HEALTHY CAPITAL DISTRICT INITIATIVE OF NEW YORK January to July 2015
The Healthy Capital District Initiative (HCDI) is an incorporated not for profit that has helped over 35,000 needy
children and adults in the Capital Region get health services that they might not have accessed otherwise.
Director of Health Planning
• Direct the Population Health Improvement Program, a five year New York State health systems
transformation initiative designed to support shared measurement as well as assessment practices in the
six-county Capital Region which includes Albany, Columbia, Greene, Rensselaer, Saratoga and
Schenectady. The PHIP supports and advances the agenda of the State Health Innovation Plan, the
Prevention Agenda and the Design Systems Reform Incentive Payment programs. 50% NYSDOH
budget cut mid year.

• Developed and executed assigned course in Global Public Health for undergraduate health administration

AmeriHealth Caritas Partnership is 501(c)3 nonprofit division of AmeriHealth Caritas, and whose key goal is
to address health disparities nationally by implementing evidence based award-winning initiatives that
educate underserved communities and promote the importance of healthy behaviors.
Director of Health Promotion and Program Development
• Managed and evaluated six public health education programs of varying lengths, between 40 days and
one day events with total program budget of $300,000 serving 1,700 people, associated with public health
signature programs of the AmeriHealth Caritas Partnership. Supervised 1 staff.
• Collaborated with stakeholders in three cities (community-based organizations/experts), increasing
Healthcare Effectiveness Data and Information Set (HEDIS) standing; significantly improved medication
compliance for asthma management, reduced blood pressure, risk of heart attack, with non-fasting
glucose mean reduction of 8%.
• Managed team implementation for accredited Second Annual Dental Medical Conference, a forum for
135 academic, medical and dental providers identifying leading edge models that integrate the practice of
dentistry with physical health care delivery, in collaboration with the State University of New Jersey and
Rowan School of Osteopathic Medicine, Temple Kornberg School of Dentistry and Keystone First.
• Composed a variety of reports and program documentation to execute and complete the conference,
awarding 6.5 contact hours for those in attendance.
ESPERANZA, INC., Philadelphia, PA October 2010-June 2012
Esperanza is one of the largest Hispanic Faith-Based Evangelical networks in the United States. With a
national network of 12,000 faith and community-based agencies, Esperanza is one of the leading advocates
for Hispanics in America.
Vice President, Interim, National Programs (consultant)
• As interim head of division of a federal intermediary, directed and managed National Programs, a stand-
alone division with a $3.2 million budget and 9 staff. Provided oversight and technical management of
federal contracts, and cooperative agreements for programs funded by Department of Health and Human
Services, Department of Justice OJJDP and Housing and Urban Development. Supported the search for
appropriate permanent executive placement.
• Innovated and leveraged best practices for six community and faith based organizations in different states
to provide mentoring and social resilience skills for 1200 at risk Latino youth over two years and five
community and faith based organizations to provide HUD-approved housing counseling services to over
300 client families over two years in under-resourced communities in the multiple cities across the country.
DIVISION OF SOCIAL SERVICES, CITY OF PHILADELPHIA, Philadelphia, PA June 2005-May 2008
The Managing Director’s Office serves at the pleasure of the Mayor to oversee the effective operations of all
city departments and engage in special projects for the Mayor, as needed.

• Co-directed a six-person task force as well as multiple, interdisciplinary subcommittees to execute the
Mayor’s social services initiative to implement an $8M technology solution and data warehouse to effect a
single integrated interface and service delivery across six distinct health and human services-related
platforms. Published article on lessons learned, in Philadelphia Social Innovations Journal 2012.
• Established systematic methods for public health and social services program execution implementing
evidence based practice in the City’s social service agencies, such as teaching parenting skills in the
homeless shelters to reduce child maltreatment, increasing odds of success and school readiness.

National Programs Developer
• Developed, launched, and directed a three-year comprehensive training programs as part of the $2.5M
Compassion Capital Fund Grant established to build organizational capacity. Developed curriculum that
enabled 70%+ of organizations in the cohort to apply for and receive grant funding, which enabled
expansion of their management capacity and strengthened support for their community-based missions.
PHILADELPHIA HOUSING AUTHORITY, Philadelphia, PA September 2001-May 2002
Established in 1937, PHA is the nation’s fourth largest public housing authority, with more than 14,000
housing units, serving nearly 80,000 Philadelphians.
Assistant General Manager
• Led the PHA Social Services division implementing the planning, timeline development, and rollout of
programs and proposals, including the Move to Work campaign. Provided oversight, technical
management and compliance for federal programs like Head Start and H.O.P.E. VI, established to
support healthy child development and broad scale revitalization initiatives of inner-city public housing
communities.
PHILADELPHIA DEPARTMENT OF PUBLIC HEALTH, Philadelphia, PA September 1998- June 2001
The Philadelphia Department of Public Health oversees 13 divisions with the mission to protect the health of
all 1.5 million Philadelphians.

• Established systematic methods for the health-planning unit for execution of local community health
assessments and Community Health Improvement Plans, identifying evaluation measures to alleviate
impediments to identifying root causes for improving health indicators for individuals, families, and vulnerable

communities, analyzed programs and policies, and devised execution plans for public health objectives on
issues of neighborhood transformation, infant mortality, domestic violence, and others.

The Salt Lake Valley Health Department assures the health of 1.3 million residents by enforcing laws and
providing services that support the health of all residents.
Health Planner / Community Outreach, Health Commissioner’s Office
• Led the development of round table Community Health Assessment and Improvement Planning
committees with stakeholders across multiple sectors, with subject matter experts including local chamber
of commerce, State Health Department, local academics, hospitals and providers and the local managed
care company, i.e. Intermountain Health Care. Facilitated identification of local priorities to make
strategic decisions and facilitate initiatives to impact on level of clinical community care such as
increasing childhood vaccinations and reduction of injuries such as the installation of stop signs and traffic
signals in heavily trafficked areas.
• Created the public health concept and implementation of first-ever cultural competence policy and
curriculum improving communications between the Public Health Department management and affected
populations.
• Identified potential problem areas for implementation of cultural competence policy and programming was
deployed across other municipal human service agencies such as police departments and child welfare.
Formulated program responses and recommended public health program improvements. Generated
local media attention.

PROFESSIONAL EXPERIENCE HEALTHY CAPITAL DISTRICT INITIATIVE OF NEW YORK January to July 2015 The Healthy Capital District Initiative (HCDI) is an incorporated not for profit that has helped over 35,000 needy children and adults in the Capital Region get health services that they might not have accessed otherwise. Director of Health Planning • Direct the Population Health Improvement Program, a five year New York State health systems transformation initiative designed to support shared measurement as well as assessment practices in the six-county Capital Region which includes Albany, Columbia, Greene, Rensselaer, Saratoga and Schenectady. Director of Health Promotion and Program Development • Managed and evaluated six public health education programs of varying lengths, between 40 days and one day events with total program budget of $300,000 serving 1,700 people, associated with public health signature programs of the AmeriHealth Caritas Partnership. Supervised 1 staff. • Collaborated with stakeholders in three cities (community-based organizations/experts), increasing Healthcare Effectiveness Data and Information Set (HEDIS) standing; significantly improved medication compliance for asthma management, reduced blood pressure, risk of heart attack, with non-fasting glucose mean reduction of 8%. • Managed team implementation for accredited Second Annual Dental Medical Conference, a forum for 135 academic, medical and dental providers identifying leading edge models that integrate the practice of dentistry with physical health care delivery, in collaboration with the State University of New Jersey and Rowan School of Osteopathic Medicine, Temple Kornberg School of Dentistry and Keystone First. • Composed a variety of reports and program documentation to execute and complete the conference, awarding 6.5 contact hours for those in attendance. ESPERANZA, INC., Philadelphia, PA October 2010-June 2012 Esperanza is one of the largest Hispanic Faith-Based Evangelical networks in the United States. With a national network of 12,000 faith and community-based agencies, Esperanza is one of the leading advocates for Hispanics in America. Vice President, Interim, National Programs (consultant) • As interim head of division of a federal intermediary, directed and managed National Programs, a stand- alone division with a $3.2 million budget and 9 staff. Provided oversight and technical management of federal contracts, and cooperative agreements for programs funded by Department of Health and Human Services, Department of Justice OJJDP and Housing and Urban Development. Supported the search for appropriate permanent executive placement. • Innovated and leveraged best practices for six community and faith based organizations in different states to provide mentoring and social resilience skills for 1200 at risk Latino youth over two years and five community and faith based organizations to provide HUD-approved housing counseling services to over 300 client families over two years in under-resourced communities in the multiple cities across the country. DIVISION OF SOCIAL SERVICES, CITY OF PHILADELPHIA, Philadelphia, PA June 2005-May 2008 The Managing Director’s Office serves at the pleasure of the Mayor to oversee the effective operations of all city departments and engage in special projects for the Mayor, as needed. Assistant Managing Director • Co-directed a six-person task force as well as multiple, interdisciplinary subcommittees to execute the Mayor’s social services initiative to implement an $8M technology solution and data warehouse to effect a single integrated interface and service delivery across six distinct health and human services-related platforms. Published article on lessons learned, in Philadelphia Social Innovations Journal 2012. • Established systematic methods for public health and social services program execution implementing evidence based practice in the City’s social service agencies, such as teaching parenting skills in the homeless shelters to reduce child maltreatment, increasing odds of success and school readiness.

National Programs Developer • Developed, launched, and directed a three-year comprehensive training programs as part of the $2.5M Compassion Capital Fund Grant established to build organizational capacity. Developed curriculum that enabled 70%+ of organizations in the cohort to apply for and receive grant funding, which enabled expansion of their management capacity and strengthened support for their community-based missions. PHILADELPHIA HOUSING AUTHORITY, Philadelphia, PA September 2001-May 2002 Established in 1937, PHA is the nation’s fourth largest public housing authority, with more than 14,000 housing units, serving nearly 80,000 Philadelphians. Assistant General Manager • Led the PHA Social Services division implementing the planning, timeline development, and rollout of programs and proposals, including the Move to Work campaign. Provided oversight, technical management and compliance for federal programs like Head Start and H.O.P.E. VI, established to support healthy child development and broad scale revitalization initiatives of inner-city public housing communities. PHILADELPHIA DEPARTMENT OF PUBLIC HEALTH, Philadelphia, PA September 1998- June 2001 The Philadelphia Department of Public Health oversees 13 divisions with the mission to protect the health of all 1.5 million Philadelphians. SALT LAKE VALLEY DEPARTMENT OF PUBLIC HEALTH, Salt Lake City, UT 1996- 1998 The Salt Lake Valley Health Department assures the health of 1.3 million residents by enforcing laws and providing services that support the health of all residents. Health Planner / Community Outreach, Health Commissioner’s Office • Led the development of round table Community Health Assessment and Improvement Planning committees with stakeholders across multiple sectors, with subject matter experts including local chamber of commerce, State Health Department, local academics, hospitals and providers and the local managed care company, i.e. Intermountain Health Care. Facilitated identification of local priorities to make strategic decisions and facilitate initiatives to impact on level of clinical community care such as increasing childhood vaccinations and reduction of injuries such as the installation of stop signs and traffic signals in heavily trafficked areas. • Created the public health concept and implementation of first-ever cultural competence policy and curriculum improving communications between the Public Health Department management and affected populations. • Identified potential problem areas for implementation of cultural competence policy and programming was deployed across other municipal human service agencies such as police departments and child welfare.
