It can be inferred that: 1) In the Capitol, looking younger and thinner through surgery is desirable and valued. In District 12, looking older is more common and is seen as an achievement due to many people dying early. 2) In the Capitol, being plump signifies success. In District 12, being plump signifies having enough food to eat, which is envied by most who are scraping by. 3) Overall, the passage shows that appearances and signs of wealth/status are valued very differently between the lavish Capitol versus the impoverished District 12.
