• Med Hx: Non- contributory
• Dent Hx:
– Pt has bike accident at age 7 avulsing #7-9
– 3 separate RCT Tx’s for all 3 teeth
– Soccer accident age 15 avulsing #7-9
– RDP since age 15
– GPR performed initial Evaluation and referral for
orthodontic treatment
– Pt referred to Grad Perio shortly after for vertical ridge
augmentation and implant placement #7-9
