =1.89), (HEI-B=
2.10), (HEI-C= 2.25) and (HEI- E= 2.03).
12. Conclusions:
With the above cited findings in this study, conclusions were drawn
as follows:
1. Student nursing care plan entries had least accomplished and not
accomplished the requirements of nursing process along the areas of:
assessment, diagnosis, planning, implementation and evaluation.
2. The least accomplishment and not accomplishment of the student
NCP entries along the requirements of nursing process were a result
of undefined guidelines in the preparation of student nursing care
plans.

13. Recommendations:
Based on the above findings and cited conclusions of this
study the following recommendations were drafted.
1. A dialogue among the faculty of nursing programs should be
initiated to come up with a common consensus on how nursing
process will be taught and nursing care plans will be used as a
written assignment in the clinical area. Such discussion will be
beneficial and provide a consistent message to the student nurses.
2. A try-out on the proposed common guidelines in the preparation
and evaluation of student nursing care plan is recommended to
the nursing teaching-learning groups.
3. A more in-depth study should be conducted to validate the results
of the present study.
