- Thank you for joining us to learn how to complete the one-touch home evaluation. You are taking an important step to help enhance the lives of your clients. The fifteen minutes you take to complete the home evaluation will lead to opportunities for the residents to improve their health
- Healthy Homes is a national initiative from the Center for Disease Control and Prevention determined to enhance the health and safety of the Americans in their homes. [Hand begins moving in] New Hampshire has brought this initiative to an active format. One-touch activates an entire network
- Housing conditions may be attributed to many diseases such as asthma, lead poisoning, lung cancer and unintentional injuries. Focus is to identify health, safety, and quality of life issues in homes and to act to eliminate and mitigate the problems.
- The principles of a Healthy Home are keep it dry, clean, well ventilated, free from contaminants and pests, safe, well-maintained, and accessible.
- There are four steps to achieving the healthy homes goals which are activated in the One-Touch process; assessment, education, referral, and fixing. The One-touch check up form will guide you through the process.

- Educating the residents using the tool box is the next step. The tool box contains written information about unsafe conditions in the home.
- To help the residents improve their situation it is important to refer them to the appropriate agencies and individuals in your region for further information.
- The ultimate goal of the process is fixing the unhealthy and unsafe conditions with the help of the homeowner or landlord.
- This assessment is quick, easy, and a value added benefit to your preplanned visit!
- As a home visitor your role is to assess potential unhealthy and unsafe conditions, educate the resident about these potential harmful situations, and to make referrals to the appropriate agencies and individuals to help mitigate these conditions.
- If you take a few minutes before you leave for you scheduled visit to gather a few materials you will be able to make the most of your visit. Print the One-Touch Check Up form and complete as much of the program information and demographics section as you can. You may need to ask your client for additional information after you arrive. Gather your tool box. Bring your regions partner referral form.
- Finish completing the client information and demographics section of the Check-Up form while paying particular attention to age, disability, and insurance information. This data will aid with referrals in the future. Obtain client consent. This is necessary to complete the referral process. If permission is not obtained additional referrals cannot occur. [What about saying: This may be a good time to obtain client consent which will be discussed later in the presentation.]
- You don’t have to be an expert in energy efficiency. The one-touch check up form is designed to guide you through the process.
- Determine what type of fuel the home is heated with; oil, gas, wood, or other sources.
- How old is the heating system? Is it energy efficient?
- Ask if the occupants were cold last winter? What was the thermostat set at? Were parts of the home shut off during the winter in order to aid with maintaining heat?
- What is the condition of the insulation in the attic? Are there cold spots on the walls?
- Does the home consume large amounts of energy? If it is possible collect one year’s worth of heating bills.
- The series of question about occupant health will help to determine if conditions in the home are impacting the well being of the residents. The one touch check up form will help you to determine what you need to ask. Is anyone under the age of 18 living in the house diagnosed with asthma? Does he or she take any daily medications for asthma? Were any unplanned medical visits during the past 6 months related to asthma? Do any residents of the home have respiratory problems? Do occupants of the home have regular flu like symptoms or headaches when in the home? Do any occupants of the home smoke? Does smoking occur in the home?
- Depending upon how your occupant answers these questions, you have a number of resources at your disposal. If your occupant answers yes to any part of Question 1, then they may qualify for a referral to one of your partner asthma programs. Further on in this presentation we will explain how to initiate a referral in more detail.
- One-touch activates an entire network. The one-touch form will help you to assess the environmental conditions of the home.
- The age of the home can lead to potential risks related to lead paint. Homes built prior to 1978 are likely to contain this type of paint.
- Determine the age of the home. Observe for flaking, peeling and chipping paint, particularly around windows.
- Be aware of the inside and outside of the home.
- Look for evidence of pest, including rodent droppings, cockroaches, teeth marks, and bed bugs.
- Are there smoke and carbon monoxide detectors in the home? Are these alarms working?
- Did you notice any unvented space heaters or cloth dryers. Include your observations on the one-touch form.
- Determine if there are issues with excessive moisture in the home which can lead to mold. Do you see wet spot, water stains, or mold?
- Observe for areas of mold. This may appear as a discolored area on the wall or ceiling. Remember to detail your observations on the check up form.
- Did you notice any condensation in the attic or on windows of the building?
- Look for previous water issues which are indicated by rotting wood, water stains and occupant reports. Observe all areas of the home including the attic, rooms and basement.
- Observe for volatile organic compounds. These may include air fresheners, candles, paints, gasoline, solvents, cleaning products and adhesives.
- Preventing unintentional injuries is part of maintaining a Healthy Home. As you tour the home there are a few questions on the one- touch check up form that you should be aware of. [delete the word “unintentional” above] If there are older residents in the home, did you notice any grab bars in the bathroom. These may be needed by the toilet, bathtub or sink areas.
- Are there handrails present on the stairs, down long hallways (for the elderly) or in places of level changes?
- Does there appear to be sufficient lighting inside and outside of the home? Do the lights work?
- In the case of young children, are there child safety locks, gates on stairways, fencing around pools, are the cords from drapes raised and secured to prevent strangulation, is gun safety maintained, are chemical and medications locked and out of reach, and are window guards present?
- One-Touch activates an entire network. Being proactive during your visit and providing education is a key to maximizing your assessment.
- Using the materials in your tool box you can provide education about unsafe and unhealthy situations. You will find materials on smoking cessation, radon testing and abatement, the hazards of lead paint, asthma prevention and many other topics. This step in the process will help to empower your client to take control of their health and well being.
- : Be sure that you gained the consent of the occupant on the first page of the check up form.
- If you do not have it you will not be able to make referrals to the appropriate agencies and partners in the network
- Referrals should be made to the appropriate agencies in your region.
- Be sure to complete all referrals in a timely manner.
- This will allow your clients to make the appropriate changes to fix the unsafe and unhealthy conditions in the home and improve their living conditions.
- If there are issues with heating, your client may be eligible for an energy audit. The experts in housing and energy issues can direct the occupants to solutions for their heating problems.
- The documentation form should be completed per the process of your agency.
- Documentation is essential to determine the continued use of this program and the impact that Healthy Homes New Hampshire is having in the state.
