chimiothérapie (Adria-cisplatine-isfosfamideRituximab) 8 cycles
• HDM: 8j après dernier cycle, apparition d’un ictère
avec asthénie majeure

  • 2. • ATCD: ostéosarcome fémur G amputé il y a 6 mois + chimiothérapie (Adria-cisplatine-isfosfamideRituximab) 8 cycles • HDM: 8j après dernier cycle, apparition d’un ictère avec asthénie majeure
  • 3. • Interrogatoire: – – – – – Pas de notion d’hépatopathie chronique Pas de prise d’alcool, drogue Prise de paracétamol 2g/jour Pas de notion d’infection chronique au VHB Pas de notion de vaccination anti-VHB • Examen clinique: – Pas d’astérixis, pas de confusion – Pas de signes de localisation neurologiques – Ictère conjonctival franc, pas d’autres signes d’insuffisance hépatique

  4. In terms of risk factors, because the data is somewhat scant, it’s a little bit hard to make strong identifications of risk factors. However, some suggestions at least come from the literature that suggests that if you look at the agents, although there are no direct comparisons with the anti‑TNF drugs, there are at least more reports with infliximab and adalimumab than with etanercept. And in general, the same idea as with oncology applies, that the more immunosuppressive the regime, the higher the risk. The duration—as one would expect, increased risk is associated with prolonged therapy, however, there have been reports with even single doses of these biologic agents. In terms of the HBV profile, those who are HBsAg positive are at much higher risk than those who are anti-HBc positive alone, and if they have HBV DNA positivity at baseline, this certainly increases the risk.
  5. So here I’ve shown data from a couple of studies and the first study is self-reported hepatitis B screening among 131 American oncologists. And what you see is that 62% of them do no screening, 24% screen only high-risk, and a very small percentage, just 14% of oncologists, screen all patients prior to starting chemotherapy. Similarly, in a chart review of actual screening done at an urban hospital in Toronto, only 14% of patients were actually screened prior to starting chemotherapy. So the point is that few oncologists are routinely screening all patients prior to initiating chemotherapy.  
  6. So one of the problems with using a high-risk screening strategy is you have to know who high-risk patients are. So if you look among oncologists and you ask them what are the risk factors for hepatitis B infection, you can see that the majority of them do not recognize the most important risk factors. So this is from an abstract presented at the ASCO meeting in 2010 and shows that only 30% of oncologists recognized the country of origin as being the most important risk factor for chronic hepatitis B, and only a quarter of oncologists were aware of HBV guidelines on screening. So you can see that high-risk screening is a difficult position to advocate because high-risk screening requires recognition of high-risk populations, which at least to date, the data would suggest is not true among the oncology population.
