Gordon Lyster by email to acfoawards@eventsandpublishing.com. Do not include names
of people who are making their own bookings. If you don’t yet know all of your guests
at the time of booking, please keep a copy of this form and send before Friday 22nd
October.

and company name(s). This form will be used to compile the guest list available on the
