  1. Content of Narrative Project History Provide a narrative description of the historical background of the solid waste disposal facility requiring investigative or corrective action.  Project Need Provide a written narrative appropriately justifying the need of the proposed project for financial assistance. Include necessary supporting documents with the narrative (including any letters from TDEC concerning ground water contamination or potential problems at this site. Provide a clear description of the conditions at the Class I landfill or of the site which require investigative or corrective actions and the goal to be achieved through this project. Technical Assessment and Maps Provide a description of the potential sources of contamination including sufficient evidence that a release or substantial threat of a release of contaminants from the eligible facility does exist. Include any letters from TDEC concerning ground water contamination or potential problems at this site. Provide a copy of the U. S. Geological Survey (USGS) Topographical Quadrangle Map of the area. The property boundaries of the proposed site and the disposal area should be depicted on the map. Maps must show boundaries of residences within proximity of the landfill. Adjacent topo maps may be attached, if necessary  Project Plan Provide engineering and corrective action plans, if applicable, identifying all corrective actions that need to be conducted at the eligible landfill site. Plans must be designed and certified by an independent professional engineer registered to practice in Tennessee Provide a description of proposed monitoring plans for the corrective actions outlined.  Budget/Funding Requested Provide the total amount of funds requested for the proposed corrective actions. Also, include a breakdown of the cost estimate for the funding request proposed. All items should be consistent with the project plan in Item 5 above.  Timeline Provide a timeline for the completion of the project with key milestones identified.
