The complications of diabetes mellitus are numerous and multisystemic including the musculoskeletal system. The long term metabolic consequences of diabetes mellitus stay behind Several rheumatic conditions. Higher levels of diabetic complications is due to poor glycemic control. The incidence and prevalence of diabetes mellitus is rising. About 50% of people with diabetes mellitus are unaware of their condition. Approximately 25% of all patients with diabetes undergoing surgery are undiagnosed on admission to hospital. Patients with diabetes have a higher risk of cardiovascular insult and a higher perioperative risk. Surgeons and anaesthetists should be familiar with the risks of the diabetes, surgery and anesthesia. In emergency situations or non-elective cases insulin, glucose and potassium infusions (blood glucose control + rehydration) before surgery Prone to post operative complications, infection, wound care and bone healing. Pharmacotherapy, diet, regular exercises and sensible physiotherapy programmes should be the cornerstone of diabetes management.

Diabetic foot ulcers are a common and much feared complication of diabetes, with recent studies suggesting that the lifetime risk of developing foot ulcer in diabetic patients may be as high as 25% (Singh et al.,2005). Up to 50% of older patients with type 2 diabetes have one or more risk factors for foot ulceration. A list of the principal risk factors that might result in foot ulcer development are demonstrated ( Abbott et al ., 2002.)

The presence of symptoms and/or signs of peripheral nerve dysfunction in people with diabetes after exclusion of other causes is called diabetic peripheral neuropathy. The diagnosis is principally a clinical one. Patients with type 1 diabetes for 5 or more years and all patients with type 2 diabetes should be assessed annually.Treatment goals include good glycemic control,symptomatic treatment and halt progressive nerve damage.

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol that is rapidly distributed in the body and brain. Ethanol alters many neurochemical systems and has rewarding and addictive properties. It is the oldest recreational drug and likely contributes to more morbidity, mortality, and public health costs than all illicit drugs combined. The 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) integrates alcohol abuse and alcohol dependence into a single disorder called alcohol use disorder (AUD), with mild, moderate, and severe subclassifications (American Psychiatric Association, 2013). In the DSM-5, all types of substance abuse and dependence have been combined into a single substance use disorder (SUD) on a continuum from mild to severe. A diagnosis of AUD requires that at least two of the 11 DSM-5 behaviors be present within a 12-month period (mild AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria). The four main behavioral effects of AUD are impaired control over drinking, negative social consequences, risky use, and altered physiological effects (tolerance, withdrawal). This chapter presents an overview of the prevalence and harmful consequences of AUD in the U.S., the systemic nature of the disease, neurocircuitry and stages of AUD, comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and pharmacotherapies for AUD.
