The interview took place in a private setting, in the participant's home that was free of distractions
and noise. It is required to obtain an interview in a private setting for patient confidentiality and
privacy. It took about an hour to for the interview because of the required information, family health

Gloria is a 74 year old lady who lives at the residential care home at which I am currently on
placement Gloria is under the Adults with Incapacity Scotland Act 2000 due to a diagnosis of
Dementia. She is mobile with the aid of a Zimmer frame and is still as independent as her health

Gloria has a strong desire for independence and in health care this is fully supported and promoted.
As carers we should support the resident/patient to help themselves as much as possible. Using
verbal prompts I guided Gloria round to the toilet and then asked her if she required any help to
remove her clothing, which she declined. I understand that everyone has the right to decline
treatment or care of any kind. I stepped back slightly acknowledging her decision and allowed to
her continue in the removal of her clothes. I placed by hand behind her back while she did this
just to support her in case she became unsteady. Once Gloria was seated on the toilet I asked her
if she would like some privacy to which she agreed. I placed the emergency pull cord in her hand
and explained to use it if she needed any help or when she needed us to come back in. The
supervising healthcare assistant and I stepped out of the room and closed the door over. Whilst
Gloria was in the bathroom I used this as my opportunity to put on Gloves and an apron. Disposable

This assignment is a reflective account on communicating with a patient who cannot communicate
verbally. To remain confidential I will call the patient, Patient A. I'm going to discuss the importance
of non–verbal communication within a healthcare setting. Patient A was a 63 year old lady suffering
from MND which resulted in her losing her speech.
This was my first shift back from having a few days off and I returned to work on a night shift.
Patient A was admitted to the hospice that day. She was admitted for general deterioration and she
had tried to maintain her independence up until breaking point. It was handed over she has aphasia.

I explained why we were unable to swap mattresses, due to it being late at night and one not being
available but offered her a recliner chair instead. She accepted the offer, and appeared to be happy
with this.
Myself and a colleague transferred Patient A into the recliner, she was then able to get some sleep
that night in the recliner chair. This information was recorded on the patient's notes and handed over
to the day staff on the next morning.
I will now discuss how I was thinking and feeling during this event. I had been working at the
hospice for over a year and had looked after patients with MND before. I had an idea of how it can
affect communicating with the patient. I was apprehensive about approaching Patient A as I didn't
know what difficulties I may find whilst trying to communicate with her. She was able to
communicate very well using the pen and writing down, and using hand gestures. I learnt that by
speaking clearly with easy to answer questions we were able to communicate with each other
effectively. Afterwards I felt pleased that were able to determine the problem and find a solution for
her, despite the communication barriers.
Evaluation
I feel that the communicating between Patient A and myself worked well during this event. I noticed
that she was uncomfortable by her body language and I picked up quickly which method was best to

It was only when I redefined the practice in layman's terms that I realised I practice reflection in
my every day life. I tend towards analysis and evaluation on a daily basis, whether it is in
analysing how I talk to someone, how my sessions with my scout group work or how I use my
reading to increase my vocabulary.
The idea of reflective practice in an academic context however is new to me, and I did feel lost at
first. Now I can see how reflecting on my own learning and experiences has helped me to see
where I have improved week on week and I think that as a result, my confidence has improved and
I am able to contribute more in class, I have more confidence in my ability to teach and I feel that

To enable the children's voices to be heard, I explored various tools to engage the children from
the Mosaic approach (Clark & Moss 2001). I disregarded using questionnaires as the
children were unable to read and write so this would have been inappropriate. I disregarded role
play as I didn't feel I would get the responses I needed as directly, and I decided not to use tours as
not all the children were confident communicators and they may have found this difficult. I wanted
to be as inclusive as possible and ensure the technique was age and development appropriate.
Therefore I chose to gather documentation by enabling the children to use cameras to take photos
outdoors of what they liked and didn't like. To ensure confidentiality of the photos taken I kept them
in my private locked filing cabinet that only myself and my manager had access to. As my research
required the children's perceptions this technique ensured it was child led as were the later discussion
where we came together in a quiet area to talk about why they took their particular images. This
discussion enabled me to interpret what they were trying to say to me through the photos. As we
looked at the photos it prompted their memory of why they had taken the picture. It was an

My supervisor was happy. However, once I had undergone the whole experience of having a
retrograde appendix removed myself, I discovered that the pre–operative preparation and
post–operative care that was given to me, and I myself would have given something similar,
inadequate. Although my experience as an adult on an adult ward would have been slightly

communication, verbal and non–verbal, both of which are important in understanding and supporting

The communication barrier I have with AB is she is finding speaking to me other then yes, no

If AB is then not happy to communicate with me, I remind AB that I am there to support her and
will be here to talk to if she so chooses, as it is her choice. I also watch the type of body language
that AB is using to help work out how she is feeling about the situation. AB does also have picture
cards to help her to commute, but choose not to use them. I can check that she has understood what
I have said I by observing her reactions and short answers AB give.
By always reminding and allowing AB to make her own choices and respecting her wishes to
communicate with me or not I am working in a way to prevent discrimination.
Reflecting on how I communicated with AB, I believe that I worked to the best of my knowledge at
the time, but by doing some research I could find other ways of opening communication lines and I
need to watch out more for body language signs, about the way AB is feeling. Having notes from
other people about what they have tried and how they have worked or not, helps me know what
things that might help get the lines of communication open and what things to research. There are
many people that are working with or have a relationship AB, that I could ask for support and advise
like: speech and language therapies, family members, friends, GP, district nurse, other support works.

At this point, we were some of the most successful farmers in the area. We were making enough
money that my family decided we could buy a new piece of property. A piece of property that

INTRODUCTION For this assignment I will be writing a reflective account which will identify a
significant episode of care in which I had been involved with, by identifying the pathophysiology
and the disease process for the chosen patient; this will be presented by giving a brief outline of the
psychosocial influences of the illness for the patient and others who may have been involved with

The blood pressure and pulse was taken and the reading of 149/90mmHg which is showing
hypertension, and 79 pulse which both would be regarded holistically as high for this patient,
this may be due to the stress being caused. The ECG is a simple and effective electric recording of
the heart rhythm which was taken with the patient still very anxious. I decided that this was
becoming very traumatic for the patient and felt that I needed to hold the patients hand by using a
touch technique. The ECG was performed and did provide significant results for the doctor. The
actual test of taking an ECG reading did provide some difficulties due to the patient's pain and also
the psychological pain as the patient was under extreme stress and worry at that time, as referred to
before that. "Angina is the pain or discomfort experienced. Angina may also be brought on by
emotional upset". The GTN was soon administered and relieved the patient's pain near enough
automatically. The patient was assessed at 5 minute intervals and later 15 minute intervals to make
sure that the observations were at a normal standard with the blood pressure being previously high.
The Patient was also assessed throughout the night. The blood pressure did reduce to a normal rate.

Reflective Account On Communication!
Whilst on shift one day I needed to ask my key resident Mr H what clothes he wanted to wear for
that day. Mr H is very hard of hearing but refuses to wear a hearing aid as he states they make his
ears sore. Mr H has no speech difficulties. Before I proceed with Mr H's care I check his care plan
where it is documented how he likes to be communicated with. Any changes to Mr H's
communication ability should be reported and documented straight onto a daily diary sheet and
into his careplan where other staff members are able to see the changes noted. I then knock loudly
on Mr H's door so that he is aware of my prescence and await his reply, he called for me to 'come in'.

I could tell if Mr H had understood or heard me correctly by his verbal responses and his facial
expressions. Whilst communicating with Mr H I reviewed his hearing ability by making a mental
note of the times he hadn't heard me correctly and compared this with the previous day, at present

section of Mr H's care plan where others could see there were no changes at present. A short while
