PAM awareness training •Patient activation concepts. •Administration of the PAM tool and recording results. •How to intepret the results. •How to use the information introducing basic coaching concepts. Health Coaching & Activation skills training •Core Health Coaching Skills Workshop (2 Days) delivered by our trained trainers. Embed •Mentoring session between coaching trainer and PAM champions. •Introduction of PAM into service's 1-2-1, and clinical supervision. •Review of PAM at service caseload reviews, MDT, and huddle's.

Patient Story… Programmes attended: -8 week ‘Eat Well’ programme- education; peer support and goal setting. 1:1 coaching sessions- tracking progress and identifying solutions. 8 week ‘Moving Forward’ programme- goal setting, peer support and future planning. • 20 stone • High Blood Pressure • 3 TIAs • PAM Level 2 Progress: - 4 stone weight loss - Increase to PAM Level 4 - Self-managing healthy diet - Regularly attending community exercise group “I know that it is because I was involved all the way that I found the drive to keep going- being able to set my own parameters with the guidance and support of the team made all the difference. I now feel that I have much more knowledge about what I should and should not be eating and also about what works for me. “ Stephen
