follow ups. The transition that young adults go through when changing into adult care is not easy
or simple for the nurses and the patients as well as their families. Many adolescents don t know
about their own cardiac history whether it is their diagnoses, catheterization procedures, surgery or
their medications. Caring for adult patients can be difficult for pediatric nurses. As adult patients
