Pediatric Trauma
• Pediatric trauma is the leading cause of death
among children older than 1 year.
– MVC’s cause most deaths
– Falls and submersions are next
– Homicides and suicides are also common among
adolescents
Pediatric Patient Assessment
• Differs from adult assessment
• Adapt your assessment skills.
• Have age-appropriate equipment.
• Review age-appropriate vital signs.
Scene Size-Up
• Take appropriate standard precautions.
• Note child’s position.
• Note pills, medicine bottles, alcohol, drug
paraphernalia, or household chemicals.
• Do not discount the possibility of abuse.
Primary Assessment
• Use the Pediatric Assessment Triangle to form
a general impression.

• Appearance
– A child with a
grossly abnormal
appearance requires
immediate life-
support
interventions and
transportation.

– Determine
adequacy of
cardiac output
and core
perfusion.
Primary Assessment
• Stay or go
– Use findings from PAT to determine whether the
patient requires urgent care.
• Assess ABCs.
• Treat life threats.
• Transport.
– If condition is stable, finish assessment.
Hands-on ABCs
• Manage threats to ABCs as you find them.
• Steps are the same as with adults.
• Estimate child’s weight.
– Best method is pediatric resuscitation tape
measure.
Hands-on ABCs
• Airway
– Determine whether airway is open and patient
has adequate chest rise with breathing.
– If there is potential obstruction, position airway
and suction as necessary.
Hands-on ABCs
• Breathing
– Calculate the respiratory rate.
– Auscultate breath sounds.
– Check pulse oximetry for oxygen saturation.
Hands-on ABCs
• Circulation
– Integrate information from PAT.
– Listen to the heart or feel pulse for 30 seconds.
• Double the number to get pulse rate.
– After checking the pulse rate, do a hands-on
evaluation of skin CTC.
Hands-on ABCs
• Disability
– Use the AVPU scale or Pediatric Glasgow Coma
Scale to assess level of consciousness.
• Assess pupillary response.
• Evaluate motor activity.
– Assessment of pain must consider age.

• Exposure
– Perform a rapid exam of the entire body.
– Avoid heat loss, especially in infants.
– Cover child as soon as possible.
Transport Decision
• Transport immediately for trauma with:
– Serious MOI
– Physiologic abnormality
– Significant anatomic abnormality
– Unsafe scene
• Attempt vascular access en route.
History Taking
• Can conduct en
route if condition is
unstable.
• Goals:
– Elaborate on chief
complaint.
– Obtain history.
Secondary Assessment
– Head
– Pupils
– Nose
– Ears
– Mouth
– Neck
– Chest
– Back
– Abdomen
– Extremities
– Capillary refill
– Level of hydration
• May include a full-body examination or a focused
assessment
Secondary Assessment
• Attempt to take the child’s blood pressure on
the upper arm or thigh.
– Minimal systolic blood pressure =
80 + (2 × age in years)
Reassessment
• Includes the following:
– PAT
– Patient priority
– Vital signs
– Assessment of interventions
– Reassessment of focused areas
Pediatric Trauma Emergencies
• Leading cause of death among children older
than 1 year
• Anatomy and physiology make injury patterns
and responses different from those seen in
adults.
• Developmental stage will affect response.
Shock
• Inadequate delivery of oxygen and nutrients
to tissues to meet metabolic demand
• Three types:
– Hypovolemic
– Distributive
– Cardiogenic
Shock
• Compensated shock
– Critical abnormalities of perfusion
– Body is able to maintain adequate perfusion to
vital organs.
– Intervention is needed to prevent child from
decompensating.
Shock
• Decompensated shock
– State of inadequate perfusion
• Child will be profoundly tachycardic and show
signs of poor peripheral perfusion.
– Hypotension is a late and ominous sign.
• Start resuscitation on scene.
Hypovolemic Shock
• Hypovolemia: most common cause of shock in
infants and young children
– Loss of volume due to illness or trauma
– Signs may include:
• Listless or lethargic
• Pale, mottled, or cyanotic
Hypovolemic Shock
• Management
– Position of comfort
– Supplemental oxygen
– Keep the child warm.
– Direct pressure to stop external bleeding
– Volume replacement
Hypovolemic Shock
• With compensated
shock, you can
attempt IV or IO
access en route.
– Many of the sites
are the same in
adults and children.
Hypovolemic Shock
• Once IV access is established, begin fluid
resuscitation with isotonic fluids only.
• In decompensated shock with hypotension,
begin initial fluid resuscitation on scene.
– Evaluate sites for IV access.
• If this is unsuccessful, begin IO infusion.
Distributive Shock
• Decreased vascular tone develops.
– Vasodilation and third spacing of fluids occurs.
– In a trauma setting, it is due to spinal cord injury
Pathophysiology of Traumatic Injuries
• Blunt trauma is the MOI in more than 90% of
pediatric injury cases.
– Less muscle and fat mass leads to less protection
against forces transmitted.
Pathophysiology of Traumatic Injuries
• Falls are common.
– Injury will reflect anatomy and height of fall.
• Falls from a standing position usually result in isolated
long bone injuries.
• High-energy falls result in multisystem trauma.
• Injuries from bicycle handlebars typically produce
compression injuries.
Pathophysiology of Traumatic Injuries
• Motor vehicle crashes can result in a variety of
injury patterns depending on restraints and
position in car.
– For unrestrained passengers, assume multisystem
trauma.
– Suspect spinal fractures with chest or abdominal
bruising in a seat belt pattern.
Assessment and Management of
Traumatic Injuries
• Begin with a thorough scene size-up.
• Use PAT to form a general impression.
– If findings are grossly abnormal, move to ABCs.
• Initiate life support interventions.

Traumatic Injuries
• Pneumothorax may be present with
penetrating trauma of the chest or upper
abdomen.
– Perform needle decompression.
– Signs and symptoms may include:
• Tachycardia
• Jugular vein distention
• Pulsus paradoxus

• Any trauma patient should be considered to
be at risk for developing shock.
– Assess circulation.
– The only sign might be an elevated pulse rate.
• Once ABCs are stabilized, continue assessment
of disability with AVPU.

• Place a cervical collar, and immobilize on a
long backboard as indicated.
• Perform rapid exam to identify all injuries.
• Cover the child with blankets.
• Treat any fractures.
Transport Considerations
• Some traumas are load-and-go because of
severe injuries and unstable condition.
• For these situations:
– Perform lifesaving steps on scene or en route.
– Transfer quickly per local trauma protocols.

with suspected
spinal injury require
spinal stabilization.
– Do not place a collar
that is too big on a

History Taking and Secondary
Assessment
• If patient is stable:
– Obtain additional history.
– Perform a more thorough physical exam.
• Look for bruises, abrasions, other subtle signs of injury
that may have been missed.
Fluid Management
• Airway management and ventilatory support
take priority over circulation management.
– Tachycardia is usually the first sign of circulatory
compromise in a child.
– Hypotension is a late finding.
Fluid Management
• Establishing vascular access:
– Large-bore IV catheters should be inserted into a
large peripheral vein.
– 20 or 22 gauge needles may be considered “large
bore.”
– Definitive care can only be provided at the ED.
– To maintain perfusion, administer a bolus of 20
mL/kg of isotonic crystalloid solution.
Pain Management
• Pain is often undertreated in children.
– Use tools to elicit child’s self-report of pain level.
– Use a calm, reassuring voice, distraction
techniques, and medications when appropriate.
Pathophysiology, Assessment, and
Management of Burns
• Assessment and management are similar to
that of adults, with a few key differences.
– Larger skin surface–body mass ratio
• More susceptible to heat and fluid loss
– Worrisome patterns of injury or suspicious
circumstances should raise concerns of abuse.

• Scene safety is
important.
• Estimate the
percentage of BSA
burned may affect
decisions on fluids
and transport.
Assessment and Management of Burns
• Burns suggestive of abuse:
– Mechanism or pattern observed does not match
history or child’s capabilities
• Remove burning clothing; support ABCs.
– Give 100% supplemental oxygen.
Assessment and Management of Burns
• Clean burned areas minimally.
• Avoid lotions or ointments.
• Cover burn and patient as needed.
• Analgesia is a critical part of management.
• Transport to an appropriate medical facility.
Child Abuse and Neglect
• Child abuse: Any improper or excessive action
that injures or harms a child or infant
– Physical abuse
– Sexual abuse
– Emotional abuse
– Neglect
Risk Factors for Abuse
• Risk factors for abuse:
– Younger children
– Children who require extra attention
– Lower socioeconomic status
– Divorce, financial problems, and illness
– Drug and alcohol abuse
– Domestic violence in the home
Suspecting Abuse or Neglect
• If you suspect
abuse, trust your
instincts.
• Look for “red flags”
that could suggest
maltreatment.

• Carefully document what you see.
– Child’s environment
– Condition of home
– Interactions among caregivers, child, EMS crew
• Prehospital personnel are legally obligated to
report suspicion of abuse.

• Involve police early to secure the scene.
• Approach ED staff with concerns.
• Be aware of local regulations.
• Focus on assessment and management.

history that is
inconsistent with
the clinical picture.

Mimics of Abuse
• Can be difficult to
distinguish some
normal skin findings
from inflicted
injuries
• Mongolian spots
may be mistaken for
bruises.

• Medical conditions can mimic bruises.
– Purpura
– Petechiae
• Exposure to sun can cause reactions with
certain medications or fruits.
– Phytophotodermatitis

customs produce
skin markings.
– Coining
– Cupping

  • 3. Pediatric Trauma • Pediatric trauma is the leading cause of death among children older than 1 year. – MVC’s cause most deaths – Falls and submersions are next – Homicides and suicides are also common among adolescents
  • 4. Pediatric Patient Assessment • Differs from adult assessment • Adapt your assessment skills. • Have age-appropriate equipment. • Review age-appropriate vital signs.
  • 5. Scene Size-Up • Take appropriate standard precautions. • Note child’s position. • Note pills, medicine bottles, alcohol, drug paraphernalia, or household chemicals. • Do not discount the possibility of abuse.

  • 7. Primary Assessment • Appearance – A child with a grossly abnormal appearance requires immediate life- support interventions and transportation.
  • 8. Primary Assessment • Work of breathing – Reflects attempt to compensate for abnormalities in oxygenation, ventilation
  • 9. Primary Assessment • Circulation to skin – Determine adequacy of cardiac output and core perfusion.
  • 10. Primary Assessment • Stay or go – Use findings from PAT to determine whether the patient requires urgent care. • Assess ABCs. • Treat life threats. • Transport. – If condition is stable, finish assessment.
  • 11. Hands-on ABCs • Manage threats to ABCs as you find them. • Steps are the same as with adults. • Estimate child’s weight. – Best method is pediatric resuscitation tape measure.
  • 12. Hands-on ABCs • Airway – Determine whether airway is open and patient has adequate chest rise with breathing. – If there is potential obstruction, position airway and suction as necessary.
  • 13. Hands-on ABCs • Breathing – Calculate the respiratory rate. – Auscultate breath sounds. – Check pulse oximetry for oxygen saturation.
  • 14. Hands-on ABCs • Circulation – Integrate information from PAT. – Listen to the heart or feel pulse for 30 seconds. • Double the number to get pulse rate. – After checking the pulse rate, do a hands-on evaluation of skin CTC.
  • 15. Hands-on ABCs • Disability – Use the AVPU scale or Pediatric Glasgow Coma Scale to assess level of consciousness. • Assess pupillary response. • Evaluate motor activity. – Assessment of pain must consider age.
  • 17. Hands-on ABCs • Exposure – Perform a rapid exam of the entire body. – Avoid heat loss, especially in infants. – Cover child as soon as possible.
  • 18. Transport Decision • Transport immediately for trauma with: – Serious MOI – Physiologic abnormality – Significant anatomic abnormality – Unsafe scene • Attempt vascular access en route.
  • 19. History Taking • Can conduct en route if condition is unstable. • Goals: – Elaborate on chief complaint. – Obtain history.
  • 20. Secondary Assessment – Head – Pupils – Nose – Ears – Mouth – Neck – Chest – Back – Abdomen – Extremities – Capillary refill – Level of hydration • May include a full-body examination or a focused assessment
  • 21. Secondary Assessment • Attempt to take the child’s blood pressure on the upper arm or thigh. – Minimal systolic blood pressure = 80 + (2 × age in years)
  • 22. Reassessment • Includes the following: – PAT – Patient priority – Vital signs – Assessment of interventions – Reassessment of focused areas
  • 23. Pediatric Trauma Emergencies • Leading cause of death among children older than 1 year • Anatomy and physiology make injury patterns and responses different from those seen in adults. • Developmental stage will affect response.
  • 24. Shock • Inadequate delivery of oxygen and nutrients to tissues to meet metabolic demand • Three types: – Hypovolemic – Distributive – Cardiogenic
  • 25. Shock • Compensated shock – Critical abnormalities of perfusion – Body is able to maintain adequate perfusion to vital organs. – Intervention is needed to prevent child from decompensating.
  • 26. Shock • Decompensated shock – State of inadequate perfusion • Child will be profoundly tachycardic and show signs of poor peripheral perfusion. – Hypotension is a late and ominous sign. • Start resuscitation on scene.
  • 27. Hypovolemic Shock • Hypovolemia: most common cause of shock in infants and young children – Loss of volume due to illness or trauma – Signs may include: • Listless or lethargic • Pale, mottled, or cyanotic
  • 28. Hypovolemic Shock • Management – Position of comfort – Supplemental oxygen – Keep the child warm. – Direct pressure to stop external bleeding – Volume replacement
  • 29. Hypovolemic Shock • With compensated shock, you can attempt IV or IO access en route. – Many of the sites are the same in adults and children.
  • 30. Hypovolemic Shock • Once IV access is established, begin fluid resuscitation with isotonic fluids only. • In decompensated shock with hypotension, begin initial fluid resuscitation on scene. – Evaluate sites for IV access. • If this is unsuccessful, begin IO infusion.
  • 31. Distributive Shock • Decreased vascular tone develops. – Vasodilation and third spacing of fluids occurs. – In a trauma setting, it is due to spinal cord injury
  • 32. Pathophysiology of Traumatic Injuries • Blunt trauma is the MOI in more than 90% of pediatric injury cases. – Less muscle and fat mass leads to less protection against forces transmitted.
  • 33. Pathophysiology of Traumatic Injuries • Falls are common. – Injury will reflect anatomy and height of fall. • Falls from a standing position usually result in isolated long bone injuries. • High-energy falls result in multisystem trauma. • Injuries from bicycle handlebars typically produce compression injuries.
  • 34. Pathophysiology of Traumatic Injuries • Motor vehicle crashes can result in a variety of injury patterns depending on restraints and position in car. – For unrestrained passengers, assume multisystem trauma. – Suspect spinal fractures with chest or abdominal bruising in a seat belt pattern.
  • 35. Assessment and Management of Traumatic Injuries • Begin with a thorough scene size-up. • Use PAT to form a general impression. – If findings are grossly abnormal, move to ABCs. • Initiate life support interventions.
  • 36. Assessment and Management of Traumatic Injuries • Pneumothorax may be present with penetrating trauma of the chest or upper abdomen. – Perform needle decompression. – Signs and symptoms may include: • Tachycardia • Jugular vein distention • Pulsus paradoxus
  • 37. Assessment and Management of Traumatic Injuries • Any trauma patient should be considered to be at risk for developing shock. – Assess circulation. – The only sign might be an elevated pulse rate. • Once ABCs are stabilized, continue assessment of disability with AVPU.
  • 38. Assessment and Management of Traumatic Injuries • Place a cervical collar, and immobilize on a long backboard as indicated. • Perform rapid exam to identify all injuries. • Cover the child with blankets. • Treat any fractures.
  • 39. Transport Considerations • Some traumas are load-and-go because of severe injuries and unstable condition. • For these situations: – Perform lifesaving steps on scene or en route. – Transfer quickly per local trauma protocols.

  • 41. History Taking and Secondary Assessment • If patient is stable: – Obtain additional history. – Perform a more thorough physical exam. • Look for bruises, abrasions, other subtle signs of injury that may have been missed.
  • 42. Fluid Management • Airway management and ventilatory support take priority over circulation management. – Tachycardia is usually the first sign of circulatory compromise in a child. – Hypotension is a late finding.
  • 43. Fluid Management • Establishing vascular access: – Large-bore IV catheters should be inserted into a large peripheral vein. – 20 or 22 gauge needles may be considered “large bore.” – Definitive care can only be provided at the ED. – To maintain perfusion, administer a bolus of 20 mL/kg of isotonic crystalloid solution.
  • 44. Pain Management • Pain is often undertreated in children. – Use tools to elicit child’s self-report of pain level. – Use a calm, reassuring voice, distraction techniques, and medications when appropriate.

Pathophysiology, Assessment, and Management of Burns • Assessment and management are similar to that of adults, with a few key differences. – Larger skin surface–body mass ratio • More susceptible to heat and fluid loss – Worrisome patterns of injury or suspicious circumstances should raise concerns of abuse.
  • 46. Assessment and Management of Burns • Scene safety is important. • Estimate the percentage of BSA burned may affect decisions on fluids and transport.
  • 47. Assessment and Management of Burns • Burns suggestive of abuse: – Mechanism or pattern observed does not match history or child’s capabilities • Remove burning clothing; support ABCs. – Give 100% supplemental oxygen.
  • 48. Assessment and Management of Burns • Clean burned areas minimally. • Avoid lotions or ointments. • Cover burn and patient as needed. • Analgesia is a critical part of management. • Transport to an appropriate medical facility.
  • 49. Child Abuse and Neglect • Child abuse: Any improper or excessive action that injures or harms a child or infant – Physical abuse – Sexual abuse – Emotional abuse – Neglect
  • 50. Risk Factors for Abuse • Risk factors for abuse: – Younger children – Children who require extra attention – Lower socioeconomic status – Divorce, financial problems, and illness – Drug and alcohol abuse – Domestic violence in the home
  • 51. Suspecting Abuse or Neglect • If you suspect abuse, trust your instincts. • Look for “red flags” that could suggest maltreatment.
  • 52. Assessment and Management of Abuse and Neglect • Carefully document what you see. – Child’s environment – Condition of home – Interactions among caregivers, child, EMS crew • Prehospital personnel are legally obligated to report suspicion of abuse.
  • 53. Assessment and Management of Abuse and Neglect • Involve police early to secure the scene. • Approach ED staff with concerns. • Be aware of local regulations. • Focus on assessment and management.

  1. Pediatric trauma is the leading cause of death among children older than 1 year. Motor vehicle crashes cause the most deaths in this age group, followed by falls and submersions. Among adolescents, homicide and suicide are also common causes of death. Children’s anatomy and physiology make injury patterns and responses to trauma different from those seen in adults. Developmental stage will affect response to injury. For a young child, being strapped to a backboard may be as traumatic as the injury leading to the EMS call!
  2. Your assessment, like your general approach, will differ somewhat with pediatric patients. You may need to adapt your assessment skills. Use age-appropriate equipment. Review age-appropriate vital signs.
  3. Scene size-up On the way to the scene, prepare for a pediatric scene size-up, equipment use, and assessment. Collect information from dispatch. Take appropriate standard precautions. Observe for hazards. Resist the temptation for hasty assessment because you know the patient is a child. Personal safety is your priority. Note child’s position. Look for clues to MOI or NOI. Helps guide assessment and management priorities Should enable you to determine severity When a child is unable to communicate or is unresponsive, assume MOI was significant enough to cause head or neck injuries. Immobilize spine with a cervical collar. Place a pad under head and/or shoulders to facilitate airway management. Note any pills, medicine bottles, alcohol, drug paraphernalia, or household chemicals that would suggest toxic exposure or possible ingestion. Carefully observe the scene or vehicle (if involved) for clues to the potential severity of any injuries. Do not discount the possibility of abuse. The following should increase your suspicion for abuse: Conflicting information from parents or caregivers Bruises or other injuries that are not consistent with the MOI described Injuries that are not consistent with the child’s age and developmental abilities Note parents’ or caregivers’ interaction with the child. Do they appear to be appropriately concerned, angry, or indifferent? Does the child seem comforted by their presence or scared by them?
  4. Primary assessment Use the Pediatric Assessment Triangle to form a general impression. After ensuring scene safety, begin with your general impression of how the patient looks (“sick-not sick” classification). Pediatric Assessment Triangle (PAT) helps you form a “from-the-doorway” impression and distinguish between sick and not-sick patients. Standardized approach with three elements—appearance, work of breathing, and circulation 15- to 30-second assessment that paints an accurate clinical picture of cardiopulmonary status and level of consciousness Conducted prior to assessing ABCs. Does not require touching the patient. Rapid, hands-off systematic approach to observing an ill or injured child Helps establish urgency for treatment or transport
  5. Appearance First element of PAT Often the most important factor in determining the severity of illness, need for treatment, and response to therapy. Reflects adequacy of ventilation, oxygenation, brain perfusion, body homeostasis, and central nervous system (CNS) function TICLS (tickles) mnemonic highlights the most important features of a child’s appearance: Tone Interactiveness Consolability Look or gaze Speech or cry You can also evaluate level of consciousness by using the AVPU scale, modified as necessary for the child’s age. Observe from a distance, allowing child to interact with caregiver. Use TICLS mnemonic while observing from the doorway. Delay touching the patient until you have formed a general impression; your touch may agitate the child. Unless a child is unconscious or critically ill, take your time in assessing appearance by observation. Abnormal appearance may result from numerous physiologic abnormalities: Inadequate oxygenation or ventilation, as in respiratory emergencies Inadequate brain perfusion, as from cardiovascular emergencies Systemic abnormalities or metabolic derangements, such as with poisoning, infection, or hypoglycemia Acute or chronic brain injury A child with a grossly abnormal appearance is seriously ill and requires immediate life-support interventions and transportation. The remainder of PAT plus hands-on assessment (ABCs) may help identify the cause, severity, and need for treatment and transportation.
  6. Work of breathing Often a better assessment of oxygenation and ventilation status than auscultation or respiratory rate Reflects child’s attempt to compensate for abnormalities in oxygenation, ventilation Proxy for effectiveness of gas exchange Hands-off assessment includes listening for abnormal airway sounds and looking for signs of increased breathing effort. Some abnormal airway sounds can be heard without a stethoscope and can indicate the likely physiology and anatomic location of the breathing problem. For example, snoring, muffled or hoarse voice, or stridor can indicate obstruction at the level of the oropharynx, glottis or supraglottic structures, or glottis or subglottic structures, respectively. May result from croup, bacterial upper airway infections, bleeding, or edema. Lower airway obstruction is suggested by abnormal grunting or wheezing. Grunting is a form of auto-PEEP (positive end expiratory pressure). A way to distend lower respiratory air sacs or alveoli to promote maximum gas exchange Involves exhaling against a partially closed glottis Short, low-pitched sound Best heard at the end of exhalation Often mistaken for whimpering Suggests moderate to severe hypoxia Seen with lower airway conditions such as pneumonia and pulmonary edema Reflects poor gas exchange because of fluid in the lower airways and air sacs Wheezing is a musical tone caused by air being forced through constricted or partially blocked small airways. Often occurs during exhalation only Can occur during inspiration and expiration during severe asthma attacks Often heard only by auscultation, though severe obstruction may result in wheezing that is audible without a stethoscope Abnormal positioning and retractions are physical signs of increased work of breathing. Easily assessed without touching the patient Sniffing position: Child is trying to align the axes of the airways to improve patency and increase air flow Often reflects a severe upper airway obstruction Child who refuses to lie down or leans forward on outstretched arms (tripoding) is creating the optimal mechanical advantage to use accessory muscles of respiration. Retractions: The recruitment of accessory muscles of respiration to provide more “muscle power” to move air into the lungs in the face of airway or lung disease or injury To observe, expose the chest. More useful measure of work of breathing in children than in adults because a child’s chest wall is less muscular, so inward excursion of skin and soft tissue between the ribs is more apparent. May be evident in the supraclavicular area (above the clavicle), the intercostal area (between the ribs), or substernal area (under the sternum). Head bobbing: Form of retractions seen only in infants Use of neck muscles to help breathing during severe hypoxia Neck extends during inhalation; head falls forward during exhalation. Nasal flaring: Exaggerated opening of the nostrils during labored inspiration Indicates moderate to severe hypoxia Combine the characteristics of work of breathing to make your general assessment of oxygenation and ventilation status. Abnormal airway sounds Abnormal positioning Retractions Nasal flaring Together with appearance, work of breathing suggests: Severity of illness Likelihood that the cause is in the airway or is a respiratory process
  7. Circulation to skin Goal of rapid circulatory assessment: Determine adequacy of cardiac output and core perfusion. When cardiac output diminishes, the body shunts circulation from nonessential areas (eg, skin) toward vital organs. Circulation to skin reflects overall status of core circulation. Three characteristics considered: Pallor Mottling Cyanosis Pallor (paleness) may be the initial sign of poor circulation or the only visual sign with compensated shock. Indicates reflex peripheral vasoconstriction that is shunting blood toward the core May also indicate anemia or hypoxia Mottling reflects vasomotor instability in the capillary beds demonstrated by patchy areas of vasoconstriction and vasodilation. May also be a physiologic response to a cold environment Cyanosis, a bluish discoloration of the skin and mucous membranes, is the most extreme visual indicator of poor perfusion or poor oxygenation. Acrocyanosis, blue hands or feet in an infant younger than 2 months, is distinct from cyanosis. Normal finding when a young infant is cold True cyanosis is seen in the skin and mucous membranes. Late finding of respiratory failure or shock After assessing appearance and work of breathing, look for pallor, mottling, cyanosis. Combine the three pieces of PAT to estimate severity of illness and the likely underlying pathologic cause. Abnormal appearance with poor circulation may indicate shock from a cardiovascular cause.
  8. Stay or go Use PAT findings to determine whether patient is stable or requires urgent care. If unstable: Assess ABCs. Treat life threats. Transport immediately. If stable, perform the entire patient assessment process.
  9. Hands-on ABCs After PAT, complete primary assessment: Assess ABCs and mental status. Prioritize care and need for transport. Manage threats to ABCs as you find them. Prioritize sequence of life-support interventions to reverse critical physiologic abnormalities. ABC steps are the same as with adults but with differences related anatomy, physiology, and signs of distress. If you suspect cardiac arrest, which is rare, the order is CAB. Chest compressions would be the priority. Also assess disability and exposure. Early in the assessment, estimate child’s weight. Much of your care will depend on child’s size. Best method is pediatric resuscitation tape measure (Broselow tape, or length-based resuscitation tape) Also provides medication doses and equipment sizes Estimates weight and height in pediatric patients weighing up to 75 lb (34 kg). To use, follow these steps: Measure child’s length from head to heel with the tape (red portion at the head). Note the weight in kilograms that corresponds to the measured length at the heel. If child is longer than the tape, use adult equipment and medication doses. From the tape, identify appropriate equipment sizes and medication doses.
  10. Airway PAT may suggest an airway obstruction based on abnormal airway sounds and increased work of breathing. Determine whether the airway is open and patient has adequate chest rise with breathing. Check for mucus, blood, or a foreign body in the mouth or airway. If there is potential obstruction from the tongue or soft tissues, position the airway and suction as necessary. Determine whether airway is open and patent, partially obstructed, or totally obstructed. Do not keep the suction tip or catheter in the back of a child’s throat too long because young patients are extremely sensitive to vagal stimuli and the pulse rate may plummet.
  11. Breathing Breathing component of primary assessment involves the following: Calculate the respiratory rate. Auscultate breath sounds. Check pulse oximetry for oxygen saturation. Verify the respiratory rate per minute by counting the number of chest rises in 30 seconds and then doubling that number. Healthy infants may show periodic breathing or variable respiratory rates with short periods of apnea (< 20/s). Counting for only 10 to 15 seconds may give a falsely low respiratory rate. Interpreting the respiratory rate requires knowing the normal values for the child’s age and putting the respiratory rate in context with the rest of the primary assessment. Rapid respiratory rates may reflect high fever, anxiety, pain, or excitement. Normal rates may occur in a child who has been breathing rapidly with increased work of breathing and is becoming fatigued. Serial assessment may be useful; trend may be more accurate than a single value. Auscultate breath sounds with a stethoscope over the midaxillary line to hear abnormal lung sounds during inhalation and exhalation. Listen for extra breath sounds such as inspiratory crackles, wheezes, or rhonchi. Rhonchi often indicate harsh breath sounds or sounds that may be transmitted from the upper airways. If you cannot determine whether the sounds are being generated in the lungs or upper airway, hold the stethoscope over the nose or trachea and listen. Listen for adequacy of air movement. Diminished breath sounds may signal severe respiratory distress. Auscultation over the trachea may help distinguish stridor from other sounds. Check pulse oximetry reading to determine oxygen saturation while child breathes ambient air. Place the probe on a young child’s finger (same as adult). Infants or young children may try to remove the probe, it may be helpful to place the probe on a toe, possibly with a sock covering it. Greater than 94% saturation while breathing room air indicates good oxygenation. Evaluate pulse oximetry reading in the context of PAT and primary assessment. A child with a normal pulse oximetry reading, may be expending increasing amounts of energy and increasing work of breathing to maintain oxygen saturation. Primary assessment would identify the respiratory distress and point to the need for immediate intervention despite the normal oxygen saturation level.
  12. Circulation Integrate information from PAT about circulation with pulse rate/quality and skin CTC (color, temperature, and condition plus capillary refill time) to obtain an overall assessment of circulatory status. Listen to the heart or feel pulse for 30 seconds; double the number to get pulse rate. It is important to know normal pulse rates based on age. Interpret the pulse rate within the context of the overall history and primary assessment. Tachycardia may indicate early hypoxia or shock or a less serious condition such as fever, anxiety, pain, or excitement. Feel for the pulse to ascertain the rate and quality of pulsations. If you cannot find a peripheral (distal) pulse (that is, radial or brachial), feel for a central pulse (that is, femoral or carotid). Check the femoral pulse in infants and young children and the carotid pulse in older children and adolescents. Start CPR if there is no pulse. After checking the pulse rate, do a hands-on evaluation of skin CTC. Check whether the hands and feet are warm or cool to the touch. Check capillary refill time in the fingertip, toe, heel, or pads of the fingertips. Normal time is 2 seconds or less. Put CTC information in context because cool extremities and delayed capillary refill are commonly seen in a child in a cool environment.
  13. Disability Use the AVPU scale or Pediatric Glasgow Coma Scale to assess level of consciousness. Assess pupillary response to a beam of light to assess brainstem response. Note if dilated, constricted, reactive, or fixed. Evaluate motor activity, looking for symmetric movement of the extremities, seizures, posturing, or flaccidity. Combine this information with PAT results to determine neurologic status. Inadequate treatment of pain is common with children and has many adverse effects. Pain causes morbidity and misery for the child and caregivers. It interferes with your ability to accurately assess physiologic abnormalities. Children who do not receive appropriate analgesia may be more likely to have exaggerated pain responses to subsequent painful procedures. Post-traumatic stress may be more common among children who experience pain during an illness or injury. Assessment of pain must consider developmental age. The ability to identify pain improves with age. In infants and preverbal children, it may be difficult to distinguish crying and agitation due to hypoxia, hunger, or pain. Further assessment and discussion with caregivers are essential. Pain scales such as the Wong-Baker FACES scale may prove helpful. Remaining calm and providing quiet, professional reassurance to parents and child is critical for managing pediatric pain and anxiety. Techniques for reducing pain include: Distraction techniques with toys or stories Visual imagery techniques and music Sucrose pacifiers in neonates Pharmacologic methods for reducing pain are available in a number of EMS systems. Eg, acetaminophen, opiates, benzodiazepines, and nitrous oxide Weigh the benefits of analgesics or anxiolytics against risks, including potential route of administration. Respiratory depression, bradycardia, hypoxemia, and hypotension are potential side effects of sedatives. Intravenous medications are often most effective but require establishing IV access, which is a painful procedure. Assessment and management of pain and anxiety is part of vital sign assessment and field care. Requires a thorough understanding of nonpharmacologic techniques, drugs, potential drug contraindications and complications, and management of complications
  14. Exposure Proper exposure is needed to complete the primary assessment. At least partial undress to assess the work of breathing and circulation Perform a rapid exam of the entire body to look for unsuspected injuries and anatomic abnormalities. Avoid heat loss, especially in infants. Cover child as soon as possible. Keep temperature in the ambulance high. Use blankets when necessary.
  15. Transport decision After completing the primary assessment and beginning resuscitation when necessary, you must make a crucial decision: immediate transport or continued assessment and treatment on scene. Immediate transport is imperative if the emergency call is for trauma and the child has one of the following: Serious MOI Physiologic abnormality Potentially significant anatomic abnormality Scene is unsafe In these cases, manage all life threats, and begin transport. Attempt vascular access on the way to the ED. If the call is for an illness, the decision to stay or go is less clear and depends on the following factors: Expected benefits of treatment EMS system regulations Comfort level Transport time
  16. History taking If the child seems to be in physiologically unstable condition, you may decide to transport immediately and conduct history taking and secondary assessment en route to the ED. Goals: Elaborate on chief complaint (OPQRST). Obtain patient history (SAMPLE).
  17. The extent of the examination will depend on the situation and may include the following: Head The younger the infant or child, the larger the head is in proportion to the rest of the body, increasing the risk for head injury with deceleration (eg, motor vehicle crashes). Look for bruising, swelling, and hematomas. Significant blood can be lost between the skull and scalp of a small infant. Fontanelle Bulging suggests elevated intracranial pressure caused by meningitis, encephalitis, or intracranial bleeding. Sunken appearance suggests dehydration. Pupils Note size, equality, and reactivity to light. Response is a good indication of brain function, particularly with trauma. Nose Young infants prefer to breathe through their nose, so nasal congestion with mucus can cause respiratory distress. Gentle bulb or catheter suction of the nostrils may bring relief. Ears Look for drainage from the ear canals. Leaking blood suggests a skull fracture. Check for bruises behind the ear or Battle sign, a late sign of skull fracture. Pus may indicate an ear infection or perforation of the eardrum. Mouth In the trauma patient, look for active bleeding and loose teeth. Note the smell of the breath. Some ingestions are associated with identifiable odors, such as hydrocarbons. Acidosis, as in diabetic ketoacidosis, may impart an acetone-like smell. Neck Examine the trachea for swelling or bruising. Note if the patient cannot move his or her neck and has a high fever. May indicate bacterial or viral meningitis Chest Examine for penetrating injuries, lacerations, bruises, or rashes. With injury, feel the clavicles and every rib for tenderness and/or deformity. Back Inspect for lacerations, penetrating injuries, bruises, or rashes. Abdomen Inspect for distention. Palpate gently; watch for guarding or tensing of the abdominal muscles. May suggest infection, obstruction, or intra-abdominal injury Note tenderness or masses. Look for seat belt abrasions or bruising. Extremities Assess for symmetry. Compare both sides for color, warmth, size of joints, swelling, and tenderness. Put each joint through full range of motion while watching the eyes of the pediatric patient for signs of pain, unless there is obvious deformity suggesting a fracture. Capillary refill (in children younger than 6 years) Normal time should be 2 seconds or less. Assess by blanching the finger or toenail beds; soles of the feet may also be used. Cold temperatures will increase capillary refill time, making it a less reliable sign. Level of hydration Assess skin turgor. Note the presence of tenting, a condition in which the skin slowly retracts after being pinched and pulled away slightly from the body. In infants, note whether the fontanelles are sunken or flat. Ask parent how many diapers the infant has soiled over the last 24 hours. Determine whether the child is producing tears when crying. Note the condition of the mouth. Is the oral mucosa moist or dry?
  18. Attempt to take the child’s blood pressure on the upper arm or thigh, although accurate measurement may be difficult due to lack of cooperation and need for proper cuff size. Make sure the cuff has a width two thirds the length of the upper arm or thigh. One formula for determining the lower acceptable limit in children ages 1 to 10 years: minimal systolic blood pressure = 80 + (2 X age in years). Eg, a 2-year-old should have a minimal systolic blood pressure of 84. Lower rate indicates decompensated shock. Given the technical difficulty, make one attempt. If unsuccessful, move on to the rest of the assessment.
  19. Reassessment Includes the following: PAT Patient priority Vital signs Unstable condition: Every 5 minutes Stable condition: Every 15 minutes Assessment of the effectiveness of interventions Eg, medications administered, splints applied, bleeding controlled Reassessment of the focused exam areas Reassess all patients to observe their response to treatment, guide ongoing treatments, and track the progression of identified pathologic and anatomic problems. New problems may also be identified. May guide the choice of transport destination and communications with medical oversight or ED staff
  20. Shock is inadequate delivery of oxygen and nutrients to tissues to meet metabolic demand, and you may encounter three types in both children and adults: hypovolemic, distributive, and cardiogenic. Determine the cause of shock and quickly determine whether the child is in a compensated or decompensated state.
  21. Compensated shock Child has critical abnormalities of perfusion, but the body is (for the moment) able to maintain adequate perfusion to vital organs by shunting blood from the periphery, increasing the pulse rate, and increasing the vascular tone. Normal appearance, tachycardia, and signs of decreased peripheral perfusion, such as cool extremities with prolonged capillary refill Timely intervention is needed to prevent child from decompensating.
  22. Decompensated shock State of inadequate perfusion; mechanisms to improve perfusion are no longer sufficient to maintain a normal blood pressure. Includes hypotension Relative to the age of the child Child will be profoundly tachycardic and show signs of poor peripheral perfusion. May have an altered appearance, reflecting inadequate perfusion of the brain. Because children typically have strong cardiovascular systems, they are able to compensate for inadequate perfusion by increasing the pulse rate and peripheral vascular resistance more efficiently than adults. Hypotension is a late and ominous sign; urgent intervention is needed to prevent cardiac arrest. Initial management: Position of comfort, supplemental oxygen After primary assessment, make a transport decision based on the severity of the problem. Start resuscitation on scene for any child who shows signs of decompensated shock. Rapid transport is imperative, but risk of deterioration to cardiac arrest is too high to permit a “load-and-go” approach.
  23. Hypovolemic shock Most common cause of shock in infants and young children, with loss of volume occurring due to illness or trauma Because of their small blood volume (80 mL/kg body weight), a combination of excessive fluid losses and poor intake in an infant or a young child with gastroenteritis (“stomach flu”) can result in shock relatively quickly. The same vulnerability exists with hemorrhage from trauma. Patient will often appear listless or lethargic and may have compensatory tachypnea. May appear pale, mottled, or cyanotic In medical shock, you may see signs of dehydration such as: Sunken eyes Dry mucous membranes Poor skin turgor Delayed capillary refill with cool extremities
  24. Management In an injured child, the site of bleeding may be identified. Allow the child to remain in a position of comfort, administer supplemental oxygen, and keep the child warm. Apply direct pressure to stop any external bleeding. Volume replacement is the mainstay of treatment for hypovolemic shock.
  25. With compensated shock, you can attempt to establish IV or IO access en route. Many IV access sites are the same in adults and children. Most common: Dorsum of the hand and antecubital fossa In children, veins in the foot may also be used. Scalp veins, external jugular veins less common
  26. Once IV access is established, fluid resuscitation should begin with isotonic fluids only, such as normal saline or lactated Ringer’s. If SBP is less than 100mmHg, begin with 20 mL/kg, then reassess the patient’s status. In the presence of severe head injury, maintain a SBP of 110-120mmHg Warm IV fluids (when possible) can counteract the effects of systemic hypothermia from environmental exposure, blood loss, or open wounds. Multiple fluid boluses may be necessary during transport. In decompensated shock with hypotension, begin initial fluid resuscitation on scene. Evaluate sites for IV access. If this is unsuccessful, begin IO infusion.
  27. Distributive shock Decreased vascular tone develops. Results in vasodilation and third spacing of fluids due to increased vascular permeability Leakage of plasma out of blood vessels, into surrounding tissues Results in a drop in effective blood volume and functional hypovolemia In a trauma setting, this is due to spinal cord injury Early in distributive shock, child may have warm, flushed skin and bounding pulses. Result of peripheral vasodilation Signs, symptoms of late distributive shock are much like hypovolemic shock In addition, neurologic deficits are apparent due to spinal cord injury. Frontline treatment is volume resuscitation. Child is in a state of relative hypovolemia.
  28. Pathophysiology of traumatic injuries Blunt trauma is the MOI in more than 90% of pediatric injury cases. Children have less muscle and fat mass than adults, thus less protection against the forces transmitted in blunt trauma.
  29. Falls are common; injuries will reflect child’s anatomy child and height of fall. Eg, 6-year-old playing on the monkey bars is most likely to sustain an upper extremity fracture when falling onto an outstretched arm. Internal or head injuries would be uncommon with this mechanism. An infant, with a big head and no protective reflexes, who falls out of a shopping cart will commonly have a skull fracture, and could have intracranial hemorrhage. Falls from a standing position usually result in isolated long bone injuries. High-energy falls (such as from a window, ejection from a motor vehicle, car-versus-pedestrian collision) may result in multisystem trauma. Injuries from bicycle handlebars typically produce compression injuries to intra-abdominal organs. Duodenal hematomas, pancreatic injuries, upper extremity injuries are common. Consider head injury if patient went over the handlebars, especially if not wearing a helmet.
  30. Motor vehicle crashes can result in a variety of injury patterns depending on use of restraints, position in car. For unrestrained passengers, assume multisystem trauma. Restrained passengers may sustain chest and abdominal injuries associated with seat belt use. Suspect spinal fractures with chest or abdominal bruising in a seat belt pattern. Air bags pose a particular threat for head and neck injuries in young children. Multisystem trauma is likely with car-versus-pedestrian collisions. Chest, abdominal, and lower extremity injuries occur on impact. Head and neck injuries may result from the fall when the child is thrown. Motor vehicle crashes can result in a variety of injury patterns depending on use of restraints, position in car. For unrestrained passengers, assume multisystem trauma. Restrained passengers may sustain chest and abdominal injuries associated with seat belt use. Suspect spinal fractures with chest or abdominal bruising in a seat belt pattern. Air bags pose a particular threat for head and neck injuries in young children. Multisystem trauma is likely with car-versus-pedestrian collisions. Chest, abdominal, and lower extremity injuries occur on impact. Head and neck injuries may result from the fall when the child is thrown.
  31. Assessment and management of traumatic injuries First steps are the same as for medical emergencies. Begin with a thorough scene size-up: Address safety concerns. Determine MOI or NOI as you approach the scene. Note number of patients. Follow standard precautions. Determine whether additional resources are needed. Use PAT to form a general impression. If findings are grossly abnormal, quickly move to ABCs. Immediately think of head injury with abnormal appearance. With isolated closed head injury, breathing and circulation may be normal. Abnormal appearance may reflect inadequate oxygenation of the brain owing to shock or respiratory failure. Abnormalities in work of breathing will tip you off to chest or airway injury and abnormal circulation to a hemorrhage problem. If multisystem injuries are present, all three sides of the PAT may be abnormal. Initiate life support interventions as you identify problems. Assess the airway for obstruction with teeth, blood, vomit, or edema. Suction as needed. For cervical spinal injury, open airway with jaw-thrust maneuver. If child cannot maintain the airway, consider an NG or oropharyngeal airway. If you attempt ET intubation, maintain cervical spinal precautions. Establishment of an emergency surgical airway in a child is fraught with complications and has a high failure rate. Tracheotomy should be reserved for surgeons in a controlled setting. Remote chances of needing to perform a needle cricothyrotomy in a child In younger children, identification of the cricothyroid membrane is difficult. Breathing assessment includes evaluation for symmetric chest rise and equal breath sounds. 100% supplemental oxygen Bag-mask ventilation as needed NG or OG tube for stomach decompression
  32. Pneumothorax: Not common in pediatric blunt chest injury, but may be present with penetrating trauma of the chest or upper abdomen. Jugular venous distention and tracheal deviation less likely in a child. If mechanism suggests a possible tension pneumothorax and child is in significant respiratory distress, perform needle decompression. Signs and symptoms of tension pneumothorax include the following: Tachycardia Difficult ventilation despite an open airway Absent or decreased breath sounds on affected side Jugular vein distention (may not be present with associated hemorrhage) Hyperresonance to percussion on affected side Tracheal deviation away from affected side (late sign, not always present) Pulsus paradoxus Trachial deviation, hyperresonance, pulsus paradoxus, even decreased breath sounds can be difficult to assess in a young child.
  33. Any trauma patient should be considered to be at risk for developing shock from visible external bleeding or internal bleeding. Assess circulation by checking pulse rate and quality, capillary refill, skin temperature, and blood pressure. In pediatric patients, only sign of compensated shock might be an elevated pulse rate. Children have a remarkable capacity for peripheral vasoconstriction. Can maintain blood pressure despite significant blood loss If MOI is concerning and child is tachycardic, assume compensated shock and initiate volume resuscitation with 20 mL/kg of isotonic fluid (normal saline or lactated Ringer’s). Ideally, insert two peripheral IV lines, but an IO line may be best in a child with hemorrhagic shock. Control external bleeding as you would in any trauma patient. Once ABCs are stabilized, continue assessment of disability with AVPU. Assessment of appearance will already have identified an altered LOC. Check pupils and motor function.
  34. Place a cervical collar and immobilize on a long backboard as indicated. If increased ICP is a concern, keep the head midline to facilitate jugular venous return to the heart. If child is not in shock, elevate the backboard or head of the stretcher to 30°. Perform shock resuscitation with IV fluids—brain hypoperfusion will worsen the situation. If child has acute signs of herniation such as a “blown” pupil or Cushing’s triad (elevated blood pressure, bradycardia, abnormal respiratory pattern), consider mild hyperventilation guided to anETCO2 of 32 to 35 mm Hg and administer mannitol. Assess “exposure”—rapid exam to identify all injuries. Log roll the child to examine back and buttocks. Cover the child with blankets. Cover the head, especially in infants and young children. Avoid drafts from heating or air conditioning units. Children have a relatively large skin surface area-body mass ratio, increasing risk for heat loss, hypothermia. Consider warm IV fluids, warm oxygen, and warm patient transport environment Keep patient covered. Remove wet clothing. Treat any fractures as you would in an adult. Check equipment ahead of time for appropriate pediatric splints.
  35. Transport considerations After stabilization, you face a transport decision. Some traumas are load-and-go because of severe injuries and unstable condition, eg: Trauma involving an ominous MOI regardless of how the child looks on scene Unstable or compromised airway Shock Difficulty breathing Severe neurologic disability For these situations, perform lifesaving procedures on scene or en route, and quickly transfer to an appropriate trauma center according to local protocols.
  36. All trauma victims with suspected spinal injury require appropriate spinal stabilization. Indications are the same for children and adults. You may have difficulty finding an appropriately sized cervical collar for infants or young children. Do not place a collar that is too big on a small child—use towel rolls and tape to immobilize the head. Apply tape across temples and forehead. Avoid chin or throat; may impair ventilation. Choose a pediatric immobilizer with a recess for the child’s large occiput. Or place a towel or small blanket under the shoulders and back to prevent neck flexion in infants and toddlers. Secure child firmly onto backboard, but leave room for adequate chest expansion. Being immobilized is a frightening experience, especially for a young child. Use developmentally appropriate language to explain what you are doing and why. Keep a parent close by when possible. Nearest appropriate facility depends on local protocols, capabilities of local EDs. In some areas of the country, you may be directed to take the patient directly to a pediatric trauma center or to arrange for air transport to a pediatric trauma center. In others, children are evaluated primarily at local EDs, then transferred to a pediatric trauma center.
  37. History taking and secondary assessment If patient is stable and does not meet load-and-go criteria, obtain additional history and perform a more thorough physical exam. Full-body, back-to-front detailed physical examination en route for all trauma patients with significant MOI. For infants, this includes checking anterior fontanelle for bulging (sign of increased ICP). Look for bruises, abrasions, other subtle signs of injury that may have been missed. Revisit primary assessment during en-route reassessment; condition can change quickly.
  38. Fluid management Circulatory compromise from trauma is less common in children than in adults, so airway management and ventilatory support take priority over management of circulation. Tachycardia is usually the first sign of circulatory compromise in a child. Can also be caused by agitation, being scared, or being restrained Children are able to compensate for blood loss from any cause, including trauma, so hypotension is a late finding in pediatric trauma. Once a pediatric patient is hypotensive, they are already in severe trouble. Recognizing circulatory compromise and treating early are even more important in the pediatric population.
  39. Consider the following when establishing vascular access in the injured child: Large-bore IV catheters should be inserted into a large peripheral vein whenever possible. In infants and young children, 20 or 22 gauge IV needles may be considered “large bore.” Definitive care can only be provided at the ED, so never delay transport to start an IV line; perform en route. To maintain perfusion, administer an initial bolus of 20 mL/kg using an isotonic crystalloid solution (ie, normal saline, lactated Ringer’s) maintaining a SBP of 100mmHg. Frequently reassess vital signs, and provide additional IV fluid boluses of 20 mL/kg if no improvement is noted following the initial bolus. Up to 60 mL/kg can be given for resuscitation of a pediatric patient. If patient is over 50 kg, use adult resuscitation fluid volumes. If condition does not improve following two boluses of an isotonic crystalloid, blood loss is likely severe and patient may need surgical intervention. Provide rapid transport with continuous monitoring en route. If the child is hypotensive and IV access cannot be obtained within 90 seconds, consider inserting an IO needle.
  40. Pain management Pain is often undertreated in young children; do not overlook signs. Consider pain assessment as important as vital signs. Use one of many tools to elicit the child’s self-report of pain level. Tachycardia and inconsolability may be the only way a child has to express pain. Findings may be similar to those of early shock or fear. Treatment includes use of a calm, reassuring voice, distraction techniques, and medications when appropriate. Commonly used pain medications include morphine and fentanyl. Analgesia (Pain Control) Nitrous Oxide self administered as needed, may be used concurrently with another analgesic Morphine 0.1mg/kg slow IVP, IO, IM, SC (maximum dose 10mg) Fentanyl 1-2mcg/kg slow IVP, IO, IM, IN (dilute to a volume of 5-10ml prior to IV/IO administration and give over two minutes), IN ½ volume in each nare ,should be rapid and non-diluted-max 1 ml/nare (maximum dose 25mcg) Anxiolytic (Sedation) Valium 0.05mg – 0.2mg/kg IVP/IO/IM(max 5mg) Versed 0.05-0.1mg/kg IVP/IO/IM/IN(max 2mg) Ativan 0.05mg/kg IVP/IO/IM(max 1mg) Consider the use of antiemetics Phergan <6 years: Do not administer 6-12 years: 6.25mg IVPIO (Dilute to a volume of 5-10ml prior to administration) 12-18 years: 12.5mg IVPIO (Dilute to a volume of 5-10ml prior to administration) Zofran 0.1mg/kg (max 4mg) IVP/IO Intubated patients should receive pain medication and sedation if they are hemodynamically stable. The following medications may be used as appropriate in order to facilitate the establishment of an airway: Pre-Medicate with Lidocaine 1-2mg/kg IVP/IO (In the case of suspected head injury) Pre-Medicate children less than ten years of age with Atropine 0.02mg/kg IVP Pre-Medicate with Versed 0.1mg/kg IVP/IO) if blood pressure is greater than 100mmHg systolic (start with small doses (2.5-5mg) and titrate to effect, particularly when giving IV) Pre-Mediate with Fentanyl 1-2mcg/kg- max. dose 25mcg IVP/IO (dilute to a volume of 5-10ml prior to administration and give over two minutes) or Pre-Medication with Morphine 0.1-0.2mg/kg IVP/IO Administer Etomidate 0.4mg/kg IVP/IO May need to be redosed depending on transport time. Side effects of narcotics and benzodiazepines include respiratory depression, hypoxemia, bradycardia, and hypotension. Weigh benefits and risks of administering. Children who are in shock and hemodynamically unstable are not good candidates for narcotics or sedatives. May worsen their already precarious status With all children receiving such medications, carefully monitor pulse rate, respiratory rate, pulse oximetry, and blood pressure.
  41. The initial assessment and management of pediatric burn victims is similar to that of adults, with a few key differences. Larger skin surface-body mass ratio makes them more susceptible to heat and fluid loss. Worrisome patterns of injury or suspicious circumstances should raise concerns of abuse.
  42. Assessment and management of burns Scene safety is an important element in a burn call. Check for ongoing dangers such as fire, chemicals, or other hazardous materials. From-the-doorway assessment may identify signs of smoke inhalation. Eg, abnormal airway sounds, respiratory distress, soot around the nose Quickly move patient and crew to a well-ventilated area. Estimation of percentage of BSA burned may affect decisions on fluids, transport. For adolescents, use rule of nines that you use for adults. For younger children, modify rule to account for disproportionately larger head size. Infants: Head and trunk each account for 18% of BSA; arms 9%; legs 14% each. Size of child’s palm (not including fingers) represents about 1% total BSA. Can use to assess extent of burn
  43. Remove burning clothing, and support ABCs. With signs of smoke inhalation, consider early intubation. Have a range of tubes available. Airway edema and sloughing may mandate use of a smaller tube than originally estimated. Give all burn victims 100% supplemental oxygen. Smoke inhalation may cause bronchospasm resulting in wheezing and mild respiratory distress. Consider a bronchodilator such as albuterol or epinephrine IM. If possible, insert an IV line and initiate fluid resuscitation in transport for patients with more than 5% of burned BSA. Start with 20 mL/kg of isotonic fluid. Reassess the need for additional boluses; large burns can lead to huge fluid shifts.
  44. Clean burned areas minimally to avoid hypothermia; cover with clean, dry cloth. Avoid lotions or ointments; they can trap heat and bacteria. Cover burn and patient as needed to avoid heat loss. THERMAL BURNS Remove clothing which is smoldering or non-adherent to the patient Remove any potentially constricting items IF the BSA burned is greater than 20%, cover with dry dressings If the BSA burned is less than 20%, use cool wet dressings CHEMICAL BURNS Decontaminate Remove clothing Flush involved areas with water Remove any potentially constricting items Wrap burned area in clean dry dressings after decontamination ELECTRICAL BURNS Use reverse triage Follow ACLS protocols Immobilize the cervical spine Prolonged respiratory support may be necessary Apply sterile dressings to entry and exit burns Analgesia is a critical part of early management; burns can be incredibly painful. Pain Management: (Child) Analgesia (Pain Control) Nitrous Oxide self administered as needed, may be used concurrently with another analgesic Morphine 0.1mg/kg slow IVP, IO, IM, SC (maximum dose 10mg) Fentanyl 1-2mcg/kg slow IVP, IO, IM, IN (dilute to a volume of 5-10ml prior to IV/IO administration and give over two minutes), IN ½ volume in each nare ,should be rapid and non-diluted-max 1 ml/nare (maximum dose 25mcg) Consider the use of antiemetics (limit side effects/narcotic agonist) Administer Phenergan as follows: <6yrs: Do not administer 6-12yrs: 6.25mg IVP/IO (dilute to a volume of 5-10ml prior to administration) 12-18yrs: 12.5mg IVP/IO (dilute to a volume of 5-10ml prior to administration) May use Zofran Children: 0.1mg/kg (max of 4mg) IVPO Carefully monitor any child given narcotics or benzodiazepines for signs of respiratory or hemodynamic compromise. Transport to an appropriate medical facility once the patient’s condition is stabilized. Larger burns, full-thickness burns, and burns involving the face and neck are best treated at a regional burn center.
  45. Child abuse is any improper or excessive action that injures or otherwise harms a child or infant; it includes physical, sexual, and emotional abuse as well as neglect. In 2009, national child protective services reported more than 800,000 confirmed cases of child abuse in the United States resulting in approximately 2,000 deaths. Physical abuse: Inflicting injury to a child. Sexual abuse: An adult engaging in sexual activity with a child, ranging from inappropriate touching to intercourse. Emotional abuse and child neglect: Often difficult to identify, may go unreported. Neglect: Refusal or failure on the part of the caregiver to provide life necessities. Emotional abuse: Lack of emotional support by the caregiver. Keep the possibility of abuse in mind with injured children. Information you gather from scene size-up and interviews may prove invaluable. Act on any suspicions of abuse. Child abuse involves a pattern of behavior. A child who is abused once is likely to be abused again, possibly fatally.
  46. Risk factors for abuse Certain risk factors make abuse more likely. Younger children Children who require extra attention (eg, handicaps, chronic illnesses, other developmental problems) Occurs in all socioeconomic levels, but more prevalent in lower socioeconomic status Divorce, financial problems, and illness Drug and alcohol abuse Domestic violence in the home
  47. Suspecting abuse or neglect When you are called to the home of an injured child and suspect abuse, trust your instincts. Look for “red flags” (summarized in the mnemonic CHILD ABUSE): History inconsistent with the type of injury— eg, a child who fell from a tree with bruises only on the buttocks Account of the injury that is inconsistent with developmental abilities—for example, a 2-month-old infant rolling off a bed An old injury that went unreported Inappropriate actions or language from the caregiver
  48. Assessment and management of abuse and neglect To recognize abuse, you first have to suspect it. Carefully document what you see. Difficult but important to remain impartial. Record what you see and hear in a detailed incident report, but do not editorialize. Include notes about child’s environment, condition of home, and interactions among caregivers, child, and EMS crew. Record concerning comments verbatim, who made them, and when. In all states, prehospital personnel are mandated reporters. Legal obligation to report suspicion of abuse to child protective services and law enforcement. Failure to report may be a crime; review state law. Reporting is done in conjunction with emergency physicians, social workers, and child protection teams at the ED.
  49. Involving police early is important to secure the crime scene and collect evidence. In cases where death occurs, the medical examiner will investigate the cause of death. Notify of any death if child is not being transported to the ED. Approach ED staff with concerns, not the caregiver. Be aware of local regulations; you may have a legal—and an ethical—obligation to report to local child protection services (CPS). Primary objective of CPS: Ensure children’s safety within their families. Scene can be emotional; focus on trauma assessment and management, and then the child’s safety. Base general impression on PAT; may range from normal to grossly abnormal. Minor inflicted injuries to severe internal or central nervous system injuries Shaken baby syndrome: May find abnormal appearance, no external signs of injury Severe brain injury when a caregiver violently shakes the infant.
  50. Few caregivers will admit to having hurt the child; be alert for a history that is inconsistent with the clinical picture. Check skin while looking for bruises, especially in different stages of healing or in concerning locations. Active toddlers often have bruises on their shins but rarely on backs or buttocks. Bruises in identifiable patterns are rarely incurred accidentally. Eg, belt buckles, looped cords, or straight lines Use CHILD ABUSE mnemonic when you obtain additional history. Ask yourself: “Does the caregiver’s explanation make sense?” Could this child produce this bruise or injury through normal activities?”
  51. Mimics of abuse Can be difficult to distinguish some normal skin findings from inflicted injuries. Mongolian spots Birthmarks generally found on the lower back and buttocks of children of Asian or African American descent May be mistaken for bruises because of their unique bluish coloring
  52. Other medical conditions can mimic bruises. Purpura of meningitis, Henoch-Schönlein purpura Petechiae of idiopathic thrombocytopenic purpura and leukemia Exposure to sun can cause reactions with certain medications or fruits (limes, mangos) Causes phytophotodermatitis, reddish-purple skin discolorations.
  53. Certain cultural customs also produce skin markings that can mimic child abuse. Coining and cupping: Traditional Asian practices, often used to treat fever Markings can be impressive but practice is not harmful
