  3. 3. STANDARD IMPLANTS Implant lengths of 10mm or greater have been considered the standard in implant dentistry. Implants are generally available in lengths from about 6 mm to as much as 20 mm. The most common lengths employed are between 8 and 15 mm, which correspond quite closely to normal root lengths. Most implants are approximately 4 mm in diameter A diameter of at least 3.3 mm is normally recommended to ensure adequate implant strength.

Sinus Lifting Grafting of the maxillary sinus is most commonly used and a very predictable procedure for vertical bone augmentation in the posterior maxilla Sinus floor augmentation is either performed by an external or an internal (transcrestal) approach Sinus floor augmentation can be performed with immediate or delayed implant placement Bone substitute materials are as effective as autogenous bone for sinus floor augmentation. Less than 4-6 mm of vertical residual bone height in the posterior segment of the maxilla is an indication for a sinus augmentation procedure

  13. 13. Short Dental Implant Short implants have demonstrated to be a predictable treatment with survival rates between 74-96% at 5 years. The determinant factor for short implant success is known to be the low quality bone, as it compromises primary stability at placement . Short implants showed marginal bone loss, prosthesis failures and complication rates similar to standard implants. The use of a short implant (4 to 8 mm long) is an interesting and straight forward alternate treatment modality No consensus about the definition of short implants, some considering them to be <10 mm , while others consider <8 mm . Current clinical tendencies consider implants ≤7 mm as short or extra-short implants . 03
  14. 14. 04 ALL on 4 ***Two straight implants are inserted in the anterior region (usually lateral incisor) and two dorsally tilted implants in the posterior zone (2nd premolar). ***The limitation for the placement of the dorsal implants is the maxillary sinus and the mental foramen. In the severely resorbed maxilla the concept can be performed by placing zygomatic implants (Pterygoid implants, Zygomatic implants). Modern treatment methods with reduced number of implants and tilted implant placement allow a less invasive treatment approach with only one surgical intervention, and without augmentive surgery
  15. 15. .  Zygomatic implants are an evidence- based surgical and prosthetic solution for both two-stage and immediate loading protocols.  Today, zygomatic implants are usually placed using an immediate loading protocol 05 Zygomatic Implants  The main indication for zygomatic implants is the severely resorbed edentulous maxilla.  Indications for zygomatic implant insertion include: alternative for sinus augmentation, failed sinus augmentation, rehabilitation after tumor resection or trauma, failure of conventional implants, failure of previous bone grafts  The placement of zygomatic implants requires adequate training and surgical experience
  16. 16. 05 Pterygoid Implants The terms "pterygoid implants", "pterygo-maxillary implants" and "tuberosity implants" are used interchangeably in scientific literature (for what is described as implants in the retromolar region). By definition, all "pterygoid implants" encompass the tuberosity region and engage the pterygoid plate but not all "tuberosity implants" necessarily engage the pterygoid plates (they are placed in a more anterior position of the pterygo-maxillary area, parallel to the posterior wall of the sinus).
