                                        Health Access Plans provide limited benefits and all covered services are subject to calendar-year maximums. These are not major medical health plans and are not
                                        replacements for them. The amount of benefits depends upon the plan selected, and the premium will vary with the amount of benefits. Read all coverage documents
                                        carefully upon receipt. For a complete listing of benefits, limitations and exclusions, please refer to your coverage documents. In AL and WI, individual medical
                                        coverage cannot be marketed through the workplace of an employer with 2-50 full-time employees.

What’s the difference between our three limited-benefit plans?
        • PLAN A — No hospital benefits, calendar year (vs. lifetime) maximums, no pre-existing condition limitation
        • PLAN B and PLAN C — Coverage for pre-existing conditions after you have been continuously insured for 12 months,
          hospital benefits, a $1 million lifetime maximum; Plan C has higher calendar year maximums than Plan B.

    The rates for these limited-benefit plans are valid only for policies issued with effective dates September 1, 2009, and later. Rates quoted more than 30 days
    in advance of the requested effective date are subject to change and are not guaranteed. Issuance of coverage and rates are subject to approval. Rates are
    based on primary applicant’s age as of the effective date of the policy. This is not an insurance contract. Only the actual contract provisions apply. Final rates
    may vary. Employers cannot contribute to individual premiums. In certain states, membership in Health Advocates Alliance is required in order to buy
    this health insurance. Fees paid for membership in Health Advocates Alliance are used for benefits, marketing, distribution and administrative expenses.
    Assurant Health also may realize some benefit from these fees.

It’s important to know what’s not covered through your plan. Note that no benefits are provided for the following,
except where state mandates apply:

And, Health Access gives you just that — access — access to the health care system, access to doctors and other providers, and access to discounts. Here’s what Health Access offers: Affordability Choose from three modestly priced plans to find the one that fits your budget and lifestyle. Usefulness These plans cover services you’ll actually use, such as doctors’ office visits, prescriptions, preventive care and immunizations. Plus, should you need them, you’ll also have hospital benefits. Value You’ll have first-dollar benefits, copays and no high deductible. And, we’ve partnered with several services to help you squeeze extra value out of your plan and maximize any limits. Accessibility From zero medical questions on one of the plans, to only a few on others, it’s easy to qualify for all Health Access plans.
