  5. 5. More importantly, could you explain how we use our Care Delivery System every day in our work? 5
  6. 6. So, we’re here to help! • Patient & Family Centered Care (PFCC)! • PFCC is our Nursing Care Delivery System. It’s based on the fact that patients and their families have the right to know everything you know about the patient’s care and condition. • It’s a partnership to ensure we are meeting the specific and individualized needs of each patient and their families. 6
  7. 7. • PFCC is so important that it has its own Institute! • It is clear that when providers, patients and families work together great things happen: – – – – 7 Quality goes ↑ Safety goes↑ Provider & Patient Satisfaction goes ↑ Costs go ↓

We must work hard to think about everything from the patient/family perspective! • If you or someone you love were a patient wouldn’t you want to know.. . – What those X-rays mean? – What are they typing into that computer? – What is that medication they are giving me, or my loved one, and why? – Why are they talking about me as if I’m not here? 8
  9. 9. What, exactly IS PFCC? There are 5 Core Concepts of PFCC: • Dignity & Respect • Information Sharing • Collaboration • Participation • Leadership & Empowerment 9
  10. 10. We use PFCC every day! • We already incorporate PFCC into our nursing practice (though we can always do it better!) • Here are some examples of PFCC at WakeMed: 1 0
  11. 11. Dignity & Respect WakeMed nurses listen to and honor patient and family perspectives and choices.   12. 12. Information Sharing WakeMed nurses share complete and unbiased information with patients and families in ways that are affirming and useful to them.   • We “WakeMeditized” the Institute’s pinwheel logo as well as the core concepts of Patient &amp; Family Centered Care. In order to effectively incorporate our care delivery system at WakeMed, we need to have all the PFCC core concepts working together – just like a pinwheel needs all of its parts to work.
  • Notice this refers to All members of the WakeMed team. We are beginning the rollout of PFCC with Nursing; it will then be rolled out to the entire health system. Nurses will serve as role models for PFCC, helping other employees put themselves in our patients’ and families’ shoes.
