assessment was requested and by whom.
 A summary of essential details related to the history of the client, both before you were
the therapist (if relevant) as well as during your work with the client(s) including:
 Duration.How long you have met the client?
 What was the initial presenting problem? History of the problem, treatment history
(Counseling, psychiatric medication, other medical issues).
 Assessment process : Assessment tools and methods used and scoring interpretations.
(Making judgment).
 Diagnosis: What is your judgment of the issue (issues?)
 Treatment: What is your recommended way of supporting the client based on your
diagnosis?
