  1. 1. Beacon Media 2014 – 2015 Risk Assessment Form Production title Mouse Date 10th February, 2016 Task Risk Assessment Prior to Main Filming Task Location Car Yard – South Croydon Personnel Candice Russell Name Role Bhavna Dougah Sweetah Brandon Webster Logan Robert Russell Car Yard Owner Potential hazard Risk rating (0:no risk, 5:high risk) How can risk be managed and who is responsible? Potholes in the narrow road outside the car yard entrance 3 Croydon council is responsible for the repair of the narrow road, as there are business addresses, registered along this strip. Request to the council to rectify this risk has been duly submitted. All actors will be shown the potential hazard so that they can avoid any accidents Parked vehicles outside of the yard and movement of vehicles 3 The parked vehicles are parked at their owner’s risk. As the road is narrow and poorly constructed, the majority of moving vehicles tend to move very slowly, approximately 5mph. All actors will be notified of the possibility of moving vehicles. Car parts on the floor in the garages and yard 5 I will be responsible for the safety of the actors and I will brief and show them the potential hazards, before filming. Any real obstruction will be removed in order to comply with health and safety issues. Sharp tools on work benches 5 I am responsible for the safety of my actors and again I will brief and show them any potential hazards. They will not be touching any of the tools, but should they feel uncomfortable passing these during filming then the most hazardous items will be removed to protect my actors’ safety. Vehicles parked in the yard 2 These vehicles are part of the scripted props. I will be responsible for any damaged caused and actors’ will be briefed accordingly. Weather 3 The weather may interrupt the alleyway scene, but not the car yard scene. If the weather is hazardous on the day of filming, I will reschedule to an appropriate time. However, I will endeavour to check the weather forecast leading up to the shoot. Dog walkers/wildlife 2 Actors’ will be made aware of possible dog walkers and that some dogs, may not be on a lead and I will bring to their attention that there have been sightings of foxes around.
