2. 什麼是fibromyalgia?
什麼是纖維肌痛症?

4. 纖維肌痛症（Fibromyalgia）
 是一種疼痛病症，臨床上主要的症狀為慢性廣泛疼痛與
壓痛，
 患者常抱怨身體到處在痛，同時身體也很容易的因按壓
而引發疼痛，
 通常做了一堆檢查也查不出個所以然來。
 纖維肌痛的患者有許多臨床常見的疾病，如憂鬱症、焦
慮症、睡眠障礙、疲勞無力、頭痛、顳顎關節障礙症、
麻木刺痛、大腸急躁症、忘東忘西、無法專心等，這些
症狀會大大降低了患者的生活品質。

7. 纖維肌痛症的症狀
 全身到處都痛，沒有依個明確的定點，
 不管怎麼休息還都是累，
 睡不好，記憶力變差，
 去醫院檢查老半天都沒有問題。

9. 那些人容易得纖維肌痛症
 所有人口約2%會發生纖維肌痛症。
 其中，以成年女性佔大多數，約為男性患者的7倍左右
 好發年齡在三十至四十幾歲之間。
 發生纖維肌痛症的原因有許多，其中大約有百分之四十
的病人 被認為沒有明顯的原因，稱為原發性纖維肌痛症。
 此外，頸部的受傷而導致纖維肌痛症的案例也不少，有
時會造成廣泛性的背部疼痛。不良的工作姿勢、長期的
疲勞、情感悲傷的心因素、或其他的免疫因素都可能是
造成纖維肌痛症的原因。

沒有特定的檢查來確診!
 若血液檢查或是診斷影像有異常時，亦不能排
除纖維肌痛症的可能!

怎麼治療? 會不會好?

21. 纖維肌痛症
 診斷治療 - 知道這個病，就好一半。
 藥物治療 – 抗癲癇，抗憂鬱藥物治療，止痛藥
FAD 核定纖維肌痛症用藥:
Pregabalin (Lyrica 利瑞卡 75mg)
Dulxetine (Cymbalta 千憂解 30mg)
Milnacipran
 行為認知治療

22. 纖維肌痛症 藥物治療
 通常傳統的止痛藥是不太有
用的
 在治療方面，過去採取一般
止痛藥治療的方式效果不佳，
加上患者常合併焦慮與憂鬱
症，因而許多醫生將此症視
為精神疾患，因此有許多種
藥被拿來治療此疾病。

23. 藥物治療
 弱效鴉片類藥物
 抗憂鬱藥（amitriptyline、fluoxetine、duloxetine、
milnacipran、meclobemide、pirlindole）、
tropisetron、pramipexole、
 FAD 核定纖維肌痛症用藥:
Pregabalin (Lyrica 利瑞卡 75mg)
Dulxetine (Cymbalta 千憂解 30mg)

25. 其他非藥物治療
 在非藥物治療方面，熱浴療法（heated pool
therapy）為B級，其他如運動課程、物理治療、
精神支持、放鬆療法等。
 疼痛點局部注射可能會造成依賴性產生，一般
不建議使用於此類患者。

29. Herpes zoster
 Herpes zoster (shingles) presents as a painful
vesicular rash and is caused by reactivation of
the varicella-zoster virus within the dorsal root
or cranial nerve ganglia.
 Abnormal skin sensations range from itching
and burning to hyperesthesia and severe pain.

30. Management of Acute Herpes Zoster
 Antiviral therapy is first-line treatment and
should be initiated within 72 hours of rash
onset to increase the rate of healing and
decrease pain.
 Acyclovir (Zovirax),
 Famciclovir (Famvir),
 Valacyclovir (Valtrex)

32. Postherpetic neuralgia
 Postherpetic neuralgia is the most common
complication of herpes zoster.
 >80 y/o : 30%
 60~65 y/o: 20%
 <50 y/o: rare

Capsaicin : 紅辣椒(red papper)的活性成分，具選擇性
作用於 (c-fiber)及 (Aδ- fiber)的週邊初級感覺神經元 (去
活化)

35. Lidocaine 5 % patch
 In clinical trials (4–12 weeks), 50 % reductions
in pain have been reported in approximately
one-quarter to one third of patients.
 Moreover, results from two 4-week open-label
clinical studies indicated that the lidocaine 5 %
patch was more effective than pregabalin for
alleviating PHN pain
限用於皮膚帶狀疱疹後神經痛，並符合下列：1.其他止痛劑或NSAIDs治療後仍無法控制疼痛或有
嚴重副作用 2.每日最大劑量3片，日劑量超過2片時，需於病歷記載理由。症狀改善應逐步調低劑
量 3.不得與Gabapentin口服併用

36. Tricyclic antidepressants
 50 % reduction in PHN pain .
 Despite their efficacy, tricyclic antidepressants
for PHN may not be the most appropriate option
for all patients.
 These agents display a relatively slow onset of
action and have the potential for troublesome
systemic adverse events, such as dry mouth,
constipation, urinary retention and sedation.

37. Lyrica for postherpetic neuralgia
 The anticonvulsants gabapentin and
pregabalin bind to α2-δ proteins, where they
act as voltage-gated calcium channel blockers,
inhibiting central pain pathways

40. Gabapentin
 Gabapentin is started at 300 mg daily, with an
increaseof 300 mg every day as needed for pain relief.
 The recommended dose range is 1,800–3,600 mg/day.
 A daily dose greater than 1,800 mg is not generally of
greater benefit.
 Pain relief occurs as early as one week after the start of
the treatment.
 The common side effects are somnolence (frequent),
dizziness, and transient peripheral edema.

42. Lyrica
 The initial dose is 150 mg/day in divided doses
(75 mg twice daily or 50 mg three times daily).
 Could be increased to 300 mg/day within one
week
 Further dose titration (to 600 mg/ day) after 2–
4 weeks
 The maximum dose is 600 mg/day.

(Zostavax) for persons 60 years and older.

帶狀疱疹疫苗不會因為施打
年齡而影響效果，無論是60~69、
70~79、或是80歲以上，都能使帶
狀疱疹的患病率下降55%
