DURATION-8 was a Phase III, randomized, double-blind, active-controlled, parallel-group multicenter, 28 week study evaluating the efficacy and safety of co-initiating treatment with exenatide once weekly (EQW) 2 mg and dapagliflozin (DAPA) 10 mg once daily (n=231) compared to EQW 2 mg alone (n=230) and DAPA 10 mg QD alone (n=233) in patients with type 2 diabetes (T2D) who have inadequate glycemic control on metformin (MET) and diet and exercise.
There was a screening visit and a 1 week placebo (PBO) lead in period prior to randomization.
Inclusion criteria included: ≥18 years with T2D and A1C of 8.0–12.0%, despite stable MET(≥1500 mg/day) for ≥2 months.
Exclusion criteria included: exposure to exenatide or any GLP-1 RA; exposure to DAPA or any SGLT2i; fasting plasma glucose ≥280 mg/dL; serum calcitonin concentration ≥40 pg/mL; clinically significant abnormal free T4 values or patients needing initiation or adjustment of thyroid treatment according to the investigator. Abnormal thyroid-stimulating hormone value at screening will be further evaluated by free T4; presence or history of medullary thyroid carcinoma or MEN 2 or a family history of medullary thyroid carcinoma or MEN 2; known active proliferative retinopathy; history of, or currently have, acute or chronic pancreatitis, or have triglyceride concentrations ≥500 mg/dL; creatinine clearance <60 mL/min or a measured serum creatinine value of ≥1.5 mg/dL for male patients and ≥1.4 mg/dL for female patients; history or presence of inflammatory bowel disease or other severe GI diseases; history of unstable or rapidly progressing renal disease.
Primary outcome was the change in A1C from baseline to 28 weeks.
Secondary outcome measures included change in total body weight, change in FPG, change in 2-hour PPG, proportion of patients achieving A1C<7%; proportion of patients achieving weight loss ≥5%; and change in seated systolic blood pressure at 28 weeks.
Exploratory endpoints included change in A1C by baseline A1C and change in weight by baseline A1C.
After the primary 28-week treatment period, patients will enter a double blind 24-week extension period and a subsequent 52-week extension period (total of 2 years).
Patients requiring rescue therapy based on fasting plasma glucose (FPG) criteria starting at week 8 received open-label titrated basal insulin.

Of the 695 patients enrolled and randomized, 3 patients in the EQW+DAPA group, 4 patients in the EQW group, and 3 patients in the DAPA group were excluded from the intent to treat (ITT) population due to missing post-baseline A1C assessment.
ITT was defined as randomized patients who received ≥1 dose of study drug with at least 1 post-baseline A1C assessment.
Overall, demographic and baseline characteristics were comparable across treatment groups, with the exception of fewer women in the EQW group and fewer hispanic patients in the DAPA group.
