1. Sono circa 11 anni da quando è stata introdotta la HAART, ossia la Terapia
Antiretrovirale Altamente Attiva.
L’impressionante riduzione della viremia sperimentata dai pazienti trattati con la
HAART ci ha inizialmente portati a sperare che l’eradicazione completa
dell’infezione potesse essere raggiunta con 2 o 3 anni di trattamento continuo.
Invece, a oggi neanche una persona è stata curata, cioè è guarita, e le speranze
iniziali per una cura hanno lasciato spazio a un pessimismo così profondo da
rendere l’eradicazione quasi un tabù.
Questa presentazione ha lo scopo di osservare obiettivamente i progressi che sono
stati realizzati e i passi che sono ancora da fare per curare l’infezione da HIV.

Good morning…………. I would like to first thank the organizers for the kind
invitation to speak……….. It has now been 11 years since the introduction of Highly
Active Antiretroviral Therapy, or HAART, was introduced. The dramatic reductions
in viremia experienced by patients on HAART led initially to the hope that
eradication of the infection could be achieved with 2-3 years of continuous
treatment. However, to date not a single person has been cured, and the initial
hopes for a cure have given way to a pessimism so profound that eradication is
almost a taboo subject. What I would like to do today is to take an objective look at
the steps we need to take in order to cure HIV infection and review the progress
that has been made.

In my opinion, there are three steps. First we need to stop the virus from
replicating. This step is essential because it takes away HIV’s best weapon, its
ability to evolve. It is hard to see how we can succeed unless we can stop
replication. The approach is to use HAART to block specific steps in virus
replication. Second, we need to identify all of the stable reservoirs in which non-
replicating forms of the virus persist in the body. Finally, we need to find a way to
eliminate each of these reservoirs. So I will review progress towards each of these
goals, concentrating in the first one. I’ll will begin with some general background.

farmaci, i livelli virali nel plasma crollano dalla situazione di equilibrio pre terapia

When patients start on a standard 3 drug HAART, plasma virus levels fall from the
pre therapy steady state level all the way down to below the limit of detection of
clinical assays – which is 50 molecules of HIV RNA/ ml of plasma. This rapid decay
reflects that fact that the infected cell populations that produce most of the plasma
virus have very short half lives, on the order of days. If we stop new rounds of
infection with HAART, the previously infected cells die off and viremia decays. It
was originally hypothesized that this decay would continue, allowing complete
eventual eradication of the infection.

unique mechanism of persistence that exploits …

the normal physiology of CD4+ T cells. Most of the CD4 cells in the body are in a
profoundly quiescent state. These resting cells include naïve T cells which have not
yet responded to any foreign antigen as well as memory cells that have previously
participated in an immune response. These cells circulate throughout the tissues
awaiting encounter a foreign antigen that they can recognize.

When one of these cells encounters an antigen, it becomes activated and divides,
ultimately generating lots of activated effector cells with the same specificity. At the
conclusion of the immune response, many of these activated cells die, but some
survive and return to a resting state as memory cells. These cells persist for long
periods of time, decades in fact, allowing future responses to the same antigen.

In HIV infection, the virus replicates mainly in the activated cells and tends to kill

However, one rare occasions some of the activated T cells can become infected as
they are in the process of returning to a resting state. This results in a stably
integrated form of the viral genome in a long lived memory T cell. And what is
particularly interesting is that as the cell undergoes this profound change from an
activated to a resting state, HIV gene expression is turned off.

The end result is a stably integrated but silent or latent form of the viral genome in a
long lived memory T cell. This is a perfect mechanism for viral persistence - it allows
the virus to persist essentially as pure information, unaffected by immune responses
or antiretroviral drugs. If the cell becomes activated in the future, it can begin to
produce virus again.

11. Nel 1995 il gruppo di Siliciano ha sviluppato delle metodiche di analisi per
individuare queste cellule infette latenti nei pazienti. È emerso che queste cellule
sono presenti in ogni persona infettata dall’HIV, ma rare. Solo una cellula CD4+ T
su un milione è portatrice di questa forma latente di HIV.
Il problema, è che queste cellule sopravvivono nei pazienti in trattamento con
HAART, come mostrato da Tony Fauci, Doug Richmond e dal loro gruppo nel 1997.

In 1995, our group developed assays to detect these latently infected cells in
patients. It turns out that they present in everybody with HIV infection, but are rare.
Only one in a million resting CD4+ T cells carries this form of latent HIV. The
PROBLEM is that these cells persist in patients on HAART as was shown by Tony
Fauci, Doug Richmond and by our group in 1997. We have measured the decay
rate of this latent reservoir in patients who have are doing well on HAART. As you
can see, the decay is extremely slow. Note that the time scale is years, not days.
Here are the actual data, representing measurements a large number of patients
who have suppression of viremia to below 50 copies/ml for as long as 7 yrs. At this
rate of decay, it would take 70 yrs to clear this reservoir/

So, we know that HAART can reduce levels of FREE virus in the plasma to below
the limit of detection. We also know that the virus can persist in a stable latent
reservoir in resting CD4+ T cells. The next important point is that all patients on
HAART are actually viremic. That is, with especially sensitive methods, it is possible
to detect free virus particles in the plasma of patients on HAART whose viral loads
are below the limit of detection. We call this residual viremia.

>>In other words, what HAART really does is to drop the level of virus in the
blood down to a new plateau level that is a little bit below the limit of
detection of current clinical assays. This was first shown by Roger
Pomeranz and confirmed by Sara Palmer and colleagues using an assay
that can detect a single virus particle. The explanation that is commonly
given for this residual viremia is that is reflects ongoing cycles of viral
replication that continue but at a lower level due to the effects of the drugs.
This is a disturbing scenario because ongoing replication in the presence of
drugs can lead to the evolution of resistance and treatment failure.
Fortunately, there is an alternative hypothesis.

We believe that in the optimal situation, HAART actually stops ongoing viral
replication and that the residual viremia is due to the release of virus from
stable reservoirs of long lived cells infected prior to the initiation of therapy.
For example, the memory T cells that carry latent HIV. You can imagine that
every day a small number of these cells become activated and produce
virus. That virus cannot infect new cells because of the suppressive effects
of the drugs. Nevertheless, the virus particles released from this reservoir
can be detected as HIV RNA in the plasma. This is a much more optimistic
hypotheis because is says that HAART can really stop the virus from
replicating. This hypothesis makes several testable predictions.

So here is a phylogenetic tree displaying the sequences of virus in the latent
reservoir of a single patient on HAART. I will add the sequences we deteced in the
plasma using extremely senstive sequencing methods. You can see that these two
sets of sequences are phylogenetically intermingled and in some cases identical,
consistent with the idea that the residual viremia is derived in part by release of
virus from latently infected cells that become activated.

A second prediction is that the residual viremia should continue without viral
evolution. We should not see progressive evolutionary change in the
residual plasma virus or the eventual appearance of resistance. This is
because the complete cycles of replication necessary for evolutionary
change are not occurring.

So as you can see, the plasma sequences, the colored triangles, have not diverged

This absence of new resistance mutations is also true for the virus present in
the plasma during blips, the transient elevations in viremia that many
patients on HAART experience. Blips are likely to be a reflection of the fact
that everybody on HAART is viremic, due to release from stable reservoirs,
and that there will inevitably be biological and statistical fluctuations in the
level of residual viremia which can be occasionally captured as a blip,(11:46
read)(10:55)

22. Così, in collaborazione con Frank Maldarelli e John Coffin al NCI e John Mellors
all’università di Pittsburg, abbiamo condotto uno studio di intensificazione.
In questa slide, vediamo i risultati di un sottogruppo di pazienti trattati con un regime
terapeutico basato su efavirenz (SUSTIVA®) e una forma potenziata del potente inibitore
della proteasi atazanavir (REYATAZ®), aggiunto per 8 settimane.
Prima dell’intensificazione, le cariche virali in questi pazienti erano mediamente tra 1 e 10
copie/ml, ben al di sotto del limite di identificabilità degli esami clinici, ma prontamente
misurabili con un esame capace di individuare una singola copia sviluppato da Palmer e
coll..
Possiamo osservare che, quando la forma potenziata di atazanavir veniva aggiunta non
accadeva sostanzialmente nulla.
In pratica, l’intensificazione non aveva effetti misurabili sulla viremia residua. E ciò è stato
verificato in tutti e 15 i pazienti studiati, con tre diversi farmaci di intensificazione.

So in collaboration with Frank Maldarelli and John Coffin at the NCI and John Mellors at the
University of Pittsburg, we have carried out an intensification study. Here are the results
from a subset of patients who were on efavirenz based regimen and had a boosted form of
the potent protease inhibitor atazanavir added for 8 weeks. Prior to intensification, the viral
loads in these patients averaged between 1 and 10 copies/ml, well below the limit of
detection of the clinical assay but readily measurable with a single copy assay developed
by Palmer and colleagues. This is what happens with boosted atazanavir is added.
Basically NOTHING happens. And here are the levels after intensification. The result is that
intensification has no measurable effect on residual viremia. This has been the case in all
15 patients we have studied and with 3 different intensification drugs.

This result strongly suggests that most of the residual viremia results from
the release of virus from stable reservoirs. We cannot rule out some very
minor contribution from ongoing replication to the residual viremia, but
clearly release from stable reservoirs is the major problem. Before I turn to
the problems of reservoirs, I want to describe some new research into the
basic pharmacology of HIV drugs provides an a very compelling explanation
for how HAART can actually stop all ongoing viral replication.

This is a hypothetical dose response curve for an anti-HIV drug. The fraction of infected
events that remain uninhibited decreases as the concentration of drug increases. We can
describe this curve with a simple mathematical equation, I realize it is a little early in the
morning for equations, but this one simply says that the amount of inhibition depends on
three variables, the drug dose D, the IC50, a standard pharmacologic measure of drug
potency, and the exponent m which is a measure of the steepness or slope of the dose
response curve. This slope parameter is analogous to the Hill coefficient, a measure of
cooperativity in the binding of multiple drug molecules to a multivalent receptor. Since the
RT and protease inhibitors bind to a single site on the target enzyme, this slope parameter
has generally been ignored or assumed to be equal to 1 (which makes it drop out of the
equation). If the slope is greater than 1, the curve is simply a bit steeper.

Rather, we should use a log scale, so that we can see the fraction of successful
infection events decrease from 100 to 10 to 1% and even much lower. So if we plot
the SAME dose response curves from the previous slide on this kind of a plot, the
result is very surprising. It turns out that the curves for drugs with different slopes
actually DIVERGE DRAMATICALLY in the clinically relevant concentration range
such that drugs with a higher slope value cause a much higher level of inhibition –
by orders of magnitude. Therefore, we decided to measure the slope parameter for
antiretroviral drugs.

copia rilevante per virus. All’opposto, molte copie dell’HIV proteasi sono simultaneamente coinvolte
nella maturazione di una singola particella virale per una forma unica di cooperatività intermolecolare
che è evidente in una curva dose-risposta ripida.
Ora, si tratta di comprendere quale differenza questo comporti nel determinare l’efficacia dei singoli
farmaci.

We have used this assay to measure the slope parameter for five different classes of antiretroviral
drugs, expecting that the slope would be one for these drugs. For the nucleaoside analoge RT
inhibitors, the slope was 1 in every case. However, for the non-nucleoside RT inhibitors, the slope
was about 1.7. For the protease inhibitors, the slope ranged from 2 up to 4.5. For the entry inhibitors
like T20, the slope was 1.7. and for five structurally diverse integrase inhibitors, the slope was exactly
1. Mechanistically, this begins to make sense when we consider that for the two drug classes that
show a slope of 1, the nucleoside analogue RT inhbitors and the integrase inhibitors, the target is an
enzyme nuleic acid complex for which there is only one relevant copy per virus. In contrast, many
copies of HIV protease are simultaneously involved in the maturation of a single virus particle
allowing for a unique form of intermolecular cooperativity that is manifest in a steep dose response
curve. Now what difference does this make in how well the drugs actually work.

To answer this question, Lin Shen, a graduate student in our laboratory, developed a
simple index called Instantaneous Inhibitory Potential which is simply the number of logs, or
10 fold reductions in single round of infection events caused by a drug at a clinically
relevant concentration. So for example, here are two drugs that would be judged equally
potent by the standard IC50 measure but with different slope values. The drug with a slope
value of 1 causes a 100 fold or 2 log decrease in infection events at Cmax, the peak
plasma concetration, and thus the IIP is = to 2 while a drug with a slope value of 3 causes a
1 million fold or six log reduction in infectivity and has an IIP of 6. Thus the IIP captures the
dramatic effect of the slope parameter in an index that has intuitive meaning – it is simply
the number of logs by which the drug inhibits a single round if infection.

So Lin has measured the IIP values for all of the current antiretroviral drugs. For the nucleoside
analogues, the IIIP values range from 1 to 4. For the non-nucleoside RT inhibitors, the values go as
high as 6, and for the protease inhibitors the IIP ranges from 2 up to 10. Lower values are seen for
the entry and integrase inhibitors. Perhaps the most striking finding is that some of the protease
inhibitors can produce a 10 log reduction in a single round of infection – that is a 10 billion fold
reduction. Now it is not so hard to understand how HAART can completely stop ongoing cycles of
replication. In fact, with this level of inhibition it would be suprising if HAART didn’t stop all ongoing
replication. This result also provides, at long last, an explanation for why the antiretroviral drug
regimens that have been proven to be the most effective in numerous clinical trials contain a NNRTI
or a PI. These are the classes that have this extraordinary ability to inhibit viral replication.
Now although HAART has the ability to completely block viral replication, we are left with the problem
of finding and eliminating viral reservoirs.

As I mentioned, sequences of the residual plasma virus are similar to those found in the latent reservoir.
However, at least in some patients, the story is more complex. This is only part of the phylogenetic tree for this
patient. Here is the rest of the tree. What is particularly interesting is that in this patient, most of the plasma
virus sequences are identical to one another. In other words, the same exact sequence has been isolated
repeatedly from the plasma in multiple independent samples, indicated by the different colors, over a 3 year
period. In about half of the pateints studied we have seen that the residual viremia is dominated by a small
number of viral clones. These clones do not show evidence of sequence evolution over time. Most
interestingly, these clones are not found in resting CD4+ T cells, suggesting that they come from another
source. We believe that this phenomenon reflects the rare infection of a progenitor cell, perhaps in the
monocyte macrophage lineage, that is able to proliferate after infection, thereby copying the viral genome
without error into progeny cells with differentiate and produce exactly the same virus for their normal lifespan.
The notion that this is a second major reservoir for HIV, perhaps in a cell with some capacity for self renewal, is
a disturbing one, and this phenomenon will clearly require more investigation.

What about efforts to eliminate viral reservoirs. Most of the intial attempts have focused on
agents that induce cellular activation and in the process turn on latent HIV. The problem
inherent in these approaches is that they are not selective for infected cells, but rather
activate all resting CD4 cells, leading to the production of proinflammatory cytokines in such
large amounts that unacceptable toxicity results. The ideal solution to this problem would
be to find agents that reactivate latent HIV without inducing global T cell activation. The
viruses produced could not infect additional cells if the patient remained on a supressive
HAART regimen and we would hope that the infected cells would die as a result of the virus
gene expression or immune system attack. Unfortunately we do not have such an agent
right now. But we can take hope from the fact that several groups have developed way to
generate latently infected cells in the test tube from primary T cells. This provides a
realistic model system which can be used to search for drugs that can target that latent
reservoir.

In our own studies, we take primary resting CD4+ T cells, transduce them with the
pro-survival gene bcl-2. We can then infect with a modified and relatively non-
cytopathic form of HIV and they will survive long enough to return to enter a latent
state of infection. We can then use these cells to screen compound libraries for
drugs that will reactivate latent HIV.

So to summarize, I would suggest that as a result of the efforts of literally thousands of
people in academia and in industry who participated in the development of HAART, the first
step has been accomplished. It would be a mistake to conclude that we are one third of the
way to finding a cure because the second and third steps are likely to be very difficult
although some progress has been made. Nevertheless, the fact that current HAART
regimens can halt ongoing cycles of replication is extremely significant – it takes away the
main weapon that HIV has against us, its ability to mutate. poses an important challenge for
all of us. It HAART can stop virus replication, treatment failure is not inevitable. This means
that if we can develop forms of the current HAART drugs that are sufficiently non-toxic to be
taken for life, and if we can make these drugs available to everyone who needs them, then
we can offer everyone who is currently living with HIV infection the change for a normal life.
