Minimally Invasive Spine Tumor Surgery
Tumor is abnormal mass of rapidly growing cells without any
physiological function. That means it does not perform any function
for the body but derives its nutrition from the body. It can occur
anywhere in the body. The cause is unknown. Those tumors that are
found in and around the spinal cord are known as spinal tumors.
They may be primary tumors whose cells of origin is the spinal
tissue, or they may be secondary tumors which have spread (metastasize) via blood stream
from some other focus in the body.
The Spinal cord is covered by a layer of protective tissue called as meninges. The entire
spinal cord is encased within the vertebral column.

The Spinal tumor are of three types depending on
their location:
• Intramedullary – within the substance of the spinal cord.

• Intradural-Extramedullary- within the layers (meninges)
covering the spinal cord
• Extradural- between the meninges an the vertebral body. Most of the spinal tumors are
extradural.

Tumors are also classified as benign and malignant. Tumors that are confined to a
particular area in the body are known as benign tumors. Those tumors that spread rapidly
to various parts of the body are known as malignant tumor. A tumor can initially start as a
benign one and can convert to a malignant one in a few cases.
The benign tumors are easier to treat as they are localized in their extent and their complete
removal is possible. Whereas incase of metastatic or malignant tumor, the earlier they are
detected, the better the chances of their removal. But in the malignant tumor the chances of
recurrence are also there as they are rapidly growing tumors.

Objective of Spinal Tumor Surgery
The Primary objective of the surgery is to reduce the pain caused by the spinal
tumor, to restore and preserve neurological function and provide spinal stability.
Spinal Tumor surgery is generally indicated in case there is a localized tumor which can
be removed with minimum damage to nerve and spinal cord, or there is persistent
neurological deficit and pain which is unresponsive to non operative treatment, or
there is vertebral bone destruction affecting spinal stability. Surgery may include
resection (partial removal) or excision (complete removal) of tumor. When the tumor is
removed (partially or completely) pain and neurologic problems may clear up.

Details of the Surgery
Treatment for spinal tumor comprises of Non-surgical
procedures like radiation and chemotherapy and surgical
procedures like Percutaneous spinal tumor surgery, conventional
open surgery or sometimes the more extensive approach, in
which the surgery is performed in several phases, depending on
the extent of tumor. The choice of treatment depends on various
factors like the age & overall health of the patient and the type
and extent of tumor. These factors are of paramount importance,
since the spinal cord is a very complex structure and needs careful handling, else the
treatment may have adverse effects, leading to permanent damage to the surrounding nerves
and spinal cord. Ideally, while treating a spinal tumor, the aim of our expert surgeons is to
completely eliminate the tumor.
Percutaneous curettage/ scrapping technique is used in cases where there are no
neuorological deficits. Since it is a minimally invasive procedure, the postoperative hospital
stay is short and the recovery is faster.

In case of Primary tumor of spinal column, an extensive approach is used which is completed
in two stages. The first stage includes separating the tumor from the spinal cord and
generally uses the posterior approach, i.e. the surgery is performed from the back. The
second stage involves an anterior approach, in which the tumor is released from blood
vessels and other organs. Finally the tumor is resected (partial removal) or excised (complete
removal) removed. This is referred to as decompression surgery. At times just the bulk of
tumor is made smaller (debulk) to relieve the compression. In certain cases the blood supply
to the tumor may be cut off causing the tumor to shrink. This is known as embolisation. If
there is associated vertebral body collapse and destruction, it is reinforced and reconstructed
by kyphoplasty or vertebroplasty or spinal fusion. Once the tumor is removed, the spine is
stabilized, using rods, wires, screws and other instrumentation. The spine may be fused or
solidified for stabilization.
Radiosurgery (CyberKnife): This is a non-surgical procedure that delivers precisely
targeted radiation to treat certain spine tumors. Radiosurgery treatment is administered
during one or more sessions using high-dose of radiation. This treatment does not
immediately remove the tumor like surgery. Rather, the tumor disappears with time.
