  • Es la manifestación clínica de las alteraciones bioquímicas producidas por el aumento de la permeabilidad de la barrera de filtración glomerular que da origen a proteinuria masiva con hipoalbuminemia.
  • Clásicamente se define por la presencia de proteinuria masiva (> 3,5 g/24h/1,73 m2 o > 40mg/m2/hora en orina recolectada durante 24 hs o índice proteinuria/creatininuria > 2 en muestra aislada de orina) e hipoalbuminemia (< 2,5 mg/dl)

    4. Hiperlipidemia: Colesterol total > 200 mg/dl y triglicéridos > 200 mg/dl.
  • Primarias: Enfermedad de cambios mínimos (mas en niños), Glomeruloesclerosis focal y segmentaria (GS F y S), Glomerulopatia membranosa (mas en adultos), Glomerulonefritis membranoproliferativa
    Secundarias: Diabetes mellitus, amiloidosis, Crioglobulinemia (infección crónica por hepatitis viral B y C).
    Farmacos: AINE’s, sales de oro, mercurio, litio, bismuto, plata, penicilamina, probenecid, captopril, pamidronato, rifampicina, heroína.
    Enfermedades autoinmunes: LES, enfermedad mixta del tejido conectivo, dermatomiositis, AR, síndrome de Sjögren, púrpura de Henoch-Schönlein, vasculitis sistémicas.

    CUCI, sarcoidosis, amiloidosis, reflujo vesicouretral, necrosis papilar, obesidad mórbida, nefropatía cróica del injerto renal.
  • el endotelio con fenestraciones (F), membrana basal glomerular (MBG) y epitelio visceral, formado por los podocitos (P) que dejan ver el diafragma (D) entre los pedicelos.
  • Los edemas son blandos, inicialmente posturales en tejidos laxos (periorbitario, sacro, miembros inferiores y escroto o vulva) pero puede progresar a anasarca con hidrotórax y ascitis, que puede manifestarse por dolor abdominal, hernia, prolapso rectal, disnea, etc. Cuando la hipoalbuminemia es grave sobrevienen signos de hipovolemia como taquicardia, hipotensión arterial, oliguria y por último shock hipovolémico. La presencia de fiebre obliga a descartar infección agregada (neumonía, peritonitis primaria, erisipela) debido a la inmunodepresión que presentan estos pacientes. La hipertensión arterial es infrecuente en la forma a cambios mínimos, por lo que su presencia debe orientar hacia otros tipos histológicos.
