1. UPakWeShip’s guide to completing the documentation
required by Customs for importing your belongings into the
USA.
2. There are 3 forms that need to be completed.
The first is a 3299 which we explain on the next few pages how to complete.
The second is what is called a supplemental declaration that asks further
questions not covered in the 3299.
The third form is simply a power of attorney specifically designed to authorize our
customs broker to act on your behalf to customs clear your shipment.
Have these forms printed and with you so that you can complete them as we go
through them question by question.
4. 1/ Full name….last, first and middle
2/ Your date of Birth, month first
3/ Date of your arrival in the USA. (remember your US customs form 3299 is not
valid until your arrival in the USA so plan on being here before your shipment
arrives)
4/ Your US address, you can put a temporary address in here if that’s all you have
5/ Enter the name of the airport where you, or where you will clear customs
yourself in the USA
6/ Name the airline and flight number you will be traveling to the USA on.
7/ List other family members traveling with you.
5. 8/ Leave blank.
Part 11
9/ most people should be marking was with an X Name the Country where you
were living and how long you have been living there. For duty free entry you need
to have lived overseas for at least 1 year.
9c/ X if you are a returning resident / non resident / emigrating or a visitor.
10/ Most US residents should mark an X in A (1) and B.
Most non residents would X (1)

4/ Enter the country who issued your passport and your passport number.
5/ Enter social security number. If you don’t have one you need to apply at social
services in all major cities and then enter the number.
6/ Enter resident alien number if you have one. Not applicable to US residents.
7/ Enter US address
8/ Enter address where you lived overseas.
9/ Enter reason for moving.
10/ Enter your employers name and address or write self employed or retired for
example.
10. 11/ Enter position with company if applicable
12/ Enter the length of your contract if applicable
13/ Enter nature of the business you will be working for.
14/ Enter the name of the official who can verify the above is correct together with
phone and email address if possible.
15/ Leave blank
16/ leave blank
17/ Please sign, date and X as the importer.
Finished!
Now you just have the Power Of Attorney to complete, see next page
11. This power of attorney authorizes our customs broker to act on your behalf to
clear your shipment through customs.

12. Now please email these forms as PDF’s to your coordinator.
If you are a US citizen we also need a copy of your passport and any other family
members mentioned on the 3299 you completed.
If you are a resident, we need a copy of your passport and green card and any
other family members mentioned on the 3299 you completed.
If you are a Non resident we need a copy of your passport, visa and I94 card for
yourself and any other family members mentioned on the 3299 you completed.
