sufrir reacciones transfusionales graves e infecciones transmitidas.

19. http://www.who.int/bloodsafety/clinical_use/en/
It is necessary to reduce the unnecessary transfusions. This can be achieved
through the appropriate clinical use of blood, avoiding the needs for
transfusion and use of alternatives to transfusion.
The commitment of the health authorities, health care providers and
clinicians are important in prevention, early diagnosis and treatment of
diseases/conditions that could lead to the need for blood transfusion.
Blood transfusion is an essential part of modern health care. Used correctly, it can save life
and improve health. However, as with any therapeutic intervention, it may result in acute or
delayed complications and carries the risk of transmission of infectious agents.

(3) delivers blood in a timely fashion such that patient health and preparedness are
not unduly compromised.

Aims
Main Tasks
Key messages
The key messages of PBM are:
▪▪ Anaemia and/or bleeding is detrimental for patients
▪▪ Transfusions carry a much bigger hazard ratio than previously realized
▪▪ In hemodynamically stable patients, transfusion should not be the default
response to anaemia and/or bleeding
▪▪ PBM has the potential to prevent and treat anaemia and blood loss, thus
avoids transfusion and therefore improves patient safety and outcomes
Introducción al Patient Blood Management
91. Aims
Main Tasks
Key messages
▪▪ Effective PBM can deliver improved patient outcomes through the conservation
and management of patient’s own blood and better management of co-morbidities.
▪▪ Effective PBM optimises appropriate use of the limited donor blood supply.
▪▪ Effective PBM minimises the cost associated with the procurement and delivery of
blood and bloodproducts.

PATIENT BLOOD MANAGEMENT Introducción al Patient Blood Management
100. DETECCIÓN Y TRATAMIENTO DE LA ANEMIA Primer Pilar del Patient Blood Management
"Whenever clinically feasible, patients undergoing elective surgery with a high
risk of severe postoperative anaemia should have their haemoglobin level and
iron status tested, preferably at least 28 days before the surgical procedure.
For patients >60 yr old, vitamin B12 and folic acid should also be measured".
101. Primer Pilar del Patient Blood ManagementDETECCIÓN Y TRATAMIENTO DE LA ANEMIA
102. Primer Pilar del Patient Blood Management
Recommendations
• Healthcare pathways should be structured to ensure anaemia screening and correction before
surgery (Grade 2B).
• Patients should be counselled about the relationship between anaemia, morbidity and mortality, and
should be given the opportunity to defer non-urgent surgery until anaemia is investigated and treated
(Grade 1C).
• To avoid causing unnecessary delay to patients, anaemia screening should take place when referral
for surgery is first made, in order to allow investigation and correction if appropriate (Grade 1C).
• Where surgery is urgent, whatever time is available before operation should still be used for
anaemia investigation and treatment initiation (Grade 1C).
DETECCIÓN Y TRATAMIENTO DE LA ANEMIA

135. First Recommendation
The AABB recommends a restrictive RBC transfusion threshold
in which the transfusion is not indicated until the Hb level is
7g/dL for hospitalized adult patients who are hemodynamically
stable, including critically ill patients, rather than a liberal
threshold when the Hb level is 10g/dL (strong recommendation,
moderate quality evidence).
For patients undergoing orthopedic surgery or cardiac surgery
and those with preexisting cardiovascular disease, the AABB
recommends a restrictive RBC transfusion threshold (Hb level
of 8g/dL) (strong recommendation, moderate quality evidence).
The restrictive Hb transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, but
RCT evidence is not available for all patient categories.
