consider all pupils who
have additional
support needs and
identify those who may
need planning and
support for transition.

With parental agreement, the Transition Plan can be passed to post school providers. S2 or S3 schools consider all pupils who have additional support needs and identify those who may need planning and support for transition. Pupil and parent consulted. Referral to Transition Team for initial assessment. Request for advice from different agencies. Referral to other SW team - not for Transition Team No input from social work required. Pupil leaves school Transfer of information 6 months before pupil leaves. Transition Team will support process of move to post-school. Review provision after 3 months. Allocated to Transition Team. Assessment of need. Explore choices and post school options. Work with YP, family and involved agencies. Transition 1 Meeting Organised by school’s ASL Team Leader for all pupils with additional support needs and enhanced transitional planning required. Collect views of pupil and parents. Identify agencies likely to be involved in transition and contact them. Discuss where a referral to the Transition Team should be undertaken. Use Transition Toolkit.
